Objective To discuss the effect of health education to family members of patients with chronic heart failure. Methods 150 patients with family members as accompanies were divided into the observation group and the control group with 75 patients in each group. Health education was given to patients as well as their family members in the observation group, while health education was given only to the patients in the control group. The mastering of disease-related knowledge by family members of the observation group was observed after 15 days. The rehospitalization of the two groups by telephone follow-up was carried out six months after discharge. Results The mastering of disease-related knowledge by family members of the observation group was better compared with before health education. The rehospitalization rate of the observation group was 13.3%, which was lower than 44.0% of the control group. Conclusions Health education to family members of patients with chronic heart failure can increase treatment effect of patients, decrease disease relapse and lower rehospitalization rate.

OBJECTIVE To investigate the distribution of 2916 strains and analyze the antimicrobial susceptibility of major pathogens and to provide evidences for clinical therapy.METHODS The distribution, and the antimicrobial susceptibility of pathogens, which were collected and isolated from all of the clinical specimens from Jan 2007 to Oct 2008 in our hospital, were studied restrospectively.RESULTS Totally 2916 strains(except fungi) of pathogens were isolated from patients, among which 1857 strains were Gram-negative bacteria(63.7%) and 1056 strains were Gram-positive cocci(36.2%).The five common bacteria isolated from the specimens were Escherichia coli(14.3%),Klebsiella pneumoniae(11.8%),Pseudomonas aeruginosa(10.7%),Staphylococcus aureus(10.5%),and Acinetobacter baumanii(8.7%).The ESBLs producing E.coli and K.pneumoniae accounted for 31.4% and 33.7%,respectively;meticillin-resistant S.aureus(MRSA) accounted for 53.3% and meticillin-resistant coagulase-negative staphylococci(MRCNS) accounted for 49.6%;vancomycin-resistant S.aureus was not isolated Gram-negative bacilli had the lowest resistance to carbapenems, and then to piperacillin/tazobactam(TZP) and cefoperazone/sulbactam(CFS),showing multi-resistantce.Gram-positive cocci were more sensitive to vancomycin, rifampicin, nitrofurantoin than to other antibacterials.CONCLUSIONS The drug resistance of the isolated bacteria is common. It is very important to monitor the drug resistance of the bacteria regularly, for guiding the clinic use of antibiotics rationally, and infection control.

Objective Since there are significant variation of the dietary structure recent years in China,it is necessary to re-investigate the fatty acid composition of human breast milk for the presentation of the latest data of fatty acid composition in China. Methods Using a gas chromatography GC-2010,the composition of fatty acids was detected in the human colostrums and the mature breast milk(consecutively from postnatal day 1 to day 7 and from postnatal day 42)obtained from 62 healthy postpartum women in Shanghai and Chongqing, two big cities of China, from Jan to July,2008. Results The level of total fatty acid(TFA)tended to increase significantly from the colostrums to the mature breast milk. No significant difference in the level of TFA was found between two cities. The significantly higher monounsaturates(MUFA)level(44.06% vs. 33.85%,P < 0.01)and lower linoleic acid(LA,C18 : 2n-6)level(18.43% vs. 27.62%,P < 0.01)of the mature breast milk were observed in Chongqing women compared with those in Shanghai women. The docosahexenoic acid(DHA)level of the mature breast milk in Shanghai women was significantly higher than that in Chongqing women(0.41% vs. 0.29%,P < 0.01). There was no significant difference in the level of arachidonic acid(AA,C20 : 4n-6)between two cities. Conclusions The fatty acid composition in human breast milk tends to vary with the extension of the lactation. There is significant difference in the fatty acid compositions in human breast milk between Shanghai and Chongqing owing to different dietary habits in the different regions of China.

Objective In the present study, we investigated the effects of advanced oxidation protein products(AOPP) on reactive oxygen species(ROS) production in murine osteoblastic MC3T3-E1 cells by NADPH oxidase enzymes pathway. Methods Experiments were divided into three groups, including control group, rats albumin(RSA) group, and AOPP group. Different concentrations of AOPP were added to the osteoblastic MC3T3-E1 cells culture medium. The production of ROS in MC3T3-E1 cells was measured by the fluorescence intensity of intracellular fluoroprobe ( DCFD ) . In order to verify the effect of enzyme of the production of ROS, the specific inhibitors of corresponding enzymes were added in the MC3T3-E1 cells which were cultured in the medium with AOPP. Finally, western blot and immunofluorescence were used to observe the changes of NADPH oxidase enzymes subunits. Results Different concentrations of AOPP (50,100,200μg/ml) induced MC3T3-E1 cells to produce different amount of ROS. The higher concentrations of AOPP were added, the more ROS were produced. Furthermore,200μg/ml AOPP induced the maximum amount of ROS production(P<0. 05). Meanwhile, AOPP induced MC3T3-E1 cells to produce different amount of ROS with a time-dependent manner. The peak amount of ROS production in MC3T3-E1 cells was observed in 3h when AOPP were added (P<0. 05). In addition, when specific inhibitors of corresponding enzymes were added in the MC3T3-E1 cells, the production of ROS were significantly suppressed by C-SOD, DPI, and apocynin(P<0. 05). On the other hand, AOPP can up-regulate the expression of Nox4 protein of the MC3T3-E1 cells, which is one of the subunits of NADPH oxidase enzymes. Meanwhile, AOPP can also induce the membrane migration of p47phox subunit. Conclusion AOPP induces osteoblastic MC3T3-E1 cells to produce ROS by NADPH oxidase enzymes pathway, and which may be one of the pathogenesis of AOPP involved in osteoporosis.

Objective To improve the diagnosis and treatment of hip gouty arthritis. Methods Retrospective analysis of cases of gouty arthritis from 2014 January to 2017 March was conducted.Patients with gouty arthritis were divided into hip joint group and common joint group (without invaded hip) and hip joint group was further divided into acute group and chronic group.Clinical data of each group were compared. Results Compared with common joint group, hip joint group usually combined with obesity or previous ipsilateral hip trauma history.Male patients in hip joint group were younger and the patients with unilateral hip involvement were without obvious local swelling, heat and pain.CT scan and sonography can provide assistance for early diagnosis and operation or pathological diagnosis was needed for the confirmation when necessary. Conclusions Hip gouty arthritis lacks specific diagnostic criteria in clinic, and is easily confused with other diseases associated with hip. Therefore, the possibility of the hip gouty arthritis should be taken into consideration when the diagnosis of the hip disease is not clear.
