2.Application of fast-track surgery in perioperative period of colon cancer: a randomized controlled clinical trial

Objective This study was designed to compare the safety and efficacy of fast track surgery (FTS) with conventional perioperative management in colonl cancer patients undergoing radical resection.Methods From May 2011 to May 2013,66 patients with colorectal cancer were randomly assigned to FTS group and control group.Outcomes were assessed by length of postoperative hospital stay,medical cost,nutritional status,gut function,and postoperative complication.Results FTS group was associated with a significantly shorter postoperative hospital stay,lessmedical cost,earlier first passage of flatus,less loss of body weight in the postoperative period compared with control group,and there were no differences in morbidity or mortality between two groups.Conclusion The radical resection under the guidence of fast track rehabilitation is safe and effective method.Significantly reduce patients' perioparative stess response and accelerate the recovery.
3.Effects of low molecular weight hirudo peptides on focal cerebral ischemic reperfusion injury in rats

Purpose To observe the protective effects of low molecular hirudo peptides on focal cerebral ischemia reperfusion injury in rats and to explore its possible mechanism. Methods Middle cerebral artery occlusion (MCAO) was used to prepare focal cerebral ischemia and the neurological scores and infarction area of brain slices, and water content of brains were assessed. Superoxide dismustase (SOD) activity and malondi-aldehyde ( MDA) content in homogenate of ischemic brain tissue were determined by spectrophotometric assay. Results Hirudo peptides could reduce the percentage of infarction area and the water content in the cerebral hemisphere, increase SOD activity and decrease MDA content in ischemic brain tissue. Conclusion Low molecular hirudo peptides have protective effects on focal cerebral ischemia injury, and its mechanism may be related to the antioxidant action.
4.Effects of LPS and TNF-? on expression of SSeCKS by endothelial cell

Objective:To study the effects of LPS and TNF-? on the expression of SSeCKS and morphology as well as cytoskeleton of endothelial cell, so as to explore the role of SSeCKS in cell morphology changes.Methods:The cultured Bovine pulmonary artery endothelial cells(BPAEC) was induced by LPS, TNF-? and the expression of SSeCKS was detected by in situ hybridization,Western blot and immunohistology. Immunofluorescent staining method with confocal laser-scanning fluorescence microscope was used to observe the effects of LPS and TNF-? on the morphology of endothelial cells and the organisation of SSeCKS as well as cytoskeleton.Results:Firstly, we found that TNF-? could induce the expression of SSeCKS in a concentration and time dependent manner , meanwhile LPS had no effects on SSeCKS expression. Secondly, it was observed that LPS and TNF-? induced reorganization of F-actin and SSeCKS in endothelial cell. Thirdly,PKC inhibitor Ro-31-8220 reversed the effect of LPS,TNF-? on F-actin and SSeCKS in endothelial cells.Conclusion:The results demonstrate that TNF-? could induce endothelial cell to express SSeCKS; PKC plays a role in the reorganization of SSeCKS and F-actin in endothelial cells induced by LPS and TNF-?; the results suggest that the mechanism for reorganization of cytoskeleton induced by LPS, TNF-? be partially related to the SSeCKS of ECs.
5.Expression of progesterone receptor membrane component-1 is associated with the malignant phenotypes of breast cancer.

OBJECTIVETo investigate the correlation of the expression of progesterone receptor membrane component-1 to the clinicopathological features of breast cancer.
METHODSThe expression of PGRMC1 was detected immunohistochemically in 60 surgical specimens of breast cancer, and the association of PGRMC1 with the histological grade, lymphatic metastasis, TNM stage and prognosis was assessed.
RESULTSPositive CA-9 expression was detected in 37 (61.67%) of the cases. PGRMC1 expression was significantly correlated to lymph node metastasis (P=0.004), tumor size (P=0.03), TNM stage (P=0.039), overall survival rate (Log-rank=10.378, P=0.0001), and tumor-free survival (Log-rank=4.443, P=0.035), but not to the patients' age (P=0.196) or histological grade (P=0.526). Multivariate survival analysis indicated that PGRMC1 was an independent prognostic factor of breast cancer (P=0.002).
CONCLUSIONSThe expression of PGRMC1 is strongly associated with the progression of breast cancer, and may serve as a useful prognostic indicator of the malignancy.

6.Chemical constituents from Hypericum perforatum.

OBJECTIVETo study the chemical constituents from Hypericum perforatum.
METHODCompouds were isolated by chromatographic techniques. Their structures were identified by spectral methods. The inhibitory activity of recombinant human PTP1B was evaluated.
RESULTNine compounds were elucidated as D-Mannitol (1), 1,2-benzenedicarboxylic acid bis(1-methylpropyl) ester (2), (7E, 6R,9S)-9-hydroxy-4,7-megastigmadien-3-one (3), (6S,9R)-roseoside (4) , 2,6-dimethoxy-4-hydroquinone-l-O-beta-D-glucopyranoside (5), 2,6-di-methoxy-4-hydroxybenzyl alcohol 1 -O-beta-D-glucopyranoside (6), syringate 4-O-beta-glucopyranoside (7), hypericin (8), skyrin-6-0-beta-D-glucopyranoside (9) , (R)-2,3-dihydroxypropyl 3,4-dihydroxy-benzoate (10). At a concentration of 2 micromol x L(-1), compound 8 inhibited recombinant human PTP1B with inhibitory rate of 96.4% and IC50 of 2.5 micromol x L(-1).
CONCLUSIONcompound 10 was new, compounds 2-4, 7 were obtained from Hypericum for the first time and compounds 5-6 was isolated from this plant for the first time. Compound 8 showed remarkable PTP1B inhibitory activity.

