1.Rapid diagnosis of acute dyspnea using the combined test of NT-pro-BNP and left ventricular ejection fraction

Objective To investigate the value of N-termind pro-B-type natriuretic peptide(NT-pro-BNP)in differential diagnosis of dyspnea in emergency department, and to investigate the rapid diagnosis cutoff of dyspnea due to acute congestive heart failure. Methods Ninety. one cases of dyspnea in emergency department recruited from January to June ,2008 were divided into two groups: acute cardiac dyspnea group and none acute cardiac dyspnea group. To evaluate the value of different parameters in differential diagnosis of dyspnea in emergency department and analysis the area under the receiver-operating characteristic of different parameters for the diagnosis of acute cardiac dyspnea. To achieve the best cutoff of different parameters for the diagnosis of dyspnea due to acute congestive heart failure finally. Results Among two groups, NT-pro-BNP (acute cardiac dyspnea vs. none acute cardiac dyspnea,6203.50 ng/L vs. 1410.00 ng/L,P < 0.01), Troponin Ⅰ (acute cardiac dyspnea vs. none acute cardiac dyspnea,0.12 μg/L vs. 0.03 μg/L,P <0.01) ,left ventricular ejection fraction(acute cardiac dyspnea vs. none acute cardiac dyspnea,46.25% vs. 65.60%, P < 0.01), left atrial diameter (acute cardiac dyspnea vs. none acute cardiac dyspnea,42.75 mm vs. 36.00 mm,P <0.01) had significant difference. NT-pro-BNP at cutoff of ≥3715 ng/L was highly sensitive and specific for the diagnosis of acute cardiac dyspnea. Receiver-operating characteristic analyses demonstrated that NT-pro-BNP and left ventricular ejection fraction was the best diagnostic indices of acute cardiac dyspnea. The area under the receiver-operating characteristic curve of Nt-pro-BNP was 0.828 ± 0.045 (P < 0.01),and left ventricular ejection fraction was 0.829 ± 0.049 (P < 0.01). Correlation analysis showed that NT-pro-BNP was correlated with left ventricular ejection fraction (P < 0.01). The combined test of NT-pro-BNP and left ventricular ejection fraction was performed. Specificity increased to 96.50%, total consistent rate increased to 83.50% ,positive predictive value increased to 91.30%, positive likelihood ratio 17.60, faulse diagnostic rate decreased to 3.50%. Conclusions NT-pro-BNP examination in emergency department was helpful to rapid differential diagnosis of dyspnea. It helped to differentiate the patients with acute congestive heart failure and none acute congestive heart failure causes of dyspnea.

3.Fusion gene pEGFP-C3-B7.2-hTERT induces immune response in mice and restrains transplanted hepatoma

Objective:To construct a fusion DNA vaccine pEGFP-C3-B7.2-hTERT and to observe the capability of pEGFP-C3-B7.2-hTERT DNA vaccine to induce specific anti-tumor immune responses and to inhibit growth of the hepatoma transplanted from H22 cells. Methods: The B7.2 and hTERT cDNA (amplified by RT-PCR), together with pEGFP-C3 as the vector were used to construct fusion gene plasmid pEGFP-C3-B7.2-hTERT, which was then used to vaccinated C57BL/6 mice for 3 times at a 7 d interval. Animals vaccinated with pEGFP-C3-B7.2, pEGFP-C3-hTERT, pEGFP-C3 and PBS were taken as controls. Splenocytes CTLs of immunized mice, the levels of IL-2, IFN-? in the culture supernatant, the levels of antibodies against hTERT, and the changes of the T lymphocyte subsets in the peripheral blood were all examined. The mice harboring hepatocarcinoma H22 cells were challenged with pEGFP-C3-B7.2-hTERT and the tumor forming time and the survival periods of mice were observed. 4.Path Analysis on the Influence of Occupational Benefits and Turnover Intention on the Quality Life of Nurses

Objective:To explore the relationship among occupational benefits,turnover intention and quality of life in nurses using structural equation model,thus to provide evidence for improving the quality of life.Method:We conduct a survey among 965 nurses using the general information questionnaire,the occupational benefits scale,turnover intention scale (TIS) and the SF-36 scale.Restult:The total score of occupational benefits was (135.79 ± 18.82),the TIS score was (13.48 ±4.11),and the SF-36 score was (532.91 ± 141.75).There was a positive correlation between occupational benefits and the quality of life,and the turnover intention was negatively correlated with the quality of life.Structural equation model analysis showed that the occupational benefits and turnover intention had a direct significant effect on nurses' quality of life;occupational benefits has a direct negative effect on turnover intention,and indirectly effects quality of life of nurse through the mediating effect of turnover intention.Conclusion:The nurses' quality of life needs to be improved,and occupational benefits and turnover intention have significant prediction effect on quality of life.

6.Hospital Infection among Deceased Inpatients:A 187 Case Analysis

OBJECTIVE To analyze the characters of hospital infection among deceased inpatients.METHODS The inpatients who died in hospital from Jan 2004 to Dec 2005 were analyzed retrospectively.RESULTS The infection rate of deceased inpatients was 40.11%,and the location majored in respiratory tract,urinary system and digestive tract.The infection rate was apparently higher among the cases more than 60 years old,and those with hematological disease,circulatory disease and nervous system disease.The hospital infection rate was related with long hospitalization and had seasonal fluctuation.CONCLUSIONS The high hospital infection rate among deceased pabients may be related with the seriously underlying diseases, aging,drug abuse and invasive therapy.
7.Relationship of perceived professional benefit,job satisfaction,and retention willingness of nurses

Objective To explore the relationship of perceived professional benefit, job satisfaction, and retention willingness of nurses. Methods Convenience sampling method was adopted to investigate 493 nurses from 3 tertiary grade A hospitals in Taian. Results The mean scores of nurses′perceived professional benefit, job satisfaction, and retention willingness were 134.19 ± 19.79,121.52 ± 18.41, and 21.18 ± 4.23, respectively. Perceived professional benefit and job satisfaction were positively correlated with retention willingness (P<0.01). Job satisfaction played a partial mediation role in the relationship between perceived professional benefit and retention willingness. Conclusion Job satisfaction and perceived professional benefit have a positive influence on nurses′retention willingness .
