1.Wear of Rotary Instruments: A Pilot Study

The aim of this study was to assess the wear of tungsten
carbide burs and round rotary diamond instruments
through measurements of rake angle and visual
observations respectively under a f ield emission
scanning electron microscope. Sixty short and long
head pear-shaped tungsten carbide burs and 18 round
rotary diamond instruments that had been used to
complete < 5, > 5 and > 10 cavity preparations (n=10/
group) were selected from the 3rd and 4th year dental
students, Faculty of Dentistry, University of Malaya.
There were two control groups consisting of long and
short tungsten carbide burs of ten each. Two-way
ANOVA was used to analyse the rake angle data. The
data from the two control groups were collectively
analysed following multiple paired t-test (p > .05)
which showed no significant difference between the
two types of tungsten carbide bur (short and long
head). The mean rake angle of the control group was
significantly higher (p < .05) compared to the < 5, > 5
and > 10 cavity preparation groups. The rake angle of
the > 10 cavity preparation group was significantly
lower than the other two test groups (p < .05). Round
rotary diamond instruments in the < 5 cavity
preparation group showed intact diamond particles
with distinct cutting facets comparable to the control
group. However, diamonds instruments in the > 5 and
> 10 cavity preparation groups showed blunt diamond
particles. In conclusion, wear of tungsten carbide burs
and round rotary diamond instruments were evident
after repeated use. Wear was more pronounced when
instruments were used to prepare more than ten
cavities.

Introduction: Sexual dysfunction is common but not often assessed in the routine clinical care among
males on opiate substitute treatment. Objective: To determine the association between clinical variables
and erectile dysfunction (ED) among men on methadone maintenance therapy (MMT). Methods: A
cross-sectional study involving 108 participants who attended the Drug Clinic, Hospital Kuala Lumpur.
The instruments used include the Structured Clinical Interview for DSM-IV Axis-I Disorder (SCID-I),
Beck Depression Inventory (BDI) and International Index of Erectile Function-15 (IIEF-15). Results:
Concurrent heroin abuse was significantly associated with presence of ED (p=0.024). Treatment factors
including methadone dose and duration of methadone treatment were not significantly associated with
ED. Conclusion: Education on sexual dysfunction as a potential adverse effect and its association
with illicit heroin use should be considered in the doctor-patient consultation to encourage treatment
adherence and abstinence from heroin.
3.Visual Functions And Retinal Morphology In Patients With Polypoidal Choroidal Vasculopathy Seen In An Age Related Macular Degeneration Referral Centre Of Malaysia

The aim of the present study was to evaluate visual functions and retinal morphology of Polypoidal Choroidal Vasculopathy (PCV) patients seen in an AMD referral centre of Malaysia and to further explore association between visual functions and optical coherence tomography (OCT) parameters in PCV eyes. In this single centre, cross sectional study, best corrected distance visual acuity (DVA), near vision acuity (NVA), reading speed (RS), and contrast sensitivity (CS) were measured in naïve PCV eyes. Selective parameters such as integrity of external limiting membrane (ELM), inner segment and outer segment junction (IS-OS), retinal pigment epithelium and Bruch’s membrane (RPE-BM) complex, average retinal thickness (ART) and volume (ARV), central retinal thickness (CT), centre maximum (CTmax) and centre minimum (CTmin) thickness were assessed using spectral-domain OCT. Forty-three new PCV eyes of 42 patients were evaluated. Mean (±SD) DVA, NVA, CS, RS were 0.83 ± 0.35 logMAR, 0.77 ± 0.3 logMAR, 0.69 ±.26 log CS and 59.56 ±14 words per minutes respectively. ART and CT showed good correlation with DVA (r=0.571& 0.546) and CS (r= 0.576 & 0.586). ARV and CTmax showed good correlation with CS (r=0.516 &, 0.513). The mean DVA, NVA and CS between three ELM and IS-OS status were significantly different (p=.002 & .000; p=.012 & .029; p=.005 & .001). In conclusion, present study reports visual functions and OCT characteristics of PCV patients in an AMD referral centre of Malaysia. Many of the quantitative and qualitative OCT parameters showed good association with the visual functions in eyes with PCV.

4.Inhibition of Ehrlich ascites carcinoma by Manilkara zapota L. stem bark in Swiss albino mice.

