3.Ductal Adenocarcinoma of Prostate.

A ductal adenocarcinoma of prostate is a rare entity of a prostate carcinoma, originating from the epithelial lining of the primary and secondary ducts of the prostate. Although there have been about 100 reports of a ductal adenocarcinoma of the prostate in the English literature, this is the first case report described in Korea. Ductal adenocarcinomas of the prostate are often clinically underestimated as rectal examinations and the serum prostate specific antigen levels may be normal. We experienced a case of a ductal adenocarcinoma of the prostate in a 78 year-old man.

4.Two cases of small cell carcinoma of the prostate with fulminant progression.

We managed the two cases of small cell carcinoma of prostate, which has been reported only 20 cases in the world literature. It has not been reported in our country. It`s prognosis is very poor and it may have neurosecretory manifestation. Small cell carcinoma of the prostate combined with adenocarcinoma or previous history of adenocarcinoma is found in 50% cases. The diagnosis was established by pathologic findings with special stain (neuron specific enolase) and electromicroscopic examination.

5.A Case of Bullous Pemphigoid Associated with Prostate Adenocarcinoma.

The presence of various cutaneous disorders may reflect the occult internal malignancy. Bullous pemphigoid (BP), a common autoimmune skin disease, is characterized by the presence of subepidermal blisters. The association of bullous pemphigoid with internal malignant neoplasm has occasionally been reported. However, the relationship of bullous pemphigoid and malignancy is still controversial; the evidence for this association is increasing. Here, we report a rare case of a bullous pemphigoid associated with prostate adenocarcinoma.

6.p53 and p21/WAF-1 overexpressions in prostatic adenocarcinoma.

OBJECTIVES: This study was carried out to determine the role of p53 and p21 in the pathogenesis of prostatic adenocarcinoma and their association with tumour grade. METHOD: Sixty-seven histologically confirmed prostatic adenocarcinoma cases collected from Hospital Universiti Kebangsaan Malaysia and General Hospital Kuala Lumpur were studied. The formalin-fixed paraffin-embedded tissues were stained with monoclonal antibody p53 (clone DO-7) and p21 (clone SX II8) using standard avidin-biotin complex immunohistochemistry (IHC) after microwave antigen retrieval. The adenocarcinomas were graded histologically according to Gleason score: low grade = score 2-4, intermediate grade = score 5-7, high grade = score 8-10. Histological grade was analysed against IHC expressions of p53 and p21 and against patient age and ethnicity. RESULTS: IHC positivity for p53 was expressed in 1/2 (50%) low grade, 14/33 (42%) intermediate grade, and 21/32 (66%) high grade tumours. p21 was expressed in 0/2 low grade, 16/33 (48%) intermediate grade and 15/32 (47%) high grade tumours. p53 and p21 expressions did not show statistically significant correlation with the different grades of prostatic adenocarcinoma or with each other (p = 0.42). There was no correlation between tumour grade and age or ethnicity. CONCLUSION: Although the p53 positivity rate was higher in high-grade prostate adenocarcinoma, this was not statistically significant. We found that a combined expression of p21 and p53 was not correlated with tumour grade. We could not confirm that p21 expression was dependent on p53 expression.

7.Morphometry of Nuclei in Adenocarcinoma of Prostate.

Morphometry of nuclei of the benign and malignant prostatic lesions was performed to study the relationship between nuclear size and shape and the prognosis of prostatic adenocarcinoma. Fifty one cases of prostatic adenocarcinoma and 13 cases of benign prostatic hyperplasia were included to evaluate area, perimeter, Dmax, Dmin, and 5 form factors of the nuclei by image analyzer(Zeiss Ibas 2000) using hematoxylineosin stained slides. All analytic factors of nuclear size and shape were significantly different between benign lesions and adenocarcinomas. Increased nuclear size was associated with nuclear irregularity, presence of metastasis, advanced clinical stage, and high Gleason's grade and score of prostatic adenocarcinoma. On Kaplan-Meier method, survival was decreased with older age, no hormonal treatment, stage D, high Gleason}s grade and stage as well as with larger size and irregular shape of the nuclei. In conclusion, morphometry of nuclei of the prostate can be a helpful tool to differentiate between benign and malignant lesions. Nuclear morphology is thought to be associated with prognosis of prostatic adenocarcinoma.

10.Mucinous Adenocarcinoma of Prostate.

It is not atypical for a small amount of mucin to be present in prostate adenocarcinoma. But true mucinous type of adenocarcinoma of prostate occurs rarely, with 88 cases reported in the literature. This has been histopathologically defined as containing lakes of extracellular mucin comprising at least 25% of the primary prostate tumor. The biologic behavior of this malignancy is not well known and the prognosis is considered to be poor compared with typical adenocarcinoma of the prostate. We experienced a case of mucinous adenocarcinoma of prostate that was developed in a 70 year-old man.
