1.Vacuum sealing drainage and adsorbing material in treatment of refractoriness wound in 40 cases

A total of 78 patients were recruited from Department of Orthopedics in the Fourth Affiliated Hospital of Guangxi Medical University from January to June in 2008.Forty patients admitted in odd day,and they received the therapy of vacuum sealing drainage including vacuum sealing drainage to dress wound and semipermeable film for vacuum aspiration,namely curing group;38 patients' hospital admission time was in even day,and they underwent a common therapy to dress a wound again,namely control group.Two groups of patients were compared addressing the free skin grafting,skin flap transfer or free skin flap transplantation.Results showed that the healing ratio of the curing group was higher,it's infection rate was lower,and the length of hospital stay was also shorter than the control group.Vacuum sealing drainage is a better method in clinical medicine to cure the refractoriness raw surface.
2.Effects of combined exercise program on the executive function in primary students with Chinese learning difficulties

Objective To explore the effects of combined exercise program ( fun game + rope skip-ping + figure running) on executive function ( EF) in primary students with Chinese learning difficulties. Methods Fifty-four Chinese learning disabled students were selected and randomly divided into experimen-tal group and control group. Three tasks ( the Flanker task,the 1-back task and the More-odd shifting task) were used to measure pupil’ s EF ( inhibition,updating,shifting) . Results ( 1) Compared with the control group,the primary students with Chinese learning difficulties in experimental group showed a significant en-hancement in executive function (F(1,43)=9.45, P<0.01). (2) The positive effects of 10 weeks combined ex-ercise program on executive function in primary students with Chinese learning difficulties were different. Specifically,a beneficial effect on updating(F(1,43)= 11.37, P<0.01) was got,but there were no significant effects on inhibition (F(1,43)=1.52, P>0.05) and shifting(F(1,43)=2.57, P>0.05). Conclusion The effect of combined exercise program ( fun game+rope skipping+figure running) plays a positive role in promoting updating function of primary school students with Chinese learning difficulties.
3.MR Diagnosis of Stage Ⅰ Tumor Thrombus in Renal Cell Carcinoma

Objective To evaluate the accuracy of MR in the diagnosis of tumor thrombus in renal cell carcinoma.Methods 22patients with renal cell carcinoma proved pathologically were recruited in this study.All the patients underwent MR examination beforesurgery.The results of the MR were correlated with that of the operation.Results 8 renal veins could be found tumor thrombus during the surgery,and the other 14 renal veins and 22 inferior vena cava were absent of tumor thrombus.MR could found all of the 8 tumor thrombithat detected by the surgeons.In the other 14 cases,13 of them could be excluded of tumor thrombus by MR.Only in 1 patient,the MRresult was equivocal.CT was performed in 9 patients,the diagnosis was accurate in 7 cases,and was inaccurate in 2.Ultrasound wasperformed in 16 patients,the diagnosis was accurate in 13 cases,and was inaccurate in 3.Digital subtraction angiography was performed in 4 cases,the diagnosis was correct in all of them.Conclusion MR is perfect in the diagnosis of tumor thrombus in renal cell carcinoma oncondition that the renal vein is clearly defined by MR.It can be considered as the method of choice in the evaluation of tumor thrombus inrenal cell carcinoma.
4.Screening of Coronary Artery Calcification Using Digital Radiograhpy with Dual-energy Subtraction Chest:Compared with Multi-detector Row CT

Objective To evaluate the diagnostic accuracy of dual-energy subtraction chest digital radiography(DR) for detecting coronary artery calcification as compared with multi-detector row CT(MDCT).Methods 35 patients underwent dual-energy subtraction chest DR and ECG-gated MDCT for detection of coronary artery calcifications.The radiation dose of both DR and CT were noted respectively.Two senior and two junior radiologists reviewed the radiogram and assessed the calcifications in LAD,LCX and RCA totaling 105 vessels.ROC curve plotting were used for evaluation with CAC calculated from MDCT as the gold standard.Paired t test was calculated to compare the different radiation dose between DR and CT.Results 27.6% vessels(29/105) showed calcification on DR.The positive incidence in LAD was 48.5%(17/35),LCX 22.8%(8/35),and RCA 11.4%(4/35).The ROC area was 0.866,0.854,0.725,0.642 respectively,with a CAC score of more than 300.The average radiation dose was 0.469?0.22 mGy for DR,and 12.29?1.40 mGy for MDCT,showing significant differences.Conclusion Chest DR with dual energy subtraction can be used for screening of coronary artery calcification with relatively higher sensitivity for LAD.
5.Detection and grading of endolymphatic hydrops in Ménière's disease using 3D FLAIR MRI.

OBJECTIVE:
To evaluate the extent of endolymphatic hydrops as shown by three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging (3D FLAIR MRI) performed 24 hours after bilateral intratympanic gadolinium administration and discuss the positive rate of endolymphatic hydrops in vestibule and cochlea.
METHOD:
Twenty-four hours after bilateral intra-tympanic 8 times diluted gadolinium administration, three-dimensional fluid-attenuated inversion recovery MRI, using a three-Tesla unit, was performed in 48 patients, and then assessed the extent of endolymphatic hydrops in the MRI images.
RESULT:
Forty-eight patients showed different levels of enhancement of perilymth in the inner ear. In these patients, obvious signs of endolymphatic hydrops were visualized in vestibule,including 8 slight hydrops, 17 mild hydrops and 23 severe hydrops with a diagnostic rate of 83.3%. There were almost no complications in all 48 patients after bilateral intra-tympanic injection except short vertigo in some of them.
CONCLUSION
3D FLAIR MRI resonance imaging has a high positive diagnostic rate in assessing endolymphatic hydrops of Ménière's disease and provides direct imaging evidence for diagnosing Ménière's disease.

