Objective To explore clinical effect of Chinese herbal immersion combined with Beecher's exercise for prevention and treatment of grade-0 diabetic foot.Methods A total of 107 patients who were diagnosed with grade-0 diabetic foot were selected in Department of Endocrinology of our hospital from July 2009 to October 2013.In addition to regular treatments,their feet were immersed in Chinese herbal liquid,and then were taken care by doing Beecher's exercise.The symptoms of diabetic foot were self-rated by each individual patient and mastering of knowledge about diabetic foot was evaluated both before the intervention and after three courses of treatment.Results After three courses of treatment,the self-rating symptom scored (4.37±1.45) totally,significandy lower than that before the intervention,(5.62±1.55),t=7.91,P ＜ 0.05.The mastering of six testing indexes about diabetic foot obviously improved compared between those before and after three courses of treatment [62.62％ (67/107)],23.36％ (25/107),30.84％ (33/107),46.73％ (50/107),57.94％ (62/107),50.47％ (54/107) vs.96.26％ (103/107)],86.92％ (93/107),89.72％ (96/107),85.98％ (92/107),94.39％ (101/107),87.85％ (94/107),x2=37.78,87.35,77.46,36.92,39.16,35.05,all P ＜ 0.05.Conclusions For grade-0 diabetic foot,Chinese herbal immersion treatment combined with Beecher's exercise can be applied straight forwardly on the disease focus;it's clinically safe and effective,and easy to be operated.It improves clinical symptoms of grade-0 diabetic foot by taking advantages of traditional Chinese medicine,prevent and control further development of diabetic foot and is valuable to be applied in clinical work.

Objective:To construct a fusion DNA vaccine pEGFP-C3-B7.2-hTERT and to observe the capability of pEGFP-C3-B7.2-hTERT DNA vaccine to induce specific anti-tumor immune responses and to inhibit growth of the hepatoma transplanted from H22 cells. Methods: The B7.2 and hTERT cDNA (amplified by RT-PCR), together with pEGFP-C3 as the vector were used to construct fusion gene plasmid pEGFP-C3-B7.2-hTERT, which was then used to vaccinated C57BL/6 mice for 3 times at a 7 d interval. Animals vaccinated with pEGFP-C3-B7.2, pEGFP-C3-hTERT, pEGFP-C3 and PBS were taken as controls. Splenocytes CTLs of immunized mice, the levels of IL-2, IFN-? in the culture supernatant, the levels of antibodies against hTERT, and the changes of the T lymphocyte subsets in the peripheral blood were all examined. The mice harboring hepatocarcinoma H22 cells were challenged with pEGFP-C3-B7.2-hTERT and the tumor forming time and the survival periods of mice were observed. Results: Agarose gel electrophoresis, nuclease digestion and sequencing confirmed the successful construction of pEGFP-C3-B7.2-hTERT. Objective To evaluate the efficacy of facial vein-superior ophthalmic vein approach to embolize carotid-cavernous sinus fistulas.Metheds The involved cavernous sinus was catheterized through the femoral vein-facial vein- superior ophthalmic vein approach, GDC, EDC, free microcoil, or silk were used to pack the sinus and occlude the shunt. If therer was any difficulty in catheterizing the faical vein, facial vein was exposed surgically and punctured, and then, through the superior opthalmic vein, the cavernous sinus was packed. Results 16 cavernous sinuses in 14 CCF patients(5 traumatic CCFs, 9 dural CCFs) were catheterized through facial vein-superior ophthalmic vein approach, and the technical success was achieved in 15 cavernous sinuses. Immediate angiographic cure of the shunts was achieved in 11cases, residual shunts with inferior petral sinus drainage in 2. Facial vein occlusion was encountered in 1 patient during the facial vein catheterization, further packing of the cavernous sinus was not performed, but follow-up angiography at the 21 st day revealed the spontaneous cure of the shunt. The VI cranial nerve palsy present after balloon embolization in a type A CCF was not improved after the packing of the cavernous sinus. Ocular symptoms in other patients disappeared after tranvenous embolization. The clinical follow-up period ranged from 3 to 21 months, no recurrence of the symptoms was found. Follow-up angiography in 2 patients with residual shunting showed the unchanged shunts, no further embolization was performed. No follow-up angiography was performed in other patients.Conclusions The facial vein-superior ophthalmic vein approach can be chosen as an optimum treatment for dural CCFs, and an important alternative treatment for type A CCFs after the failure of the initial balloon embolization.

