1.Use Of Nonprescription Substances Among University Students In Malaysia

The trend of using nonprescription substances for weight-loss management is common. There are several factors associated with the use of nonprescription substances. This study aimed to determine prevalence and factors associated with the use of nonprescription substances for weight-loss management among university students. A cross sectional study was conducted from August until October 2013 among 358 postgraduate students aged 22 – 45 years in Universiti Kebangsaan Malaysia (UKM). A validated self-administered questionnaire was distributed by multistage sampling to the students. The prevalence of using nonprescription substances among university students was 27.9% (n=100). There were significant association between use of nonprescription substances with age (t=2.41, p=0.017), peer influence (t=7.06, p<0.001), media influence (t=4.38, p<0.001), marital status (t=2.41, p=0.017), BMI (χ²=23.72, p<0.001), body weight dissatisfaction (χ²=14.77, p<0.001) and experienced of conventional strategies (χ²=94.81, p<0.001). After controlling the confounders, BMI (Adj OR: 2.49, 95% CI: 1.42-4.36), peer influence (Adj OR: 1.09, 95% CI: 1.03-1.15) and experienced of conventional strategies (Adj OR: 9.25, 95% CI: 4.95-17.28) were positively associated with the use of nonprescription substances. In conclusion, the prevalence of nonprescription substances use among university students is common among overweight and obese, had peer influence and experienced of conventional strategies. Therefore, there is an urgent need to provide university students with education and access to scientific and unbiased information of nonprescription substances.

2.Intraperitoneal Chemotherapy Following Refractory Intravenous Route in Advanced Ovarian Cancer

Platinum based adjuvant chemotherapy is generally recommended for ovarian cancer to improve the survival rate.
Intravenous route is commonly used, easily administered and less associated complications. However, intraperitoneal
route is gaining its popularity as a single procedure or adjunctive to the intravenous route. Numerous questions on its
eligibility and safety are still perplexed. A case review on a patient with non optimal debulking surgery of advanced
ovarian cancer was studied. Intravenous platinum based chemotherapy combined with paclitaxel failed to bring her
to clinical remission. Second line chemotherapy, gemcitabin rendered her to poor response with unresolved
debilitating ascites needing recurrent drainage. Surprisingly, a trial of intraperitoneal chemotherapy with cisplatin
revealed a great response with a complete clinical remission.
3.Profile of Patients Referred to a Specialist Oral Medicine and Oral Pathology Unit

The aim of this study was to determine the profile of patients referred to a specialist oral medicine and oral pathology unit in Kuala Lumpur by reviewing clinical dental records received in Oral Pathology Diagnostic Service (OPDS) in Faculty of Dentistry, UKM from 2001 until 2010. A total of 547 archival biopsy clinical dental records were reviewed and analysed using SPSS version 17.0. Oral and maxillofacial diseases were frequently seen in female (1.3:1), young adults (30.0%) of Malay ethnicity (64.6%). Most of the acquired specimens were from dental specialists (n=451, 84.8%), particularly from oral and maxillofacial surgeons (OMFS) (n=349, 63.8%) compared to general dental practitioners (GDPs) (n=81, 14.8%). Almost all of the biopsy specimens were of soft tissue origin (n=462, 84.4%), derived from lining mucosa (n=197, 36.0%) and were biopsied excisionally (n=325, 59.4%) more often than by incisional biopsy (n=207, 37.8%). A large proportion of the oral and maxillofacial diseases were of reactive (n=188, 34.4%) and inflammatory (n=121, 22.1%) cause. Tumours are mainly benign (n=69, 12.6%) with only small cases are malignant (n=34, 6.2%). The most common histological diagnoses were accounted by mucocele (n=56, 10.2%), pyogenic granuloma (n=47, 8.6%), fibroepithelial polyp (n=38, 6.9%), radicular cyst (n=33, 6.0%) and periapical granuloma (n=29, 5.3%). This study characterizes the clinical profile of patients seen in our oral medicine and oral pathology unit. Present findings can be used as a reference to the clinicians and pathologists in effective patient management and organization in the future.

4.Pelvic aneurysmal bone cyst

This paper describes an extremely rare case of a huge aneurysmal bone cyst (ABC) in the pelvis, occurring in the patient’s 5th decade of life. The patient presented with a history of painless huge pelvic mass for 10 years. Plain radiograph and computed tomography showed huge expansile lytic lesion arising from the right iliac bone. A biopsy was performed and histology confirmed diagnosis of aneurysmal bone cyst. Unfortunately, the patient succumbed to profuse bleeding from the tumour.
5.Popliteal Artery Entrapment Syndrome: A Rare Cause of Claudication in a Young Athlete

Popliteal artery entrapment syndrome is an important infrequent cause of serious disability among young adults and athletes. We hereby describe a case of popliteal artery syndrome and its clinical implications. Physicians should be aware of the possibility of popliteal artery entrapment in young patients presenting with progressive arterial insufficiency.

