1.Total hip replacement in patients with congenital dislocation of hip

Objectiv e To evaluate the mid-term and long-term clinical outcome of total hip re-pl ace ment in patients with hip congenital dislocation.Methods From June198 3to Decem ber1998,36hips in32patients di agnosed as congenital disloca tion of hip(CDH),aged22to69years old(with the mean of 48.5years), were treated with total hip replacement (THR).Thirty-one hips of 28patient s were fol lowed-up with the duration of 9.5years(3.5to18years).Ac c ording to our system,the congen ital dys-pla sia of hip was divid ed into th ree degrees:the first degree was subluxation in15hips,the second degree was in termedi ate dislo cation in11hips,and the third degree was high dislocatio n in5hips.The fixation of the prosthetic compo nents was as follows :10cup s and12stems with cement,21cups and19stems with un-cement.In acetab ula r preparation,most of the patients with subluxation(13hips)underwent deep ening of the acetabu lum and were installed with larger cups;deepening of the acetabulum,smaller cup and auto femoral grafting on the su perior lip fixed w ith screws were performed in2hips with subluxation and all of hips with interm e di ate and high dislocation.Results The complications included intra operativ e lesser trochanter fracture in1case,deep vein thrombosis and thromboembolism of femoral artery in1case re spec-tively,limb discrepancy in3cases,asepti c loosening in4cases(1with trochanteric nonunion and migration,1with stem l oosening and2with both stem and cup loosening).The Harrisscore was85in th e other27hips(48scores before surgery).Con clusion Total hip replacement in patients with congenital dislocation of hip could be some what difficult be cause of the liability to complications.The techni cal difficulties en coun-t ered during surgery in cluded the correction of the length of bilateral lower extremi ty,the balance of the abductor muscles,the re lease of the soft tis sue,the dealing of the su perior segmental defect of acetabulum and selectio n of the compo nents.
2.Osteoporosis and vertebral fracture

Fracture is the most serious complication caused by osteopor osis. Vertebral fracture in osteoporosis is a strong risk factor for new fractur es as well as an important parameter in prognosticating new fractures. As a stro ng risk factor, vertebral fracture reflects the degeneration of the structure of bone and the reduction of bone mineral density (BMD). Therefore, it is more val uable in predicting the risk of refracture when the reduction of BMD and vertebr al fracture is reflected. It is more convenient and easier to evaluate vertebral fracture by radiographic grading and semiquantitative grading scheme without me asuring the vertebral height. To evaluate the drugs for osteoporosis objectively and accurately, their effects on both BMD and vertebral fracture should be take n into consideration. The ideal drug should be one that can increase BMD and red uce the risk of vertebral fracture.
3.Medium-term follow-up after mobile-bearing total knee arthroplasty

BACKGROUND:Mobile-bearing prosthesis has advantages in theoretic design, in vitro kinematics and abrasion, but it remains unclear whether its clinical outcomes are better than fixed-bearing prosthesis at present. OBJECTIVE:To evaluate the medium-term results of total knee arthroplasty using mobile-bearing prosthesis to provide clinical evidence for the choice of prosthesis. METHODS:The patients who suffered from osteoarthritis or rheumatoid arthritis and underwent total knee arthroplasty with PFC Sigma RP in Beijing Tongren Hospital from December 2006 to June 2009 were included in this study. The postoperative Knee Society Score, Knee Society Score Function Score, Patel ar Score and the Pain Score, range of motion, maximun flexion and extension angle were col ected and compared with pre-operation. The complications, such as infection, patel a clicking, polyethylene insert dislocation, and deep vein thrombosis were recorded after replacement. The anterior-posterior, lateral and Merchant position X-ray images were taken to evaluate the tibiofemoral alignment, radiolucent lines, and patel ar dislocation. Then, the results of other medium-term fol ow-up researches were compared with fixed-bearing arthroplasty. RESULTS AND CONCLUSION:Final y, 31 patients (45 knees) were fol owed up. The average age was 64.56±10.33 years, and fol ow-up period was 3.9-7.6 years. The postoperative scores, range of motion, maximun flexion and extension angle were improved obviously, but there were no differences with other medium-term fol ow-up researches. No radiolucent lines, prosthetic loosening or polyethylene insert dislocation was found. Lateral patel ar release was done, but no patel ar dislocation or subluxation appeared in al patients. Two patients (2 knees) accompanied patel a clicking. Results indicated that the medium-term clinical result was satisfactory. No patel ar dislocation or subluxation was found, although only lateral patel ar release was done. This may be the superiority of mobile-bearing arthroplasty.
4.Changes of Bid Gene Expression in Neonttal Rats with Hypoxic-Ischemic Brain Dama ge