6.Arsenic trioxide inhibits phosphorylation of P27~(kip1) threonine residue 187 in human hepatic carcinoma cells

Objective To investigate the relationship between growth inhibiting effect of arsenic trioxide(As_2O_3) and phosphorylation of P27kip threonine residue 187(P27T187) in human hepatocellular carcinoma(HCC) cell line SMMC-7721.Methods SMMC-7721 were treated for 72 h with 2 ?mol/L As_2O_3.The cell growth inhibition was detected by cell counting and the cell cycle was detected by flow cytometry(FCM).The expression and localization of P27,T187 phosphorylated P27(p-P27T187) were detected by Subcellular Fractionation,Western blot and immunoflurescence.Results As_2O_3 significantly inhibited the proliferation of SMMC-7721 cell and cell cycle was arrested in G2/M.A significant decrease in p-P27T187 expression and a reciprocal increase in P27 expression were found in 2 ?mol/L As_2O_3-treated SMMC-7721 cell.Meanwhile,As_2O_3 decreased the protein levels of Cdk2 and cyclinE.The location of P27 was transferred from cytoplasm to nuclei and the expression of p-P27T187 was decreased in nuclei.Conclusion As_2O_3 inhibits the phosphorylation of P27T187,thereby promoting P27 accumu-lation in SMMC-7721 cell nuclei,inducing cel1 cycle arrest and growth inhibition.
7.Expression of JAB1 and P27~(kip1) in liver tissue of patients with hepatocellular carcinoma and its clinical significance

Objective To compare pediatric patients with periventricular leukomalacia (PVL) with normal children by using diffusion tensor MR imaging. Methods Diffusion tensor images were obtained in 15 pediatric patients with PVL and 15 age-matched normal children. 9.Measurement of alveolar surfactant after bleomycin-induced pulmonary injuries.

OBJECTIVETo observe the ultrastructures of rat alveolar type II cells and change of composition of phospholipid(PL) and content of protein in pulmonary surfactant(PS), to investigate the relation between change in composition of PL and activity of alveolar type II cells.
METHODSThe rats lung injury models were made by intratracheally instilling bleomycin(BLM) (4 mg/ml, 5 mg/kg). 28 rats were divided into four groups: 3-day group, 7-day group, 14-day group and 28-day group. Preparations of each group were stained histochemically and examined by electron microscope, content of PL in BALF, composition of PL and content of protein of each group were determined respectively.
RESULTS(1) Rats lungs in experimental groups were found that PS lost continuously, appeared homogenous and chorionic, dropped in the pulmonary alveolies. 3-day group was more apparent. Ruthenium red attaching on pulmonary surfactant was thicker in 3-day group, and the colour deeper, no difference in 7-day group and 14-day group, thinner in 28-day group. Content of PL in PS of BALF was increasing. Content of phosphatidylglycerol(PG) increased in 3-day group, decreased in 7-day, 14-day and 28-day group. The change of content of phosphatidylinositol(PI) was reversed. (2) Alveolar type II cells degenerated, necrotized, even disintegrated in 3-day group and 7-day group. 3-day group was more apparent. Proliferations of alveolar type II cells were found in each group, 7-day group was more apparent. We found that type II cells transformed to type I cells in 14-day group, extended and attached on bare basement membrane. Content of protein in PS was the highest in 3-day group, almost equal to the content of the control group in 28-day group.
CONCLUSIONMorphologic change and alternation of quality and quantity of PS after bleomycin-induced pulmonary injures specifically reflect the activity of alveolar type II cells. Measuring content of PL in BALF is one of simple and feasible method judging activity of alveolar type II cells when lungs of the rats are injured early by bleomycin.

10.Clinical observation of antimicrobial dressings tamponade combined with seal negative pressure drainage in the treatment of 52 cases of necrotizing fasciitis

Objective To observe the effect of antibacterial dressing tamponade combined with seal negative pressure drainage in the treatment of necrotizing fasciitis .Methods 52 patients with necrotizing fasciitis were selected . After full scavenging of fasciitis and necrotic tissue,then filled the whole wound with a kind of dressings with antibacterial properties-Nano silver antiseptic dressing,and a negative pressure drainage tube was placed at the bottom of the wound.Then the wound was closed with a transparent paste ,and the air was cut off,in order to make the antibacterial dressing could not only anti bacteria ,but also play a role in the growth of the wound ,and at the same time,the tissue leachate in the wound was drained through the negative pressure tube in time ,so as to accelerate the wound healing and reduce the use of antibiotics .Results Among the 52 cases,the rest were cured except for 1 case of individual cause of abandonment of treatment died of toxic shock .,the average hospitalization time was (29.0 ±15.3)days,days of using antimicrobial was (3.8 ±1.6)days,90% cases stopped using antibiotics within 1 week.Conclusion Antibacterial dressing tamponade combined with seal negative pressure drainage in the treatment of necrotizing fasciitis can reduce the switching frequency,reduced antibiotic using intensity,shorten the healing time,it is a kind of very good method.
