1.Application of risk management in quality control of medical apparatus and instruments of operating room

Objective:To explore the application effect of risk management in quality control of medical apparatus and instruments of operation room.Methods: 172 pieces of medical apparatus and instruments were divided into risk management group (n=86) andcommon management group (n=86). A series of indicators, such as effective acceptance rate, qualified cleaning rate, completed supporting facility, specific aim rate, deficiency rate of quantity, error rate of preparation, wet pack rate of sterilization, lost rate of post surgery and the satisfaction degree of physician, were applied to complete statistic analysis.Results: The effective acceptance rate, qualified cleaning rate, completed supporting facility rate and specific aim rate of risk management group were significantly higher than that of common management group (x2=5.02,x2=7.38,x2=9.35,x2=11.14,P<0.05). The deficiency rate of quantity, error rate of preparation, wet pack rate of sterilization, lost rate post surgery of risk management group were significantly lower than that of common management group (x2=6.63,x2=9.21,x2=11.34,x2=13.28,P<0.05). And the satisfaction degree of physician of risk management group (97.7%, 84/86) was significantly higher than that of common group (73.3%, 63/86) (x2=12.83, P<0.05).Conclusion: The application effect of risk management is better than that of common management in the quality control of medical apparatus and instruments in operation room.
2.The new applied research on the multifunctional tubular pipeline in femoral artery and vein

Objective:To do research on designing the new applied multifunctional integrated tubular pipeline in femoral artery and vein which combined with blood transfusion, fluid infusion, blood sampling, administration, measurement of central venous pressure. Methods: Pipeline is consisted of three parts, which are blood transfusion part and fluid infusion part, including three way switch and three way extended tube. The first way connected with the injection is for blood sampling and doses, the second way connected with the central venous is for measurement of central venous pressure, and the third way connected with the blood transfusion set and fluid infusion set is for blood transfusion and fluid infusion. In a standard thoraco-abdominal combined injury and revivification of 31 beagle, compare the time effect. Results: In the injury and revivification test, the average time cost in 532 times nursing care operation with the multifunctional integrated tubular pipeline is 19+5.1 seconds, the average time cost in 31 times individual nursing care operation of 5 procedure is 80+12 seconds. The multifunctional integrated tubular pipeline can quickly and effectively work on blood transfusion, fluid infusion, blood sampling, administration, measurement of central venous pressure. Conclusion:The multifunctional integrated tubular pipeline in femoral artery and vein saves time and human resources and reduces the infection rate in repeated venous puncture and works quickly and conveniently on animal experiment and acute fatal injury, so it is worthy a further clinical application.
3.A study on the expression of survivin, livin and XIAP mRNA in peripheral blood of patients with gastric cancer

Objective To investigate the mRNA expression of survivin, livin and XIAP gene in peripheral blood of patients with gastric cancer and its relationship with clinico-pathological features.Methods This study included 50 patients with gastric cancer and 20 healthy donors. The expression of survivin, livin and XIAP gene was detected by reverse transcription-quantitative polymerase chain reaction (RT-QPCR) using a molecular beacon probe, while recombination plasmid containing the sequence of survivin, livin and XIAP was standard. The relationship between copies of survivin, livin and XIAP gene expression in peripheral blood with gastric cancer and clinical data was analyzed. Results A linear standard curve was obtained between 103 ~ 1010 copies. The copies of survivin, livin and XIAP mRNA in peripheral blood of patients with gastric cancer did not correlate with gender, age, and histological types ( P > 0.05). There were positive relationships between copies of survivin, livin and XIAP gene with lymph node metastasis and TNM stage (P <0.05 ). The expression of survivine, livin and XIAP was all negative in peripheral blood of healthy people. 52% (17/33) of patients suffered from recurrence or metastasis who had positive expression of survivin and/or livin and/or XIAP mRNA, while it was 18% (3/17)among the negative survivin and/or livin and/or XIAP mRNA caces ( P < 0.05). Conclusions The expression of survivin, livin and XIAP mRNA can be used to detecte micro-metastasis in peripheral blood circulation of gastric cancer.
4.Clinical and pathological influence of high uric acid on idiopathic membranous nephropathy

Objective To study the clinical and pathological influence of high uric acid on idiopathic membranous nephropathy (IMN) Methods A retrospective study with 314 patients diagnosed with IMN from January 2014 to June 2016 was conducted and the clinical pathological influence of high uric acid on IMN was analyzed.Results (1) Of the total,the prevalence of hyperuricaemia patients was 23.2% (73 cases);(2) The difference of age,course of the disease,blood pressure and symptoms between hyperuricaemia IMN patients and IMN patients with normal uric acid was statistically significant (P < 0.05);(3) Laboratory test indexes such as blood lipid and renal function between hyperuricaemia IMN patients and IMN patients with normal uric acid indicated statistical significance (P < 0.05);(4) The pathological damage was aggravated in hyperuricaemia IMN patients (P < 0.05).Conclusion High uric acid can enhance the clinical and pathological damage of IMN,and comprehensive and effective treatments should be conducted to delay the development of disease.
5.Effects of BCL6 B on proliferation and migration of human colorectal car-cinoma LoVo cells and its potential mechanism