OBJECTIVETo evaluate the antitumor activity of Manilkara zapota (M. zapota) L. stem bark against Ehrlich ascites carcinoma (EAC) in Swiss albino mice.
METHODSThe in vivo antitumour activity of the ethyl acetate extract of stem bark of M. zapota L. (EASM) was evaluated at 50, 100 and 200 mg/kg bw against EAC using mean survival time. After administration of the extract of M. zapota, viable EAC cell count and body weight in the EAC tumour hosts were observed. The animal was also observed for improvement in the haematological parameters (e.g., heamoglobin content, red and white blood cells count and differential cell count) after EASM treatment.
RESULTSIntraperitoneal administration of EASM reduced viable EAC cells, increased the survival time, and restored altered haematological parameters. Significant efficacy was observed for EASM at 100 mg/kg dose (P<0.05).
CONCLUSIONSIt can be concluded that the ethyl acetate extract of stem bark of M. zapota L. possesses significant antitumour activity.

5.Imported Malaria in United Arab Emirates: Evaluation of a New DNA Extraction Technique Using Nested PCR.

Local malaria transmission in the United Arab Emirates (UAE) came to an end in 1997. Nevertheless, UAE has been subjected to substantial importation of malaria cases from abroad, concerning both UAE nationals and immigrants from malarious countries with a total number of 2,119 cases in 2007. To evaluate a new DNA extraction technique using nested PCR, blood samples were collected from 132 individuals who presented to Infectious Diseases Department in Rashid Hospital, Dubai, and Central Department of Malaria Control with fever and persistent headache. Giemsa-stained blood films and ELISA test for malaria antibodies were carried out for detection of Plasmodium infection. Plasmodium infections were identified with the genus-specific primer set and species differentiation using nested PCR. A rapid procedure for diagnosis of malaria infections directly from dried blood spots using for the first time DNA extract from FTA Elute cards was evaluated in contrast to extraction techniques using FTA classic cards and rapid boiling technique. Our new simple technique for DNA extraction using FTA Elute cards was very sensitive giving a sensitivity of 100% compared to 94% using FTA classic cards and 62% in the rapid boiling technique. No complex preparation of blood samples was required prior to the amplification. The production cost of DNA isolation in our PCR assay was much less in comparable to that of other DNA extraction protocols. The nested PCR detected plasmodial infection and could differentiate P. falciparum from P. vivax, and also detected the mixed infection.

7.Tympanometric values in young Malay adults: preliminary data.

INTRODUCTIONThe present study aimed to obtain preliminary tympanometric data of young Malay adults and to compare the results between genders.
METHODS96 undergraduate students (49 males and 47 females), aged 19-25 (mean and standard deviation 21.14 +/- 1.31) years, participated in this study. Otoscopic examination, pure tone audiometry, qualitative tympanogram and ipsilateral acoustic reflex were measured to ensure a clear ear canal, normal hearing and normal middle ear function, prior to tympanometric measurement. As a result, a total of 154 ears (80 ears from males and 74 ears from females) were selected for further statistical analyses. The tympanometric parameters measured were peak compensated static acoustic admittance (Peak Y(tm)), tympanometric width (TW) and equivalent ear canal volume (V(ea)).
RESULTSThe results showed that the mean Peak Y(tm), V(ea) and TW for males were 0.81 mmhos, 1.48 cubic cm and 113.67 daPa, respectively. The mean Peak Y(tm), V(ea) and TW for females were 0.63 mmhos, 1.12 cubic cm and 98.04 daPa, respectively. Males were found to have significantly higher mean V(ea) and mean Peak Y(tm) than females. However no significant gender difference was observed in the mean TW.
CONCLUSIONThe current study suggests that young Malay adults may require gender-specific Peak Y(tm) and V(ea) values when implementing a quantitative approach in tympanogram interpretation.

8.Dehydration rates and rehydration efficacy of water and sports drink during one hour of moderate intensity exercise in well-trained flatwater kayakers.

