1.Changes of serum adiponectin levels in patients with essential hypertension and the correlation with carotid artery intima-media thickness

Objective To investigate the changes of serum adiponectin (APN) levels in patients with essential hypertension (EH) and the correlation with carotid artery intima-media thickness (IMT).Methods Sixty patients with EH(EH group) and 30 healthy people (control group) were studied,the EH patients were divided into 3 subgroups with 20 cases each according to the standard of China guideline for hypertension prevention and control (2005).The serum APN level was detected by double antibody sandwich ABC enzyme-linked immunosorbent assay,the blood lipids levels [total cholesterol (TC),triglyeride (TG),low density lipoprotein-cholesterol (LDL-C) and highdensity lipoprotein-cholesterol (HDL-C)]were detected by automatic biochemical analyzer,and the carotid artery IMT was detected by color Doppler ultrasonography.Results The serum APN levels and carotid artery IMT in EH group and 1,2,3 subgroup were significant differences compared with those in control group [(3.94 ± 1.28),(5.25 ± 1.69),(4.01 ±1.27),(2.56 ± 0.87) mg/L vs.(7.63 ± 2.50) mg/L; (1.30 ± 0.42),(0.94 ± 0.30),(1.32 ± 0.44),(1.65 ±0.51)mm vs.(0.69 ± 0.22) mm,P < 0.05].The comparison between two subgroups,along with elevated blood pressure levels,APN levels gradually decreased,and carotid artery IMT gradually increased (P <0.05).The serum TC,TG,LDL-C and HDL-C levels in EH group and 1,2,3 subgroup were significant differences compared with those in control group [TC:(6.53 ± 2.09),(5.14 ± 1.66),(6.59 ± 2.20),(7.86 ±2.42) mmol/L vs.(4.17 ± 1.32) mmool/L;TG:(2.15 ± 0.69),(1.72 ± 0.55),(2.07 ± 0.69),(2.65 ± 0.82)mmol/L vs.(1.38 ±0.44) mmol/L;LDL-C:(3.78 ± 1.21),(3.12 ± 1.01),(3.74 ± 1.25),(4.48 ± 1.38)mmol/L vs.(2.46 ±0.78) mmol/L;HDL-C:(0.96 ±0.31),(1.39 ±0.45),(0.85 ±0.28),(0.64 ±0.20)mmol/L vs.(1.69 ± 0.54) mmol/L,P < 0.05].The comparison between two subgroups,along with elevated blood pressure levels,TC,TG and LDL-C levels gradually increased,and HDL-C gradually decreased (P <0.05).There was negative correlation between the serum APN level and carotid artery IMT (r =-0.435,P<0.01),and there was correlation between the serum APN level and blood lipids levels (TC,TG,LDL-C and HDL-C) (P < 0.01).Conclusions The APN plays an important role in the occurrence and development of EH,it is closely related to the carotid artery IMT thickening and lipid metabolism disorders.Increasing the lipid-lowering drugs may improve the efficacy of antihypertensive therapy in patients with resistant EH.

Febrile infection -related epilepsy syndrome(FIRES)is a newly named entity of epileptic encepha-lopathy in recent years.As one of the hot spots studied by domestic and foreign scholars,the etiology and pathogenesis of FIRES,which has a poor treatment and outcome,are undefined.It manifests with refractory seizures and status epilep-ticus in previously healthy school -aged children after banal febrile illness.In addition,with the negative response to currently available treatments,there are a poor prognosis and a high mortality rate in FIRES.Survivors are typically left with refractory partial epilepsy and severe cognitive impairment.Through domestic and foreign literature,this article re-views the potential pathogenesis,clinical features,treatment and prognosis of the syndrome,and provides theoretical ba-sis and new ideas for clinical diagnosis and treatment.
5.Clinical study on acupoint massage in improving cognitive function and sleep quality of elderly patients with hypertension

Objective:To investigate the effects of acupoint massage on cognitive function and sleep quality in elderly patients with hypertension. Methods:A total of 68 elderly patients with hypertension were enrolled, and their ages were all over 60 years old. They were divided into a control group and a treatment group by simple random method, with 34 cases in each group. Patients in the control group were treated by routine psychological health guidance and sleep knowledge education, while patients in the treatment group were treated by acupoint massage on the base of the interventions given to the control group. Both of their treatment courses were 3 months. Pittsburgh sleep quality index (PSQI) scores and mini-mental state examination (MMSE) scores before and after treatment were recorded and compared. Results:Two groups’ PSQI scores before treatment had no significant difference (P>0.05). After 3 months of treatment, their PSQI scores were statistically different (P<0.05). There was also no between-group statistical difference in MMSE score before treatment (P>0.05), but there was a significant difference after treatment (P<0.05). Conclusion:Acupoint massage for elderly patients with primary hypertension can improve their sleep quality and enhance their cognitive function, thus it is worthy of being applied in clinical nursing work.

