2.Effect Evaluation of Spironolactone in the Treatment of Hypertensive Heart Disease by the Real Time 3 -dimensional Echocardiography

Objective To investigate the effect evaluation of spironolactone in the treatment of hypertensive heart disease by the real time 3-dimensional echocardiography .Methods Totally 104 patients with hypertensive heart disease were in the study from Jan 2011 to Jan 2012 of our department and divided into objective group and control group .Conventional treatments were put in all the patients and the spironolactone was used in objective group patients only .The results of ventricular morphology and function indexes were compared be -tween the two groups, and then investigate the relationship between cystatin C (CycC) and left ventricular remodeling index (LVRI) at the same time.Results There were significant differences in the comparison of ventricular morphology and function indexes .The left ventric-ular end-diastolic volume(LVEDV), left ventricular end systolic volume (LVESV) and left ventricular ejection fraction (LVEF) of ob-jective group were more advantageous than flowse of control group for prognosis .The same results were observed in the comparison of Cys C and LVRI.Moreover, Cyc C and LVRI had certain correlation in the study .Conclusion The spironolactone can play a positive role in the treatment for the patients with hypertensive heart disease .In the meantime, the RT-3DE is a good evaluation method for the develop-ment of ventricular morphology and function .
3.The impact of neoadjuvant chemotherapy combined with tumor cell death and postoperative intraperitoneal chemotherapy on efficacy and quality of life of patients with ovarian cancer

Objective To observe the effect of neoadjuvant chemotherapy combined with tumor cells reduction and postoperative intraperitoneal perfusion chemotherapy on the efficacy and quality of life of ovarian cancer.Methods From October 2010 to October 2013 at the Department of gynecology,Taizhou Municipal Hospital,65 cases of ovarian cancer patients for the treatment of stage Ⅲ and Ⅳ were randomly divided into intervention group (n =32) and control group (n =33).Patients in the intervention group received neoadjuvant chemotherapy after tumor cell reduction cytoreductive surgery and received postoperative intraperitoneal hyperthermic perfusion chemotherapy and conventional surgery after chemotherapy.Patients in the control group received conventional tumor cell cytoreductive surgery plus postoperative chemotherapy.Surgical status,the effectiveness of treatment,and adverse reactions were compared between two groups.Moreover,survival quality score,relapse rate,and mortality differences were compared between two groups.Results For the intervention group,operation status and effective treatment were better than the control group.The proportion of adverse reactions had no significant difference between two groups.Survival quality had no statistically significant difference between before treatment and after receiving treatment.Quality of life in the intervention group was better than that of the control group.Relapse rate,and mortality rate in the intervention group was lower than the control group.Conclusions Accepted neoadjuvant chemotherapy combined with tumor cytoreductive surgery and surgery after intraperitoneal hyperthermic perfusion chemotherapy can improve the effectiveness of treatment of ovarian cancers,and improve the quality of life,with better short-term effect.
4.Posttraumatic growth in breast cancer survivors and its relationship with cognitive appraisal

Objective To explore the status and influencing factors of posttraumatic growth (PTG) in breast cancer patients and identify the influence of cognitive appraisal on PTG and the relationship between PTG and cognitive appraisal. Methods A cross-sectional study was conducted. Totally 356 breast cancer patients were investigated with the revised Posttraumatic Growth Inventory (PTGI), the Cognitive Appraisal Scale. Results The average score of PTG in breast cancer patients was (55.06±14.36) points. There were significant differences between different education level and health-care payment patients in the total score of PTG (t=5.886, 5.154, P<0.01). The level of PTG was correlated with the patients′ cognitive appraisal way. It was negatively correlated with uncertain appraisal about disease and threat appraisal,and positively correlated with positive appraisal and responsible appraisal. The main prediction factors for PTG were positive appraisal, uncertain appraisal about disease, responsible appraisal, health-care payment and education level. All the variables could explain 42.7% variance of PTG. Conclusions Breast cancer patients in this study report moderate level of posttraumatic growth. The level of PTG in breast cancers is influenced by variables of patients′ education level, health-care payment and cognitive appraisal. Cognitive appraisal could predict the development of PTG significantly.
5.Application of proteomics technologies pharmacology and toxicology mechanismsof in traditional Chinese medicine compatibility study