Traditional Chinese medicine(TCM)compatibility is related to the safety of clinical use of drugs and embodies the essence of interactions between drugs and organisms. This is a scientific problem thatr has attracted more attention. Currently, most of the knowledge on TCM compatibility is from predecessors′ experience. The corresponding molecular biology mechanism is poorly understood. As a powerful tool to identify a large number of proteins simultaneously,proteomics technology has the potential to reveal the protein alterations under certain conditions,and to provide more direct insights into biological processes during TCM compatibility. In this paper,we introduced the main technology of proteomics,including two dimensional gel electrophoresis,2D-DIGE,iTRAQ, QconCAT/MRM and chemical proteomics. Also,the applications of these technologies in the field of TCMM compatibility study were reviewed.

Objective To evaluate the CT features of aorto-right atrial fistula after aortic valve replacement(AVR) or ascending aortic replacement.Methods Eighty-seven patients with aortic-right atrial fistula underwent CT after operation.The CT features were retrospectively analyzed.Fistula was measured according to maximum width of the shunt.Results Aorto-right atrial fistula was detected in 87 patients after aortic valve replacement or ascending aortic replacement by CT scan. Among them,25 patients were diagnosed as mild aorto-right atrial fistula,47 patients as moderate,and 15 patients as severe.Thirty-seven patients underwent follow-up CT.Among them,10 patients with mild to moderate aorto-right atrial fistula were considered to have complete regression,8 patients with mild aorto-right atrial fistula considered to have incomplete regression,14 patients with mild to moderate aorto-right atrial fistula considered to have stable condition,and 5 patients with moderate aorto-right atrial fistula considered to have progression at the 3-month follow-up.Conclusion CT is a useful tool for defining aorto-right atrial fistula after AVR or ascending aortic replacement and for evaluating it in follow-up.

ObjectiveTo investigate the efficacy of different methods of anesthesia for laparoscopic hysterectomy.MethodsSixty ASA Ⅰ or Ⅱ patients,aged 45-60 yr,weighing 55-65 kg,scheduled for laparoscopic hysterectomy,were equally and randomly divided into 2 groups:combined intravenous-inhalational anesthesia group (group Ⅰ ) and combined spinal-epidoral anesthesia (CSEA) + general anesthesia group (group Ⅱ ).In group Ⅰ,anesthesia was maintained with inhalation of sevoflurane and infusion of remifentanil after induction of anesthesia.In group Ⅱ,CSEA was performed,after the upper level of sensory block was stable,general anesthesia was induced and maintained with inhalation of sevoflurane,and state entropy (SE) was naintained at 45-60.Arterial blood samples were taken to determine the plasma concentrations of adrenaline ( AE ),norepinephrine (NE) and dopamine (DA) after admission to the operation room,after completion of pneumoperitoneum,at 10 min after pneumopentoneum,during uterus traction,during removal of the laryngeal mask airway,and at 10 min after removal of the laryngeal mask airway (T0-5).The time for recovery of spontaneous breathing,extubation time,and time of regaining consciousness were recorded at the end of operation.The side-effects and number of patients requiring increments of analgesics were also recorded within 48 h after operation.Patient' s satisfaction was recorded at 48 h after operation.ResultsCompared with group Ⅰ,the plasma concentrations of AE and NE at T3-5 and the plasma concentrations of DA at T3,5 were significantly decreased,the time for recovery of spontaneous breathing,extubation time,and time of regaining corsciousess were significantly shortened,and the incidence of agitation and the number of patients requiring increments of analgesics were significantly decreased in group Ⅱ ( P ＜0.05).There was no significant difference in the incidence of intraoperative awareness,and nausea and vomiting after operation,and the level of patient' s satisfaction at 48 h after operation between the two groups ( P ＞ 0.05).ConctusionCSEA + general anesthesia has better efficacy than combined intravenous-inhalational anesthesia when used for laparoscopic hysterectomy.

Objective To evaluate the computed tomography (CT) characteristics of cardiovascular involvement in Beheet syndrome. Methods Eleven patients with clinically diagnosed Behcet syndrome were studied retrospectively from July 1995 to December 2007. Electron beam CT or 64-slice helical CT scanner was used and CT characteristics were reviewed. Results Eleven patients were diagnosed according to the criteria reported by the international study group for Behcet syndrome. Of them, 4 patients presented with aortic valve prolapse (2 patients with mitral valve prolapse), false aneurysm of right coronary artery was demonstrated in 2 patients, false aneurysm of left subclavian artery, aortic aneurysm and penetrating ulcers, aortic arch false aneurysm, aortic dissection, pulmonary embolism and interatrial scptum aneurysm in 1 case, respectively. Conclusion CT is a very useful method for the diagnosis and foUow-up of Behcet syndrome.