7.Migration and differentiation of neural stem cells derived from a human fetus brain in developmental cerebrospinal fluid

OBJECTIVE: To investigate the effects of human cerebrospinal fluid (CSF) during development phase on migration and differentiation of fetal brain neural stem cells (NSCs).METHODS: Fetal brain cells of gestational age of 16 weeks that were frozen in liquid nitrogen were obtained, resuscitated and incubated in DMEM/F12 medium containing epithium growth factor (EGF), basic fibroblast growth factor (bFGF), B27 and N2. The neurospheres cultured for 14 days were obtained. CSF was absorbed from the subarachnoid cavity and brain ventricle in the embryonic group. CSF was collected by lumbar puncture or ventricular puncture in the child group. The neurospheres cultured for 14 days were transplanted into the pure CSF in an incubator containing 5% CO_2 at 37 ℃. Cellular migration and growth of neurospheres in CSF were observed. Effects of CSF on neural cell differentiation were identified by immunofluorescence. RESULTS: Neural stem cells in the form of neurospheres derived from fetal brain were inoculated into the pure CSF, and cell migration were commonly observed besides few of neurospheres in child CSF culture at 6 hours following culture. Surrounding cells of neurospheres extended processes, forming cell cord that became cell webs after extension. Compared with the embryonic group, positive rate of glial fibrillary acidic protein was significantly increased in the children group (P < 0.01), but positive rates of nerve fiber and nestin were significantly decreased (P < 0.01). In addition, galactocerebroside-positive cells were only found in 3 baby CSF cultures. CONCLUSION: There existed significant affections on both migration and differentiation of human neural stem cells when cultured in pure CSF with different developmental phase, suggesting that CSF is one of major niche factors for central neural system development.
8.Association of glycemic index of the mixed food with glucose and lipid metabolism index in elderly patients with type 2 diabetes mellitus

Objective To investigate the relationship between glycemic index (GI) of mixed foods and glucose and lipid metabolism index in elderly patients with diabetes mellitus, and to analyze the effects of CI response with regard to sex and obesity. Methods Forty-three patients with diabetes mellitus were divided into two groups with randomly crossover methods: high CI group and low GI group. Mixed meals were given for ten weeks (washing time:2 weeks). The biochemistry index and anthropometric index at baseline,before and after the first and second phase were measured. At the same time,fasting blood glucose(FBG)and postprandial 2 h blood glucose (2hBG) were supervised thrice a week and the daily energy, three macronutrients, and GI were calculated during trial. Results GI was positively correlated with FBG, 2hBG, homeostasis model assessment of insulin resistance ( HOMA-IR) ,body mass index ( BMI), and low-density lipoprotein cholesterol ( r = 0. 097-0. 423, P = 0. 000 1-0.01). GI value was a strong associated factor for FBG and 2hBG in overweight male patients. Conclusions The GI of mixed foods can better reflect the level of glucose response, especially in obese men. And it had good correlations with other metabolic indexes.
9.Effect of resveratrol on glucose-stimulated insulin secretion in isolated rat pancreatic islets

Isolated freshly rat islets were transferred to 24-well plates and incubated with different concentrations of glucose or resveratrol for 1 or 24 h.The results showed that resveratrol dose-dependently inhibited glucose-stimulated insulin secretion from isolated rat islets after 1 h incubation,with 10%,35%,and 80% (P<0.05 or P<0.01) decrease at the concentrations of 1,I0,and 100 μmol/L.10 μmol/L resveratrol decreased the intracellular calcium concentration by 60% (P<0.05).After incubation for 24 h,resveratrol increased palmitatesuppressed insulin secretion to 75% (P<0.01) of control.These results suggest that resveratrol acutely inhibits insulin secretion from primary pancreatic islet via regulating intracellular calcium ion concentration,and in the long run resveratrol may protect β-cells from lipotoxicity.
10.Risk factors of mortality in painful chronic pancreatitis

Objective To investigate the survival of patients with painful chronic panereatitis and evaluate the risk factors for mortality in these patients.Methods This was a historical cohort study,subjects ≥18 yr with painful chronic pancreatitis who were treated in our center from Feb.1997 to July 2007 were enrolled.A life-table method was used to estimate the probability of the cumulative survival rate.COX proportional-hazards model was used for multivariate analysis.Results Follow-up data were obtained from 346 patients (87.2%) with the ratio of male/female was 2.4:1.The age of first admission and first symptom onset was (47±14)years and (43±15 ) years.The mean follow-up period was ( 34.3±27.1 ) months.Alcoholic origin accounted for 22.2% of all the patients,while biliary origin was 26.0%.The overall mortality rate was 9.8% (34/346),which happened (62.5±61.1 ) months after abdominal pain was present.The causes of mortality included pancreatic cancer and cancers of other sites.The cumulative survival rate estimated at 2 years was 96.3%,at 5 years 93.6% and at 10 years 86.4%.COX proportional-hazards model suggested age at onset (≥51 years),no improvement of abdominal pain or increased frequency of pain,active smoking,and no diarrhea were associated with increased mortality,and the corresponding hazard rates (HR) were 3.4,3.5,4.2,2.8 and 17.7,respectively.Conclusions The mortality rate of painful chronic panereatitis in China was lower than those reported outside China.The main cause of mortality was pancreatic cancer.Great caution shall be placed on patients with age at onset ≥51 yr,no improvement of abdominal pain,increased frequency of pain,active smoking and no diarrhea.