Traditional Chinese medicine(TCM)compatibility is related to the safety of clinical use of drugs and embodies the essence of interactions between drugs and organisms. This is a scientific problem thatr has attracted more attention. Currently, most of the knowledge on TCM compatibility is from predecessors′ experience. The corresponding molecular biology mechanism is poorly understood. As a powerful tool to identify a large number of proteins simultaneously,proteomics technology has the potential to reveal the protein alterations under certain conditions,and to provide more direct insights into biological processes during TCM compatibility. In this paper,we introduced the main technology of proteomics,including two dimensional gel electrophoresis,2D-DIGE,iTRAQ, QconCAT/MRM and chemical proteomics. Also,the applications of these technologies in the field of TCMM compatibility study were reviewed.
6.Discussion on the Improvement of Ethical Review Level of Ethics Committee Based on the Construction of Ethical Review Platform of Traditional Chinese Medicine

Though there is a great progress in research of medical ethics in China, a gap still remains between China and the developed countries now. Medical ethics committees are established one after another in many hospitals, but the level of standardization construction and ethical review level differ in thousands ways. The construction of ethical review platform on TCM can standardize the construction and investigation procedure, and can further enhance the ethical review level of medical ethics committee. Among them, institutional improvement of regulations and procedures can be acquired in short time, but the enhancement of ethical review level needs a long way to go. This article aimed to probe the thinking and methods on the enhancement of ethical review level through the practice and experience of the construction of TCM ethical review platform.
7.Study on the Contents of Naringin in Fructus Aurantii Immaturus of Different Specifications

Objective To study the content difference of naringin in Fructus Aurantii Immaturus of different specifications.Methods HPLC was performed on the Merck- Lichrospher RP-C18(4.6 mm? 250 mm, 5 ? m) with temperature of 35 ℃ .The chromatographic conditions for naringin:the mobile phase consisting of acetonitrile-0.3 % Phosphoric acid(20 ∶ 80), at the flow rate of 1 mL/min, and the detection wavelength at 283 nm. Results The content of naringin increased with the diameter of Fructus Aurantii Immature slices.Conclusion It is suggested that there exists significant differences in intrinsic quality among Fructus Aurantii Immaturus of different specifications.
8.Effect and mechanism of zinc oxide nanoparticle on cardiac development of zebrafish embryos

OBJECTIVE To explore the toxic effects of zinc oxide nanoparticls (ZnO-NPs) on cardiac development of zebrafish embryos and rat myocardial cell lines (H9c2),as well as potential molecular mechanisms.METHODS ZnO-NPs were characterized.Zebrafish embryos were exposed to different doses of ZnO-NPs (0,0.5,2.5,5.0 and 10.0 mg· L-1) for 24 to 96 h at 4 h post fertilization (4 hpf).The embryo mortality was observed.The expressions of notochord homeobox (noto),T-box 6 (tbx16),T,brachyury homolog a(ta),and tbx6 which were related to cardiac mesoderm were investigated using real-time PCR at 17 hpf.The heart rate and number of cardiomyocytes of embryos [[Tg (cmlc2:nucdsRed)] exposed to 0,2.5 and 5 mg· L-1 ZnO-NPs were detected at 72 hpf.Rat myocardial cell lines (H9c2) were treated with ZnO-NPs (0.1,0.5,1.0,5.0 and 10.0 mg· L-1) for 24 h.Cell viability was measured with Alamar Blue method.Mitochondrial ultrastructure was observed by transmission electron microscopy.Cellular ATP was detected using chemiluminescence,and oxygen consumption rate (OCR) was examined with Seahorse instrument.RESULTS The particle size of ZnO-NPs was (331 ±3)nm.The ZnO-NPs LC50 of zebrafish embryos at 48 hpf was 21.81 mg· L-1.The mRNA expressions of noto,ta and tbx6 were reduced after ZnO-NPs 2.5 mg· L-1 treatment at 17 hpf.The heart rate of 72 hpf zebrafish was 153 min-1 in the ZnO-NPs 5 mg· L-1 group,12.6% lower than that in the cell vehicle group (P<0.01),and the number of cardiomyocytes decreased by 15.5% (P<0.01) compared with the cell vehicle group.Reduced cell viability and mitochondrial vacuolation were observed in H9c2 after ZnO-NPs 0.5 mg· L-1 exposure.Compared with the cell vehicle group,the cell ATP decreased by 25.7% (P<0.05),and OCR decreased by 27.2% (P<0.01).CONCLUSION Low-dose ZnO-NPs exposure has effect on the cardiac development of zebrafish,mainly due to reduced heart rate and decreased number of cardiomyocytes.These changes may be related to the decreased expressions of cardiac development-related genes and the impairment of mitochondrial structure and function.
9.The expression of ESBLs genes in Escherichia coli isolates induced by β-lactam antibiotics and inhibited by histidine kinase inhibitors

