1.Clinical analysis of intravenous thrombolysis combined with psychological intervention in the treatment of acute cerebral infarction

Objective To discuss the clinical effect of intravenous thrombolysis combined with psychological intervention in the treatment of acute cerebral infarction. Methods 84 patients with acute cerebral infarction were randomly selected at the June 2015 to September 2016 in our hospital were divided into experimental group 42 cases, 42 cases in the control group, the control group was given aspirin and Plavix treatment, experimental group received alteplase intravenous thrombolytic therapy combined with psychological intervention, each stage of the NISS scores of the two groups were compared, the two groups of patients with SDS self rating scale score, Barthel index and NDS score were compared. Results The experimental group of stage NISS was significantly better than the control group, the differences between groups was statistically significant (P<0.05); the experimental group with SDS self rating scale score, Barthel index and NDS score was significantly better than the control group, the differences between groups was statistically significant (P< 0.05). Conclusion In the treatment of acute cerebral infarction, the effect of intravenous thrombolysis combined with psychological intervention is obvious, and it is worthy of clinical promotion and application.
2.Expression and Significance of Coxsackie-Adenovirus Receptor in Myocardia With Dilated Cardiomyopathy

Objective:To clarify the expression and significance of coxsackie-adenovirus receptor (CAR) in myocardia with dilated car- diomyopathy (DCM). Methods:Myocardium specimens derived from 20 patients with DCM underwent orthotopic cardiac transplantation and 5 nor- mal myocardium specimens were collected at autopsy from sudden deaths.In the myocardia,coxsackievirus B (CVB)and adeno- virus (ADV) genes were detected by polymerase chain reaction (PCR),and the expression levels of CAR mRNA and protein were examined by reverse transcription (RT)-PCR and immunohistochemistry,respectively. Results:Viral genomes could be amplified from the myocardia of 12 DCM patients (CVB=7,ADV=5).Viral genomes failed to be detected from myocardia of 5 controls.Therefore,the present experiment had four groups:eoxsackievirus,adenovi- rus,the other and the control.There were no CAR mRNA and protein expression in the other group or normol control group. However,the expression levels of CAR mRNA and protein were remarkedly elevated in coxsackievirus and adenovirus groups, There were no differences between coxsackievirus group and adenovirus group. Conclusion:Upregulated CAR expression can increase the susceptibility of CVB or ADV,which makes viral myoearditis pro- gressing to dilated cardiomyopathy.
3.Efficacy of compound trabeculectomy in the treatment of glaucoma

Objective To research the clinical effect of complex trabeculectomy in the treatment of glaucoma. Methods 80 patients with glaucoma were selected in the study.They were randomly divided into two groups. 40 patients in the control group were given traditional trabeculectomy technique,and 40 patients in the treatment group were given composite trabeculectomy.The incidence of shallow anterior chamber,intraocular pressure,one year after the formation of a functional follicles were analyzed and compared in the two groups.Results The incidence rate of shallow anterior chamber in the treatment group (6.98%)was lower than the control group(23.81%).6 months, 12 months after operation,the IOP of the treatment group was lower than the control group,and less than 21mmHg. After 1 year,the formation rate of functional follicles of the treatment group (90.70%)was higher than the control group (71.43%),the difference between the two groups was significant(χ2 =5.16,P <0.05).Conclusion The treatment effect of composite trabeculectomy for glaucoma is significant.

Cytomegalovims(CMV) infection is widely spread in our country.The initial CMV infection often occurs in childhood,can cause a variety of respiratory diseases in children,seriously threaten the health of children.This article describes the relationship between CMV infection and respiratory disease in children with pneumonia,wheezing,bronchial and pulmonary dysplasia,and reviews the progress in the diagnosis and treatment of cytomegalovirus infection,in order to improve the understanding of human CMV infection in respiratory diseases of children.
5.Clinical research of dose - dense combinated pharmorubicin with paclitaxel in advanced breast cancer treatment

Purpose;To observe curative effect and side reaction ,histopathology change of dose - dense combinated pharmorubicin with paclitaxel in advanced breast cancer treatment. Methods ;38 patients with advanced breast cancer diagnosed by pathology, which had e-valuation index, pharmorubicin 50mg/m2 ,d1 , intravenous injection,paclitaxel 175mg/m2,d2,intravenous injection,every 2 weeks as 1 course of treatment,after accepted 2 - 4 course of treatment,all patients were evaluated. Results: All patients were followed up,clinical complete remission( CR) 10. 5% , clinical partial remission 71. 1% , total effective rate 81. 6% . effective rate in patents of initial treatment was much higher than that in patients of retreatmen; effective rate in patents of III stage also was much higher than that in patients of IV stage; low leucocyte and Neutropenia appeared III ,IV side reaction,occupying respectively 63.2% and 71. 1% ;7.9% patients febricity owing to low leucocyte were cured by G - CSF and antibiotics,all patients successfully accomplish chemotherapy. Other side reaction,for instance thrombocytopenia.anaemia,nausea and vomiting etc,mostly displayed I , II toxic reaction,but they can be tolerated. After chemotherapy,cancer cells degeneration . cellular necrosis . retrogression and interstitial fibrosis .inflammatory cell infiltration are obviously changed. Conclusion: dose -dense chemotherapy combinated pharmorubicin with paclitaxel in advanced breast cancer treatment, remission rate was much higher and remission rate can be tolerated.

Objective To study the bioequivalence of domestic ropivacaine mesylate in compared with imported ropivacaine hydrochloride in Beagle dogs.Methods Ropivacaine mesylate and ropivacaine hydrochloride at a dose of 10 ?mol/kg was administered intravenously in a randomized cross-over test.The plasma level of ropivacaine was measured by high performance liquid chromatography(HPLC).The pharmacokinetic parameters of ropivacaine were determined by the soft pack of Practical Pharmacokinetic Program Version 97(3P97).Results The pharmacokinetic parameters of ropivacaine mesylate were T_(1/2?)(3.83?1.89) min,T_(1/2?)(41.08?4.47) min,AUC_(0-200)(148.67?87.51) ?g?ml~(-1)?min~(-1),C_(max)(3.96?1.50) ?g/ml,CL(s)(0.030?0.014) L?kg~(-1)?min~(-1),and these parameters of ropivacaine hydrochloride were(3.09?1.19) min,(38.43?4.41)min,(125.80?81.06) ?g?ml~(-1)?min~(-1),(3.72?1.31) ?g/ml,(0.030?0.013) L?kg~(-1)?min~(-1) respectively.Conclusion The bioavailability of ropivacaine mesylate is equal to that of ropivacaine hydrochloride.

Tubulointerstitial fibrosis is the final common pathway of most progressive renal diseases, and also is the pathological basis that leading to chronic renal failure. In recent years, the biological effect of tissue transglutaminase in the progressive tubulointerstitium fibrosis attracts more and more attention. Tissue transglutaminase belongs to a group of calcium-dependent mammalian enzymes that have the capacity to irreversibly cross-link proteins through the formation of e (γ-glutamyl)lysine bonds, and lead to the excessive deposition of extracellular matrix, then functions markedly in the tubulointerstitial fibrosis.
