4.The experience of treatment for zona by acyclovir at Demartology Department, Army Hospital No108

Herpes zoster caused by herpes varicella-zoster virus (VZV). From January to December 2004, 43 patients with herpes zoster were treated by Acyclovir 1000mg/day for 7 days at Demartology Department, Army Hospital No108. In this study, we found good result in 30.23% of patients, moderate result in 67.44%, poor result in 2,33%. Treatment effectiveness related to stage of disease, age when having diagnosis and age of patients.

5.Disseminated zoster in an immunocompromised.

In an immunocompromised patient such as
HIV infection, disseminated herpes zoster is a
common cutaneous manifestation. It is very
important to have clinical suspicion of HIV,
whenever a patient presents with cutaneous
manifestation of HIV. This is a case of a 32 year old
male who came in at for consult at our institution
with a chief complaint of fluid filled bumps which
started on the left abdominal area progressing to
the trunk, upper and lower extremities with
associated pricking pain. Patient was diagnosed
with disseminated zoster and was given acyclovir
with noted complete resolution of lesions.
Laboratory tests were requested which revealed
that the patient also had concomitant HIV and
Hepatitis B. Patient was referred to the Center for
Tropical and Travel Medicine for proper
management.

6.Clinical Effect of Acyclovir Therapy on Herpetic Keratitis.

Acyclovir is highly selective and relatively non-toxic anti-herpes agent. Thirt-y-seven patients with herpetic keratitis were treated with acyclovir. (23 patients with pure epithelial type, 4 patients with accompanying stromal keratitis, and 10 patients with accompanying keratouveitis). Acyclovir was very effective in 23 patients with pure epithelial herpetic keratitis. All epithelial defects healed in a mean healing time of 6.2 +/- 2.3 days. In 4 patients with stromal keratitis, 3 patients healed within 14 days and 1 patient healed on 27 days. In 10 patients with keratouveitis, 8 patients healed within 14 days, 1 patient healed on 21 days, and 1 patient was not healed even after one month. Four patients developed superficial punctate keratopathy during therapy, but no other serious adverse effects were seen.

9.Study on some characteristics of herpes zoster with elders and effects of acyclovir in the treatment

We had studied in 60 patients with herpes zoster in Friendship Hospital and recognised that: men is 83.33%, women 16.67%, mean age was 65.42 ±7.18 mean duration of viral infection was 5.7 ±2.75 days. The sites most affected were thoracic (45%), cranial (23.4%), lumbar (15%), sacral (8.3%), and others (8.3%). We also used Acyclovir to treat 60 patients with zoster and found that: Acyclovir had good result in 89.47% and the vesibullar lesions with wet, red skin still remained in 10.53% patients. Side effects of Acyclovir were: diarrhea in 8.3%, nausea in 5% and vomiting in 3.3% patients. Acyclovir did not change renal and liver functions.

10.A Case of Bilateral Herpes Zoster.

Noncontiguous bilateral herpes zoster is very rare, especially in a child. Herpes zoster duplex bilateralis means that both halves of the body are involved with the disease. A 4-year-old female, previously healthy, was presented with painful grouped papulovesicles on an erythematous base on both sides of the trunk (left T5 dermatome and right T7,8 dermatomes). Tzanck smear showed multinucleated giant cells and the histopathologic findings were consistent with herpes viral infection. She was treated with oral acyclovir for 7 days without any significant complication.