AIM:To detect the endogenous expression of B-cell leukemia/lymphoma 6 member B (BCL6B) in FHC and LoVo cells, and to investigate the effects of BCL6B on proliferation and migration of LoVo cells for further explo-ring the underlying mechanism .METHODS:The endogenous expression of BCL 6B in the FHC and LoVo cells was detec-ted by RT-PCR and Western blot .The methods of MTT assay , colony formation assay , wound healing assay and Transwell chamber experiment were employed to examine the biological functions of BCL 6B in the LoVo cells.The mRNA and protein levels of BCL6B, cyclin D1 and matrix metalloproteinase-9 ( MMP-9) were determined by RT-PCR and Western blot , re-spectively.The level of phosphorylated protein kinase B (p-AKT) was detected by Western blot.RESULTS:BCL6B ex-pression was notably repressed in the LoVo cells as compared with the FHC cells , which were significantly increased by transfection with pcDNA3.1-BCL6B.The abilities of proliferation and migration of the LoVo cells at 72 h were inhibited by 28.33%(P<0.01) and 36.11%(P<0.05) in BCL6B group.The mRNA levels of cyclin D1 and MMP-9 in the cells of BCL6B group were decreased by 39.90%(P<0.01) and 77.36% (P <0.05), and the protein levels of cyclin D1, MMP-9 and p-AKT were reduced by 44.00%(P<0.05), 47.06%(P<0.01) and 32.88% (P<0.05), respectively. CONCLUSION:BCL6B inhibits proliferation and migration of the LoVo cells , and the PI3K/AKT signaling pathway is in-volved in this process .
6.Analysis of influence factors on hospitalization expenses with hip arthroplasty

Objective To discuss the effect of rehabilitation nursing for total knee arthroplasty by discussing hospitalization expenses and its factors of rehabilitation nursing.Methods 305 patients who were underwent total knee arthroplasty from 2011 to 2013 were recruited and recovered according to rehabilitation nursing and training requirements established by our hospital.Patients admitted in 2011 were designated as control group.The comparison of American knee society knee score and knee rang of motion of patients were made and analyzed.Results The annual comparison of sores of pre-and post-operative KSS (t=5.30,7.99,11.20),and knee ROM of patients (t=8.21,4.57,7.86) showed that the difference was statistically significant (P<0.05).The total costs and rehabilitation nursing expenses inreased year by year (P<0.05).The proportion of rehabilitation and rehabilitation nursing expenses each year were 5.77% (3 157/54 679),4.60% (2 847/61 831),5.15% (3 341/64 930) and 0.51% (280/54 679),0.64% (393/61 831),0.52 %(338/64 930),respectively.Conclusions Pre-and post-operative correct rehabilitation exercises are one of the most important guarantee of curative effect.There is a lower percentage of rehabilitation nursing expenses in total costs,and the value of the nursing staff has not been fully reflected.
7.Effects of brucine on chronic myeloid leukemia cell line HL-60

Objective To investigate the proliferation inhibition and the apoptosis induction effect of brucine on human chronic myeloid leukemia cell line HL-60 cells.Methods HL-60 cells were exposed to various dosages of brucine 24,48,72 h respectively,MTT method was used to assay the growth inhibition effect of brucine on HL-60 cells and the IC50 of brucine was evaluated at the same time.The morphology was observed by AO/EB stains.The cell apoptosis and cell cycle was tested by flow cytometry with Annexin V-FITC/PI double staining and PI labeling respectively.Results The results indicated that the brucine displayed significant anti-proliferative effect on HL-60 cells in a dose-and time-dependent manner,with IC50 value of 211.8 μg/ml(24 h),107 μg/ml(48 h),83 μg/ml(72 h)respectively.The most significant inhibition was observed at 320 μg/ml for 48 h.In this condition,apoptosis morphology was induced by brucine with nuclear chromatin condensation,most of the nuclears were orange stained and condensation-like or bead-like,which was consistent with the Annexin V-FITC/PI results.The cell apoptosis rates were(2.1±1.1)%,(21.3±1.2)%,(38.6±1.3)%,(58.5±4.1)%,(75.3±0.87)%and(66.2±0.75)%in different dose of brucine,respectively.At the same time,the cell cycle analysis results showed that the cell ratio in G0/G1 phase was decreased while in G2/M and sub-G0 phases was increased,comparing with blank control group.Conclusion Brucine can inhibit cell growth dramatically,which may be related to the cell apoptosis and the cell cycle arrest.
8.Feasibility study of induction of experimental chronic pancreatitis with L-arginine

