Objective To evaluate the clinical value of hysteroscopy before in vitro fertilization and embryo transfer(IVF-ET).Methods From December 2003 to May 2005,130 patients received hysteroscopy in our hospital because of IVF-ET failure.Their intrauterine diseases were treated by hysteroscopy,and the abnormal tissues were examined pathologically.Results Among the 130 cases,87 had intrauterine diseases(87/130,66.9%) including endometritis in 40(40/130,30.8%),endometrial polyps in 23(23/130,17.7%),intrauterine adhesion in 20(20/130,15.4%),and submucous leiomyoma in 4(4/130,3.1%).In these patients,67 underwent IVF/ICSI or frozen embryo transfer within one year after the hysteroscopy,of which 20 patients achieved pregnancy.The clinical pregnancy rate was about 30%.The other 20 cases did not repeat IVF-ET in 10-20 months after the hysteroscopy,and had no pregnancy without contraception.Conclusion Pre-transfer hysteroscopy is helpful to avoid IVF-ET failure owing to intrauterine diseases.

Four hundred and five pregnant women whose husbands had positive HBeAg,HBsAg and anti HBc were recruited in the study.Among them.218 women with positive HBsAb were completely randomized into group A and group B,while 187 cases with negative HBsAb formed group C.Women in group A and group C were injected with hepatitis B immunoglobulin(HBIG)before delivery.All newborn babies in 3 groups received routine HCV vaccination.in addition babies in group A and group C also received HBIG iniection within 24 h after birth.The HBV infeetion rate of newborns was 12.1%in group A,12.8%in group B and 23.0%in group C respectively.

This study was to investigate the role of urethral catheter balloon dilatation(F16)in the treatment of intrauterine adhesion after hysteroscopic surgery.A total of 523 patients with severe intrauterine adhesion(IUA)following hysteroscopic surgery underwent urethral catheter balloon dilatation during April 2000 and December 2005.During 3～15 months'follow-up,normal menses were seen in 340 patients (65.0％),nearly normal menses in 165 patients(31.5％),and menoschesis in 18 patients(3.4％).302 patients showed normal uterine cavity with no recurrent endometrial adhesion,203 patients(38.8％)were found slight IUA,and only 18 patients(3.4％)still presented with severe adhesion.No operative complication was reposed.and the pregnant rate was 47.6％.In summary.urethral catheter balloon dilatation.based therapeutic strategies is safe.effective and beneficial.

Objective:To evaluate the efficacy and safety of temporary loop ligation of the infrarenal abdominal aorta for control ofintraoperative blood loss in patients with pernicious placenta previa.Methods:We retrospectively analyzed the clinical data of 14 patients with pernicious placenta previa,who underwent temporary loop ligation of the infrarenal abdominal aorta for control of blood loss during cesarean section between July 2013 and December 2014.Results:Eight patients received conservative management to preserve the uterus and 6 patients underwent cesarean hysterectomy.The occlusion time of the abdominal aorta was (31.42±12.67) min.The average estimated intraoperative blood loss was (1 117.85±745.13) mL.The volume of packed red blood cell transfusion was (3.91±3.24) units,and the volume of fresh frozen plasma transfusion was (192.85± 156.71) mL.Post-operative histologic diagnosis revealed 6 cases of placenta percreta,4 of increta,3 of accreta and 1 non-creta.All patients experienced an uneventful postoperative recovery.Conclusion:In patients with pernicious placenta previa,temporary ligation of the infrarenal abdominal aorta provide a safe and effective means for controlling intraoperative hemorrhage during cesarean section.Additionally,the procedure may provide an opportunity to preserve fertility by avoiding a cesarean hysterectomy.

