1.The HUKM Spinal Instrumentation System for adolescent idiopathic scoliosis. A biomechanical comparison study using finite element analysis.

This finite element analysis is aimed at comparing relative stiffness of three different posterior instrumentation constructs: the Hospital Universiti Kebangsaan Malaysia Spinal Instrumentation System (HUKM-SIS), the Cotrell-Dubousset Instrumentation (CDI) and Harrington Instrumentation System (HIS), used in the treatment of adolescent idiopathic scoliosis (AIS). The constructs were tested under various loads using MSC Patran 2001 r2a. Under increasing flexion loads, there was a linearly corresponding increase in deflection magnitudes for all constructs on the load-deflection curve. The CDI was the stiffest construct under axial, forward flexion and extension loads, followed by the HUKM-SIS and HIS. Under lateral bending loads, the HUKM-SIS construct was the stiffest followed by CDI and HIS. The HUKM-SIS construct was stiffer than HIS under torsional loads. We conclude that multiple pedicle screws increase the stiffness of posterior instrumentation constructs under all loads and inter-segmental spinous processes wiring increase the stiffness against lateral bending.

2.Chemical and biological studies of Kalanchoe pinnata (Lam.) growing in Bangladesh

Objective: To isolate compounds from K. pinnata and elucidate their structures and to explore preliminary antioxidant, antimicrobial, cytotoxic and thombolytic activities of extractives of the plant. Methods: The methanol extract of whole plant of K. pinnata has been subjected to different chromatographic separation and purification processes to isolate the secondary metabolites. The structures of the isolated compounds have been elucidated by extensive NMR studies. The free radical scavenging activity of the crude extract and its different Kupchan fractions were determined on stable radical DPPH. In vitro antimicrobial activity was determined by the disk diffusion method. Cytotoxicity screening has been performed against Artemia salina. Total phenolics content, membrane stabilizing activity and thombolytic activities were assessed by following established protocol. Results: The isolated compounds were identified as glut-5(6)-en-3-one, taraxerone, 3β-friedelanol, β-amyrin-3-acetate, 3,5,7,3',5'-pentahydroxyflavone and β-sitosterol. The chloroform soluble fraction showed potent antioxidant activity of (IC50=80.0 μg/mL) and significant cytotoxicity, while the crude extract demonstrated noticeable total polyphenol content (149.24 mg of GAE/gm of extractive), moderate membrane stabilizing activity and inhibition of clot lysis of blood. Conclusions: The obtained results rationalize the folkloric use of the plant and can be further investigated to isolate the active compounds responsible for the biological activities.
4.Gastrointestinal Stromal Tumour of Pelvic Wall: Report of a Case

Gastrointestinal stromal tumors (GIST) because of its rarity continues to be a curiosity for radiologists, surgeons and
pathologist. These tumors are usually found in gastrointestinal tract (GIT) but account for less than 1% of GIT
tumors. A 35-year-old male presented to us with complaints of painful micturation which on radiologic evaluation
revealed a large mass in relation to urinary bladder. The tru-cut biopsy reported a low grade soft tissue tumour. The
said mass was excised on laparotomy and sent for histopathological examination which reported as high grade GIST
or a pleomorphic sarcoma as possibilities. Immuno-histochemistry finally confirmed the diagnosis of a GIST with
CD 34 positive and C-Kit negative status. Approximately, 95% of GIST tumors are C-Kit positive and hardly 5% are
C-Kit negative. C-Kit negative status makes this case report more worth reporting.

5.Effectiveness of selective risk based screening for Gestational Diabetes (GDM) in Malaysia: A retrospective cohort study based on the National Obstetric Registry (NOR) of Malaysia

Introduction: Gestational diabetes (GDM) has significant
maternal and foetal implications. screening allows active
interventions which significantly improves pregnancy
outcomes. Despite World Health Organization (WHO), FIGO
and National Institute of clinical Excellence (NIcE)
recommendations for universal screening especially among
high risk population; Malaysia currently adopts a selective
risk based screening for GDM.
Objective: the objective is to audit the effectiveness of the
current practice of selective risk based screening in
detection of GDM in Malaysia.
Methodology: this is a retrospective cohort study based on
the National Obstetric Registry (NOR) which comprises of 14
major tertiary hospitals in Malaysia. the study period was
from 1st January 2011 till 31st December 2012 and a total of
22,044 patients with GDM were analysed. Logistic
regression analysis was used to calculate the crude odd
ratio.
Results: the incidence of GDM in Malaysia is 8.4%. Maternal
age of ≥25, booking bMI ≥27kg/m2, booking weight ≥80kg
and previous hypertension are non-significant risk of
developing GDM in Malaysia. Parity 5 and more was only
associated with an odds-ratio of 1.02 (95% confidence
Interval: 0.90-1.17) as compared to parity below 5. the
association of women with previous stillbirth with GDM was
not significant.
conclusion: current risk based screening for GDM based on
maternal age, booking bMI, weight and hypertension is
inappropriate. An ideal screening tool should precede
disease complications, which is the novel objective of
screening. Universal screening for GDM in Malaysia may be
a more accurate measure, especially with regards to
reducing maternal and foetal complications.