Objective To investigate the genotypes of extended spectrum β-lactamases (ESBLs) and their carrying modes in Escherichia coli (E.coli) isolates,and to analyze the mechanism of protein phosphorylation and ESBLs gene expression induced by β-lactam antibiotics or inhibited by histidine kinase inhibitors.Methods The predominant genotypes of ESBLs (KPC,TEM,SHV and CTX-M) and their carrying modes were identified by PCR and sequencing analysis.E-test and micro-tube dilution method were applied to measure minimal inhibitory concentrations (MICs) and minimum bactericidal concentrations (MBCs).Immobilized metal ion affinity chromatography,bacterial protein phosphorylation detection kit and real-time fluorescent quantitation RT-PCR were performed to analyze the enhancing effects of 1/4 MIC penicillin or cefotaxime or the inhibitory effects of histidine kinase inhibitors (closantel,bromized or iodized methylimidazol) on protein phosphorylation and the expression of ESBLs at mRNA level in E.coli isolates.Results In 183 β-lactam antibiotics-resistant E.coli isolates,TEM and CTX-M genes (83.1% and 77.1%) were highly expressed than other two ESBLs genes with a prevalent carrying mode of coexisting (65.0%) (P<0.05).Penicillin or cefotaxime at 1/4 MIC induced the protein phosphorylation and promoted the expression of TEM,SHV and CTX-M at mRNA level (P<0.05).Closantel (200 μmol),bromized methylimidazol (2 or 10 μmol) or iodized methylimidazol (20 or 50 μmol) could neither kill E.coli isolates nor inhibit their growth,but could inhibit the protein phosphorylation induced by above mentioned antibiotics and enhance the expression of ESBLs at mRNA level (P<0.05).Moreover,the susceptibility of antibioticresistant E.coli strains to penicillin and cefotaxime were increased (P<0.05).Conclusion TEM and CTX-M were the predominant genotypes of ESBLs carried by β-lactam antibiotics-resistant E.coli strains isolated from Zhejiang province,which were mostly found in a TEM plus CTX-M carrying mode.Sublethal dose of β-lactam antibiotics could up-regulate the expression of ESBLs genes in E.coli isolates via TCSS,but it could be inhibited by histidine kinase inhibitors.
10.Clinical and immunological features of 45 children with lupus nephritis

Objective To analyze the clinical and immunological features of 45 pediatric patients with lupus nephritis (LN). Methods Forty-five LN patients were included in this study. Clinical, pathological data and immunological parameters were retrospectively analyzed. Results Forty-five LN patients had 6 males and 39 females, with the mean onset age of (10.9 ± 2.8) years. Acute nephritis was the most common type, accounting for 42.2%. Nephrotic syndrome accounted for 31.1%. Renal biopsy showed class II (17.8%), III (4.4%), IV (48.9%), V (2.2%), V+III (6.7%)and V+IV (13.3%)in 42 cases. The remis-sion rate reached 91.1%in the early therapeutic stage, and 15.0%patients recurred after 24-month follow-up. Conclusions The clinical manifestations of LN children are diverse. The renal pathology is complex. The clinical manifestations in part of the chil-dren are not consistent with renal pathology.