Objective To study the changes and significance of Bid gene expression in hypoxic-ischemic brain damage (HIBD) in neonatal rats.Methods The animal models of HIBD were made. Total RNA from ipsilateral cerebral hemisph ere was extracted.RT-PCR was used to evaluate the levels of Bid gene expression at different time points after hypoxic-ischemic. Results The levels of Bid mRNA were higher in HIBD rats than those in normal rats.The ex pression of Bid mRNA in ipsilateral hemiphere following cerebral hypoxic-ischem ic began to increase at 6 h, increased to the highest level at 24 h, and then gr adually returned.Conclusion The upregulation of Bid mRNA can play a role in the induction of apoptosis fol lowing cerebral hypoxic-ischemic. 5.Bipolar hemiarthroplasty for hemiplegic side femoral neck fractures in elderly patients

Objective The feasibility and the curative effect of bipolar hemiarthroplasty for hemiplegic side femoral neck fractures in elderly patients were evaluated.Methods The patients suffering from hemiplegic side femoral neck fractures and undergoing bipolar hemiarthroplasty in Beijing Tongren Hospital were followed-up during 2012-2015.The postoperative Harris Score and the complications such as infection,deep vein thrombosis and dislocation were collected.Results Totally 25 patients(25 hips)were followed-up.The average age was 70.1 years,and mean follow-up period was 24.3 months (10-42 months).The mean Barthel index score was higher (77.4 ± 14.7,ranging from 40-95 points) after operation than before operation[25.00 ± 5.95 (15-35 points)] with a statistically significant difference(t=-21.374,P<<0.05).The postoperative Harris Score was 83.5± 8.9(60-95 points).According to Harris score,the excellent and good rate was 80.0%.The patient's satisfaction rate was 88.0%.Conclusions Choosing proper prosthesis and fixing measure,and keeping the function and tension of short extorsion muscles can achieve the satisfactory results for hemiplegic side femoral neck fractures in elderly patients.
6.Biological Characteristics of Marine Bacterium Strain E18 and the Stability of its Indigo Pigment

One strain of Pseudoalteromonas sp. E18 was isolated from the sea mud of Ningbo, Zhejiang. It can produce indigo pigment. The morphological, cultural and biochemical characteristics of the bacterium were studied. The indigo pigment was also extracted. The results showed that the maximum absorption peak of the pigment was at 579nm. The pigment was stable to UV, Na_2SO_3,It was also stable at pH 3~9. The pigment was unstable to the sunlight and high concentration H_2O_2. Temperature of 60℃~80℃ could increase the hue while temperature higher than 90℃ could reduce the hue.
7.Effects of PBDE-47 Single Exposed and Combined with PCB153 on Neurodevelopment in Rats

Objective To study the effects of 2,2',4,4'-tetrabromodiphenyl ethers(PBDE-47)single exposed and combined with 2,2',4,4',5,5'-hexachlorobiphenyl(PCB153)on learning and memory ability and the uhrastrncture of hippocampal CA1 region in rats.Methods Neonatal SD rats(CL grade)were randomly divided into groups,9 male and 9 female in each,then were exposed to single PBDE-47 at doses of 1,5,10 mg/kg and combined with PCB153 at dose of 5 mg/kg,through gavage for one time. Soya oil was used as the solvent control.The learning and memory ability and ultrastructure of neurons in hippocampal CA1 region were examined respectively 2 months after treatment.Results In the low dose groups(1 mg/kg PBDE-47 and 1 mg/kg PBDE-47+ 5 mg/kg PCB153 treated group),the ultrastructure of neurons in hippocampal CA1 region were as normal as that in the control group.As the dose increasing,in 5 mg/kg PBDE-47 group,the endoplasmic reticulum appeared swelling,expansion and degranulation.In 5 mg/kg PBDE-47 plus 5 mg/kg PCB153 group,the mitochondria appeared swelling,the ridge ruptured,the periplast puffed,and then the cytoplasm condensed.In the high dose groups(10 mg/kg PBDE-47 group and 10 mg/kg PBDE-47 plus 5 mg/kg PCB153 group),the neurons showed acutely denatured,the periplast puffed,the cell organelle dissolved and the mitochondria vacuolizated.The interaction between PBDE-47 and PCB153 on the latency period and the general pathway was presented in place navigation test(P
8.Extent and relative changes of equities in using the prenatal care services among premature delivery women with different educational levels