7.Chemical constituents from Parthenocissus quinquefolia.

The chemical constituents of Parthenocissus quinque were investigated. The chemical constituents were isolated by column chromatography on silical gel and sephadex LH-20. Their structures were elucidated on the basis of spectral analysis and of comparison of physical constant. Nine compounds were isolated from this plant and the structures of them were identified as 3,4,5-trihydroxy- benzoic acid (1), piceatannol (2), resveratrol (3), resveratrol trans-dehydrodimer (4), cyphoste mmin B (5), pallidol (6), cyphostemmin A (7), quercetin-3-O-alpha-L-rhamnoside (8), myricetin-3-O-alpha-L-rhamnoside (9), respectively. Compounds 1, 4-9 were isolated from this plant for the first time.

8.MRTF-A alleviates myocardial ischemia reperfusion injury via inhibiting TLR4/TRIF signaling pathways

Objective To observe the effect of myocardial transcription factor MRTF-A on myocardium inflammation and its mechanism.Methods Totally 30 rats were randomly divided into the sham,ischemia-reperfusion (myocardial ischemia 30 min and reperfusion 2 h),and MRTF-A groups(myocardial ischemia 30 min and reperfusion 2 h & Lentivirus infection MRTF-A) (n=10 each group).Serum myocardial enzyme activity was detected by biochemical analysis,myocardial infarct size detected by TTC,and degree of myocardial injury was measured by HE staining.The TLR4 and TRIF expression was analyzed by immunohistochemistry and qPCR.Results Compared with the sham group,the MRTF-A group significantly increased the activity of serum myocardial enzymes CK-MB and LDH (P<0.05).The infarct area of myocardial tissue was gray-white,and the infarct area was (54.31±3.07)% (P < 0.05).Myocardial fibrosis was disorder,myocardial cell was swollen and burst,and inflammatory cell infiltration was obvious.Protein and mRNA expressions of TRL4 and TRIF were significantly up-regulated (P<0.05).Compared with the ischemia-reperfusion group,the levels of CK-MB and LDH were significantly reduced after myocardial infection with MRTF-A (P<0.05).The myocardial infarction area was significantly reduced to (16.74±4.26)% (P< 0.05).The myocardial structure was nearly normal with mild edema.Protein and mRNA expression of TRL4 and TRIF decreased significantly (P<0.05).Conclusions The overexpression of transcription factor MRTF-A in myocardial cells alleviates the myocardial ischemia reperfusion injury by inhibiting the TLR4/TRIF signaling pathway and reducing the serum myocardial enzyme activity and myocardial damage.

Objective:To analyze the short-term clinical efficacy and safety of arsenic trioxide (ATO) combined with a modified N7 induction regimen in the treatment of children with high-risk neuroblastoma (NB).Methods:This study was a prospective, single-arm, multicenter phase Ⅱ clinical study. Sixty-seven high-risk NB children from eight units of Sun Yat-sen Memorial Hospital of Sun Yat-sen University, Wuhan Children′s Hospital of Tongji Medical College of Huazhong University of Science and Technology, First Affiliated Hospital of Guangxi Medical University, Hainan General Hospital, Affiliated Hospital of Guangdong Medical University, Kunming Children′s Hospital, Tongji Hospital of Tongji Medical College of Huazhong University of Science and Technology, and Guangdong Provincial Agricultural Reclamation Center Hospital were enrolled from January 2019 to August 2023 and were treated with ATO combined with a modified N7 induction regimen. The efficacy and adverse effects at the end of induction chemotherapy were assessed and analyzed, and the differences in the clinical characteristics were further compared between the treatment-responsive and treatment-unresponsive groups by using the Fisher′s exact test.Results:Among 67 high-risk NB children, there were 40 males (60%) and 27 females (40%), with the age of disease onset of 3.5 (2.6, 4.8) years. Primary NB sites were mostly in retroperitoneum (including adrenal gland) (56/67, 84%) and the common metastases sites at initial diagnosis were distant lymph node in 25 cases (37%),bone in 48 cases (72%),bone marrow in 56 cases (84%) and intracalvarium in 3 cases (4%). MYCN gene amplification were detected in 28 cases (42%). At the end of induction, 33 cases (49%) achieved complete remission, 29 cases (43%) achieved partial remission, 1 case (1%) with stable disease, and 4 cases (6%) were assessed as progressive disease (PD). The objective remission rate was 93% (62/67) and the disease control rate was 94% (63/67). The percentage of central system metastases at the initial diagnosis was higher in the treatment-unresponsive group than in the treatment-responsive group (2/5 vs. 2% (1/62), P=0.013), whereas the difference in MYCN gene amplification was not statistically significant between two groups (3/5 vs.40% (25/62), P=0.786). Grade Ⅲ or higher adverse reactions during the induction chemotherapy period were myelosuppression occurred in 60 cases (90%), gastrointestinal symptoms occurred in 33 cases (49%), infections occurred in 20 cases (30%), hepatotoxicity occurred in 4 cases (6%), and cardiovascular toxicity occurred in 1 case (2%). There were no chemotherapy-related deaths. Conclusion:ATO combined with N7-modified induction regimen had a superiority in efficacy and safety, which deserved further promotion in clinical practice.