The authors propose a musculoskeletal (MSK) magnetic resonance imaging (MRI) protocol using selected
sequences for common orthopaedic indications. Selected sequences allow optimal visualisation of the indicated
pathology while screening for other common conditions. The authors emphasise the need for standard positioning of the
patient and standard orientation of scan planes to facilitate comparison with follow-up scans.
7.Managing adolescent pregnancy: The unique roles and challenges of private general practitioners in Malaysia

Introduction: Managing adolescent pregnancy in the primary care setting is complex, as it
requires doctors to navigate through a combination of medical, social, financial and legal needs.
Objective: This study explores the perspectives of private general practitioners on their roles and
challenges in managing adolescent pregnancy in Malaysia.
Methods: Nineteen private general practitioners in Selangor and Kuala Lumpur participated in
in-depth interviews in 2015. A topic guide was used for interview navigation. Participants were
asked to discuss their experiences and approaches in managing pregnant adolescents. We used
purposive sampling to recruit consenting private general practitioners who had experience in
managing adolescent pregnancy. The verbatim transcripts of the audio-recorded interviews were
analyzed using thematic analysis. Data reached saturation at the nineteenth in-depth interview.
Results: Two themes emerged. Under the theme ‘inadvertent advocator,’ participants described
their tasks with regards to building trust, calming angry parents and delivering comprehensive
counseling and care related to the sexual and reproductive health of adolescents, including
requests for abortions. Theme two, ‘challenges of private general practitioners,’ refers mainly to
personal and religious conflicts arising from a request for an abortion and deficiencies in support
and multidisciplinary integration within their practice settings.
Conclusion: General practitioners practicing in the private sector identify themselves as active
players in supporting pregnant adolescents but face many challenges arising from the personal,
religious, professional and community levels. Addressing these challenges is important for
optimal care delivery to pregnant adolescents in this community
8.Short Review Of Fourteen Years Maternal Mortality In Achieving Mdg5 In UKMMC

Worldwide maternal mortality rate had reduced tremendously including Malaysia. At the 2000 Millenium Summit, eight Millenium Development Goals (MDGs) were established with target for the year 2015. Three years remained until the dateline to achieve MDG5, which comprised of two targets and one being reducing the maternal mortality ratio by 75%. This review compared the trend of maternal mortality and its causes in our centre to the national data. The national data had shown a reduction from 44 to 27.6 in 100 000 live births in1991 and 2008 respectively. The subsequent rate had stabilized for the past seven years. In contrast the UKM Medical Centre (UKMMC) data in the past 15 years had not been stable. The target of MDG5 seemed to be achievable by our country but may require longer time as we have yet to find ways to overcome medical care inadequacy in remote areas.
9.Association of Body Weight Status and Socio-Demographic Factors with Food Habits among Preschool Children in Peninsular Malaysia

Introduction: Changes in children's food habits are largely attributed to changes in the family and social environment. This cross-sectional study was carried out to determine the association of socio-demographic factors with food habits among preschool children in Peninsular Malaysia.
Methods: A total of 1,933 preschool children aged 4-6 years old participated in the study. Parents or guardians were interviewed on the socio-demographic characteristics and food habits of their children. Height and weight of the preschoolers were measured; BMI-for-age, weight-for-age and height-for-age were determined.
Results: The mean monthly household income was RM3,610 with 59.6% of parents having attained secondary education. The prevalence of possible risk of overweight, being overweight and obesity were 3.9%, 7.9% and 8.1%, respectively while the prevalence of underweight and stunting was 8.0% and 8.4%. A majority of the preschoolers consumed breakfast, lunch and dinner every day, with the proportion of children skipping their main meals at about 15.0%. Parents' education level and household income were significantly associated with intakes of fruits, vegetables, milk and dairy products, as well as fast food. However, there was no significant association between children's body weight status and frequency of main meals intake, fruits, vegetables, milk and dairy products, and fast food intake.
Conclusion: The preschoolers demonstrated moderately healthy food habits; nevertheless even at this young age, they were inclined towards fried foods, snacking and fast foods intake. Parents and guardians should play a more significant role in educating and promoting good nutrition and food habits among preschoolers.
10.Beta-human Chorionic Gonadotropin-secreting Lung Adenocarcinoma

Overexpression of beta-human chorionic gonadotropin (β-hCG) is frequently associated with germ cell tumours, especially choriocarcinoma. Ectopic secretion of β-hCG by non-small cell lung cancer is exceptional. We present an exceedingly rare case of pulmonary adenocarcinoma that secretes β-hCG. Our patient is a 62-year-old postmenopausal woman, a nonsmoker, who presented with a six-month history of progressive dyspnoea, associated with decreased appetite and significant weight loss. Her serum β-hCG was very high (11211.9 mIU/ml), which prompted investigations to exclude germ cell tumour. Radiological imaging revealed a 10-cm right lung mass with adrenal metastasis. No other focal lesions were detected. Microscopy of the lung biopsy specimen showed replacement of normal lung tissue by sheets of malignant cells, forming vague glands in some areas. Immunohistochemically, the malignant cells showed focal immunopositivity for thyroid transcription factor 1 (TTF-1), napsin A, cytokeratin 7 (CK7) and β-hCG. A diagnosis of β-hCG-secreting pulmonary poorly differentiated adenocarcinoma was rendered. Serum β-hCG level decreased significantly to 168.6 mIU/ml after the first cycle of chemotherapy. In conclusion, β-hCG expression in lung cancer should be recognised to facilitate prompt diagnosis and initiation of appropriate intervention.