Objective To understand the extent and relative changes of equities in using the prenatal care services among premature delivery women from 4 counties/cities in Jiangsu and Zhejiang provinces, from 1995 to 2000. Methods The study population consisted of 97 537women who delivered singleton live birth of 28 to 41 gestational weeks in 4 counties/cities of Jiangsu and Zhejiang provinces, from 1995 to 2000. The proportions of women with early prenatal visit, of those with at least five prenatal visits and the hospital delivery rates between premature and term delivery groups were calculated. Inequalities in the above indictors by comparing rate difference ( RD), rate ratio ( RR ) and concentration index ( CI ) among women with different educational levels,were examined. Chi-square and One-way ANOVA were used to compare the differences. Results The proportions of having received early prenatal among those women with premature delivery in different education levels were 82.89%, 91.06%, 93.96%, 93.11% respectively, which were less than that of those with full term delivery (86.36%, 93.95%, 95.65%, 96.41%, P=0.008). The proportions of having received early prenatal among the women with high educational levels were higher (RD=-10.05, RR=0.90, CI=0.0104) than those with low educational attainment (RD=-10.22, RR=0.89, CI=0.0131); The proportions of having received at least five prenatal visits among those women with premature delivery in different education levels were 86.54%, 93.17%, 92.99%, 96.49%,respectively, which were less than those with full term delivery (94.60%, 96.65%, 96.15%, 96.66%,P=0.005). The proportions of having had at least five prenatal visits among the women with high educational attainment were higher (RD=-2.06, RR=0.98, CI=0.0006) than those with lower educational attainment (RD=-9.95, RR=0.90, CI=0.0077); The proportions of hospital delivery among the women with full term delivery in different education levels were significantly higher (99.45%, 99.75%, 99.83%, 99.77% ) than those with premature deliveries (98.42%, 99.54%, 99.61%,100.00%, P=0.005). The proportions of hospital delivery among women with high educational attainment was higher (RD =-0.32, CI= 0.0003 ) than those with lower educational attainment ( RD =-1.58, CI=0.0013). Conclusion Education related inequities in prenatal care utilization had not been improved among the Chinese women under our study. Inequities were seen in those women with full-term delivery the preterm delivery ones.
9.The changes of three endogenous hormones during flower bud differentiation of Schisandga chinensis.

OBJECTIVETo reveal the relation between endogenous hormones and the flower bud differentiation in Schisandga chinensis.
METHODTop buds of extremely short branch and axillary buds of long branch in the same plant of S. chinensis were used as material and the contents of endogenous hormones were measured during different periods of the flower bud differentiation with HPLC.
RESULTThe result showed that flower bud differentiation and the formation of female flower were inhibited by high concentration of GA3 and were promoted by high concentration of ABA or ZT. Low ratio of GA3/ABA has the same result.
CONCLUSIONThere was a correlation between endogenous hormones and the flower bud differentiation of S. chinensis.

10.Abdominal radical surgery and regional lymph node dissection for treatment of renal carcinoma (report of 136 cases)

Objective To assess the safety and efficacy of abdominal radical nephrectomy and systematic lymph node dissection for treatment of renal carcinoma. Methods A total of 136 patients underwent radical nephrectomy and regional clearance of lymph nodes from July 2004 to June 2008.There were 92 males and 44 females in the study group.Ages ranged from 23 to 81 years,with a mean age of 54 years.The mean tumor diameter was 55 mm (range,15-170 mm).The tumor size detected by CT and MRI was consistent with that detected by B-ultrasound,98 were stage Ⅰ,13stage Ⅱ,12 stage Ⅲ,and 2 stage Ⅳ. Results All 136 cases underwent radical nephrectomy with retroperitoneal lymphadenectomy.All operations were successful without any major complication.The operative time was 90 to 180 min,with an average of 120 min,and blood loss was 20-400 ml,with an average of 50 ml.The pathological diagnoses were as follows: renal cell carcinoma 123 cases (90%), papillary renal cell carcinoma six cases(4%),chromophobic two cases(1.4%),oncocytoma two cases(1.4%),collecting duct two(1.4%),and others three cases(2.2%).Eight cases reported positive lymph nodes.Of the 136 cases,92 cases were T1 N0 M0,11 were T2 N0 M0,10 were T3 N0 M0,eight were T3 N1 M0 and two were T1 N0 M1.Ninety-five cases (70%) were followed-up at six to 40 months (mean,20 months).The one year and three year survival rates were 96% (91/95) and 86% (82/95),respectively.Conclusions Radical nephrectomy with systematic lymph dissection has advantages of accurate staging,effective resecting of renal tumors and preventing recurrence.Radical nephrectomy is an effective method for the treatment of renal carcinoma.