6.Spinal fractures resulting from traumatic injuries.

OBJECTIVETo illustrate mechanisms of spine fractures and the pattern of spinal injuries characterized by the major mechanisms in urban population of Iran.
METHODSData regarding spinal injuries including demographics, mechanism and level of spinal injury, abbreviated injury score, associated injuries and final fate of the patients were extracted from the Iranian national trauma registry database from 1999 to 2004.
RESULTSA total of 619 patients with traumatic spine fractures were identified, of whom 68.5% were males. The peak frequency of these injuries occurred in the 21-40 year age-group. Accidental falls and road traffic crashes (RTCs) were the most common mechanisms of spinal fractures (47.2% and 44.1%, respectively). RTCs tended to occur in younger patients compared with accidental falls. The most common spinal region for spinal fracture was the lumbar spine (53.63%). Cervical spine fractures were significantly more common in RTCs, while lumbar spine fractures were more common in accidental falls (P less than 0.001). A total of 171 (27.6%) patients had associated non-spinal injuries, of whom 127 had associated extremity injuries, and 55 had head injuries. Thirty-six (5.6%) patients had spinal cord injury (SCI).The injury severity score of the RTC group was significantly higher than that of accidental falls (P equal to 0.002). Fifteen (4%) patients died of traumatic injuries. The rate of death was significantly higher in RTCs compared with accidental falls (5.1% vs 2.1%, P equal to 0.039).
CONCLUSIONSThe patterns of spinal fractures are similar to those reported from developed countries. RTCs tend to affect the younger age population and are associated with a higher degree of associated injuries and mortality than accidental falls. Therefore preventive strategies should be based on reduction of the number and severity of RTCs.

7.Comparison of road traffic fatalities and injuries in Iran with other countries.

OBJECTIVETo compare fatalities and injuries in road traffic crashes (RTC) in Iran with other countries.
METHODSData were obtained from national health sources of Iran. These data included population number, registered motor-vehicles number, number of RTCs and consequent fatalities and injuries from 1997 to 2006.
RESULTSRTC fatality and injury rates increased from 1997 to 2005, but decreased in 2006. The overall men/women ratio in the RTC fatalities was 4.2:1. High RTC fatality rate of 39 per 100 000 population in Iran was almost the same as some other developing countries. In Iran, RTC fatalities in recent years were almost twice as much as the highest rate among the European countries.
CONCLUSIONSThis investigation shows that in spite of reduction of RTC fatality in Iran in 2006, it is still one of the highest in the world. Moreover, this paper describes the state of RTC-related parameters in a developing country in comparison with the developed countries.

8.Spinal Tuberculosis: Diagnosis and Management.

The spinal column is involved in less than 1% of all cases of tuberculosis (TB). Spinal TB is a very dangerous type of skeletal TB as it can be associated with neurologic deficit due to compression of adjacent neural structures and significant spinal deformity. Therefore, early diagnosis and management of spinal TB has special importance in preventing these serious complications. In order to extract current trends in diagnosis and medical or surgical treatment of spinal TB we performed a narrative review with analysis of all the articles available for us which were published between 1990 and 2011. Althoug h the development of more accurate imaging modalities such as magnetic resonance imaging and advanced surgical techniques have made the early diagnosis and management of spinal TB much easier, these are still very challenging topics. In this review we aim to discuss the diagnosis and management of spinal TB based on studies with acceptable design, clearly explained results and justifiable conclusions.

9.Spinal cord injuries from road traffic crashes in southeastern Iran.

OBJECTIVETo analyze the data of patients with spinal cord injury (SCI) induced by road traffic crashes in southeastern Iran for better understanding the pattern of these injuries and therefore for better designing health system planning.
METHODSIn this historical cohort study, the patients who had been transferred to Level I trauma center in southeastern Iran due to road traffic accidents with radiographic documented SCI were evaluated.
RESULTSAmong 64 patients with SCI, 38 patients (59.4%, 36 males and 2 females, aged 27.42 years+/-9.44 years on average) were injured by road traffic accidents. Car and motorcycle accidents were responsible for 26 cases (68.4%) and 12 cases (31.6%), respectively. And 31 patients (81.6%) had complete SCI. Conus medularis (T12-L2) was the most affected level.
CONCLUSIONSResults are discussed in terms of preventive measures, specifically those concerning the use of restraint and helmet and driving behavior. This study should be extended nationally to gain a larger case series so that the SCI risk of particular vehicle configurations, considering other crash factors, can be more precisely quantified and the characteristics for low occurrence of SCI can be more precisely identified.