Objective To compare the potential cytotoxicity induced by Veratrum nigrum coadministered with Panax ginseng, and to provide experimental evidence on the mode of herb-herb interaction based on human liver drug metabolizing enzymes.Methods The effect of V.nigrum and coadministration on cultured human hepatoma (HepG2) cells was investi-gated by detecting morphological changes , cell viability , cytomembrane integrity and apoptosis after the cells were treated for 24 h.The mRNA expression levels of drug metabolizing enzymes influenced by P.ginseng were determined by real-time quantitative reverse-transcriptase polymerase chain reaction .Results V.nigrum coadministered with P.ginseng had a better inhibitive effect on the growth of HepG2 cells at the IC50value of (15.18 ±1.03) mg/ml than at the value of IC50 (21.46 ±1.10) mg/ml of V.nigrum.Coadministration more significantly raised the LDH level in cell culture medium than at the same dose of V.nigrum.Moreover, in coadministration group, compared with the same dose of V.nigrum,the total apoptosis and necrosis of HepG2 cells were significantly increased .P.ginseng had effect on the expression of CYP3A4, CYP1A1, CYP1A2, CYP2B6 and CYP2E1 mRNA.Conclution Compatibility of medicines in a prescription also has herb-herb interactions based on drug metabolizing enzymes .The interaction mode is that the P.ginseng inhibits and induces CYPs and the modulated CYP isozymes ,inturn,have an impact on the metabolism of constituens in coadministered herbs causing herb-herb interaction .

Objective: To explore the imaging and clinical features of Behcet's disease with coronary artery involvement in order to improve the diagnosis and treatment in clinical practice. Methods: A total of 6 Behcet's disease patients with coronary involvement diagnosed and treated in our hospital from 1999-08 to 2016-11 were analyzed. Coronary angiography (CAG) and coronary CTA were performed for diagnosis. There were 5 cases received CAG and 5 cases received CTA examinations. Results: All 6 patients had the first clinical visit by sudden pericardial tapenade, myocardial infarction or cardiac shock. For diagnosis: the imaging presentation included coronary stenosis, occlusion and pseudo aneurysm formation. 3 patients had anterior descending artery involvement, 1 had the far-end of left circumflex involvement and 2 had the middle segment of right coronary involvement; all patients were combining large amount of pericardial effusion. For treatment: there were 3 patients with aneurysm received conservative treatment (1 case had open thoracic exploration), 1/3 lost contact and 2 having long-term normal life; 3 patients received coronary stent implantation without optimal effect. Conclusion: The patients of Behcet's disease with coronary involvement may easily form pseudo aneurysm; CAG/CTA has been helpful for diagnosis. Clear pre-operative diagnosis of etiology is important for treatment.

Objective To investigate the effect of intrathecal gastrodin on skin cancer pain in mice.Methods Thirty-two female Balb/c mice,aged 8 weeks,weighing 20-25 g,were randomly divided into 4 groups (n=8 each) using a random number table:sham operation group (group S),skin cancer pain group (group SCP),gastrodin group (group G),and artificial cerebrospinal fluid (ACSF) control group (group ASCF).Skin cancer pain was produced by injecting phosphate buffer solution 20 μl containing about 2 ×105 4T1 breast cancer cells into the plantar surface of the left hindpaw.At 14th day after inoculation of cancer cells,ASCF 5 μl was injected intrathecally in S and ACSF groups,and gastrodin 150 μg/kg (5 μl) was injected intrathecally in group G.Before inoculation,at 30 min before intrathecal injection,and at 15,30,60,90,120 and 150 min after intrathecal injection,the thermal paw withdrawal latency (TWL) was measured.The expression of acid-sensing ion channel (ASIC)-3 mRNA in the spinal dorsal horn was detected using the real-time reverse transcriptase polymerase chain reaction after the last measurement of the pain threshold.Results Compared with group S,the TWL was significantly decreased at each time point before and after intrathecal injection in SCP,ACSF and G groups,and the expression of ASIC-3 mRNA in the spinal dorsal horn was significantly down-regulated in group G (P＜0.05).Compared with group SCP,the TWL was significantly increased at each time point after intrathecal injection,and the expression of ASIC-3 mRNA in the spinal dorsal horn was significantly down-regulated in group G (P＜0.05),and no significant change was found in the parameters mentioned above in group ACSF (P＞0.05).Conclusion Intrathecal gastrodin can reduce skin cancer pain and down-regulate ASIC-3 expression in the spinal dorsal horn which is helpful in maintaining the analgesic effect in mice.