INTRODUCTIONThe aim of this study is to investigate the amount of water loss and percentage dehydration experienced during 1 hour of paddling on the kayak ergometer so as to help coaches and athletes tailor a suitable and adequate rehydration regime. Also, rehydration efficacy between water and a well established, commercially available sports drink (Gatorade, Quaker Oats company, USA) was investigated in this cross-over study.
MATERIALS AND METHODSTen national flatwater kayakers were monitored in a controlled setting while paddling the kayak ergometer for 1 hour at an intensity sustainable for long distance marathon. They rehydrated themselves ad libitum with provided beverage. Post-void towel-dried body mass was measured before and after the exercise with percentage dehydration calculated. Body core temperature (tympanic temperatures), ratings of perceived exertion and thirst index were monitored every 15 minutes. The amount of fluid consumed, urine output and urine specific gravity were obtained after exercise.
RESULTSThe results [mean +/- standard deviation (SD)] showed that kayakers underwent 1.10 +/- 0.52% dehydration while rehydrating with water as compared to 0.72 +/- 0.38% while rehydrating with Gatorade. Also, athletes on water rehydration had higher rating of perceived exertion (RPE) at the 30th and 60th minute mark of the exercise. Overall, athletes undergoing water rehydration lost significantly more body mass (0.70 +/- 0.39 kg) as compared to rehydrating with Gatorade (0.46 +/- 0.27).
CONCLUSIONAlthough the hydration efficacy of Gatorade proved superior to that of water, the athletes' hydration habits with either fluids did not provide adequate hydration. It is recommended that specific strategies be developed to address dehydration and rehydration issues of kayakers in Singapore.

9.Time Course Of Cigarette Withdrawal Symptoms While Using Nicotine Patch During Ramadan

Previous studies explain the time course of withdrawal symptoms among smokers pre and post quit attempt, either withor without the help of medication. Studies showed that male Muslim smokers could quit smoking during Ramadan sincefasting relate to the changes in psychosomatic, daily activities and nicotine withdrawal symptoms. This study aimed toinvestigate the time course of withdrawal symptoms among smokers who used nicotine patch to quit smoking duringfasting in Ramadan. A total of 40 eligible Muslim males who tried to quit smoking was selected and provided with smokingcessation counseling for the duration of 8 to 10 weeks while on nicotine patch. Participants level of withdrawal symptomswas recorded by using nine items of Minnesota Nicotine Withdrawal Scale over a period of 60 days. Participant’s carbonmonoxide reading and body weight were measured within six months including pre and post-Ramadan fasting. Over fourweeks of the fasting month, the measured withdrawal symptoms such as urge to smoke (P ≤ 0.001), depressed mood (P≤ 0.001), irritability/frustration or anger (P ≤ 0.05), anxiety (P ≤ 0.05), difficulty concentrating(P ≤ 0.001), restlessness(P ≤ 0.001), difficulty going to sleep (P ≤ 0.001) and impatient (P ≤ 0.05) significantly decreased except appetite by theend of week 4. Time course analyses demonstrated that all outcome measures showed good effects during cessation infasting month. The point prevalence abstinence at first month of quitting was 67.5% which is higher in fasting month.This has shown positive clinical implications in managing smoking cessation program during Ramadan with the aid ofnicotine patch.
10.Effect of 11β-HSD1 dehydrogenase activity on bone histomorphometry of glucocorticoid-induced osteoporotic male Sprague-Dawley rats.

INTRODUCTIONGlucocorticoids cause osteoporosis by decreasing bone formation and increasing bone resorption activity. Glucocorticoid action in bones depends on the activity of 11-beta-hydroxysteroid dehydrogenase type 1 (11β-HSD1) enzyme, which plays an important role in regulating corticosteroids. 11β-HSD1 is expressed by human and rat osteoblasts. We aimed to investigate the relationship between 11β-HSD1 dehydrogenase activity and bone histomorphometric changes in glucocorticoid-induced osteoporotic bone in rats.
METHODSA total of 30 male Sprague-Dawley rats (aged three months, weighing 200-250 g) were divided into three groups of ten each. Group 1 rats were the baseline control, which were sacrificed untreated at the beginning of the study. Group 2 rats underwent sham operation and were administered with vehicle olive oil intramuscularly at 0.05 ml/kg. Group 3 rats were adrenalectomised and administered with an intramuscular injection of dexamethasone 120 μg/kg body weight/day. The treatment was started two weeks after the operation, for a duration of two months. Plasma osteocalcin, plasma pyrodinoline, plasma corticosterone and 11β-HSD1 were measured, and bone histomorphometry analysis was performed.
RESULTSDexamethasone treatment caused an increase in plasma corticosterone level, together with a significant reduction in 11β-HSD1 dehydrogenase activity of the bone, along with a higher plasma level of the bone resorption marker, pyridinoline. Dexamethasone treatment also caused a reduction in trabecular volume, number and thickness, and an increase in trabecular separation.
CONCLUSIONLong-term glucocorticoid treatment reduces the 11β-HSD1 dehydrogenase activity in the bone, which can otherwise lead to bone loss due to the increased level of active glucocorticoids.