Objective To sequence follicle stimulating hormone receptro (FSHR) promoter of the ovarian granulocyte and initially research the molecular mechanism of the poor ovarian response. Methods To study the relationship between FSHR promoter mutation of ovarian granulocyte and ovarian respone. The 263 bp DNA fragments before FSHR 5'initiation site in 70 cases of patients with poor ovarian respone and 88 cases of patients with ovarian normal respone who were in the cycle of IVF-ET were sequenced, Results There were 63 cases which occurred 29th site G → A point mutation in 158 women and the mutation rate was 40. 0%. Mutation of G→A can weaken promoter activity, so that ovarian granulocyte poor respone to FSH.

Objective To evaluate the influence of Eperythrozoon infection on human and mouse erythrocytes and to explore the pathogenesis of Eperythrozoonosis. Methods The specific gene fragment of Eperythrozoon was detected by polymerase chain reaction (PCR) from the venous blood samples of five patients infected with Eperythrozoon. The complement receptor type I (CD35) expression on erythrocytes of these five patients was determined by flow cytometry. Thereafter, the Eperythrozoons were purified from human samples and injected into mice through the tail veins. Blood smear microscopy, PCR and transmission electron microscopy were used to assure the successful infection. The hematological indicators of human and mice, such as red blood cell (RBC) count,hemoglobin (Hb) content, hematocrit and superoxide dismutase (SOD) were evaluated. All results were analyzed by t test. Results More than 80% of treated mice were confirmed to be infected with Eperythrozoon successfully. A fragment of 801 bp specific gene of Eperythrozoon was detected by PCR in samples from both infected patients and infected mice, which were not detected in samples from healthy control people or control mice. CD35 was highly expressed on the erythrocytes of infected patients, but not expressed on the erythrocytes of infected mice. Both RBC counts and Hb content dramatically decreased in infected patients and infected mice. Hematocrit and the activity of SOD also slightly decreased in infected patients and infected mice. Conclusions Eperythrozoon can spread between human and mice and destroy erythrocyte structure. Eperythrozoon can upregulate CD35 expression in human, but there is no CD35 expression in mice.

Objective To investigate the therapeutic effects of hemin, an inducer of heme oxygenase, in a rat model of gestational hypertension and explore the possible mechanism. Methods Eighteen pregnant SD rats at day 12 of gestation were randomized equally into gestational hypertension model group, hemin treatment group, and normal pregnancy (control) group. In the former two groups, the rats were subjected to daily nitro-L-arginine methyl ester (L-NAME, 80 mg/kg) gavage since gestational day 14 for 7 consecutive days to induce gestational hypertension; saline was administered in the same manner in the control rats. The rats in hemin group received daily intraperitoneal injection of hemin (30 mg/kg) starting from gestational day 16. HO activity and carboxyhemoglobin (COHb) level in rat placental tissue were detected with spectrophotometric method, and soluble vascular endothelial growth factor receptor-1 (sFIt-1) and vascular endothelial growth factor (VEGF) level in the placental tissue homogenate supernatant were detected using ELSIA. Results At gestational day 20, the blood pressure and 24-h urinary protein were significantly higher in the model group than in the other two groups (P<0.05), and were higher in hemin group than in the control group (P<0.05);HO activity and COHb content in the placenta tissue were the lowest in the model group (P<0.05), and was lower in hemin group than in the control group (P<0.05). The level of sFIt-1 was significantly higher and VEGF level significantly lower in the model group than in the other two groups (P<0.05);sFIt-1 level remained higher and VEGF lower in hemin group than in the control group (P<0.05). Conclusion Hemin can reduce blood pressure and urinary protein in rats with gestational hypertension possibly by up-regulating HO activity, enhancing carbon monoxide production, reducing sFIt-1 and increasing VEGF in the placental tissue.

Postpartum depression(PPD) is a common emotional disorder in the puerperium, which has negative impact on women, infants and family. There is growing evidence that abnormal concentration of a number of nutrients (including polyunsaturated fatty acids, vitamin D and homocysteine) is associated with depression in postpartum population. Further studies on the mechanisms for the functions of nutrients and regulation of nutritional states in the puerperium will be of great value in the prevention, early diagnosis and treatment of PPD.