Objective To investigate the feasibility of induction of experimental chronic pancreatitis rat model with L-arginine.Methods Animals were randomly divided into control group,arginine 12 h group,arginine 24 h group and arginine 7 d group with 10 rats in each group.L-arginine solution was intraperitoneally injected twice with an interval of 1 h.Serum amylase and glucose levels at corresponding time points were detected and histopathological scores of pancreas were evaluated.Collagen in pancreas was stained with Van Gieson method.Results Serum amylase levels were (1 634±890 ) U/L,( 3 872±2 676 ) U/L,( 3 307±2 197)U/L and (1 561±304) U/L in control group,arginine 12 h group,arglnine 24 h group and arginine 7 d group,respectively.The serum amylase level in arginine 7 d group was significantly lower than those in arginine 12 h group and arginine 24 h group (P < 0.05 ).There was no significant difference in serum glucose level among all the groups.Histopathological scores were 0.8±0.4,5.1±2.6,6.5±2.2 and 4.5±1.6,respectively.The histopathological score of arginine 7 d group was significantly lower than those in arginine 24 h group (P < 0.05 ).Obvious collagen could be found in pancreatic parenchyma in arginine 7 d group,while little collagen was found in pancreatic tissue in control,arginine 12 h and arginine 24 h groups.Conclusions Injection of L-arglnine induced fibrosis in pancreatic parenchyma and proliferation of tubular complex 7 days later,and it could be used for chronic pancreatitis model induction.
9.Effect of jejunal casein perfusion on pancreatic exocrine secretion in experimental acute necrotic panereatitis rats

Objective To investigate the effect of jejunal casein perfusion on pancreatic exocrine secretion in experimental acute necrotic pancreatitis (ANP) rats and analyze the neuromechanism that may be involved. Methods 30 SD rats were randomly divided into three groups (control group, ANP group and ANP jejunal nutrition group). Experimental ANP was induced by intra pancreatic duct injection of sodium taurocholate (STC). Animals in ANP jejunal nutrition group were given jejunal casein perfusion 24h after model induction, while control group and ANP group received jejunal saline perfusion. Pancreatic juice was collected every 15 min for six times and the volume of pancreatic juice and protein in pancreatic juice were detected. After jejunal nutrition c-Fos expression in nucleus tractus solitarii (NTS) was determined by immunohistochemistry method in three groups. Results There was no significant difference between the volume of pancreatic juice at different time points in ANP group and ANP jejunal nutrition group, however, these parameters were significantly lower than that of control group (P < 0. 05). There was no significant difference among the 3 groups in the protein level in the pancreatic juice during jejunal nutrition infusion, however, during the periods of 0 ~ 15 min, 15 ~30 min, 30 ~45 min and 75 ~90 min, the protein levels in the pancreatic juice in ANP and ANP jejunal nutrition group were lower than that of control group (P < 0.05). After jejunal perfusion, c-Fos expression was found in ANP jejunal nutrition group but not found in ANP and control groups. Conclusions Jejunal casein perfusion enhanced NTS c-Fos expression, but did not increase the volume of pancreatic juice and protein.
10.A case of anhidrotic ectodermal dysplasia with immunodeficiency

To report a case of 16-month-old boy with anhidrotic ectodermal dysplasia with immunodeficiency who experienced disseminated herpes simplex infection. From 2 months of age, the patient experienced multiple pyrexial episodes of undetermined origin, which responded well to anti-inflammatory agents after undressed. Abnormal sweat with dry skin was noted; therefore, the skin biopsy of right axilla was performed at 7 months of age, and suggested a diagnosis of anhidrotic ectodermal dysplasia. Since 6 months of age, he developed recurrent upper respiratory infections and 2 episodes of pneumonia. Twenty days before, several glossal erosions occurred in the patient, supervened by painful and erosive eruptions and numerous blisters around the mouth and both hands with hyperpyrexia. Four days before, the patient was transferred to the department owing to skin lesion exacerbation. Cutaneous examination showed multiple crested or ulcerated plaques distributed eriorificially (mouth and nasal cavity) on the face. Several irregular, demarcated ulcers were scattered on the buttocks, scrotum and lower limbs, surrounded by grouped and umbilicated vesicles arising on erythema. Both hands were swelling, crusting and painful. Dentition was abnormal, and the patient had only 2 upper conical incisors. Routine investigation revealed that white cell count and C-reactive protein extremely elevated. Immunologic profile showed an abnormal distribution of lymphocyte subsets with decreased CD3+ T cells, CD8+ T cells and NK cells. Serum IgM level was slightly low. IgM antibodies to herpes simplex virus type 1 (HSV-1) were detected by serological testing. Based on the above-mentioned features, a diagnosis of anhidrotic ectodermal dysplasia with immunodeficiency and disseminated herpes simplex infection was confirmed. The patient was resolved favourahly after intravenous ganciclovir and antibiotics for 3 weeks without relapse of skin lesions.
