1.A Case of Severe Diabetic Ketoacidosis in a Child with Type 2 Diabetes.

Ketosis-prone diabetes includes heterogeneous disease groups characterized by provoked or unprovoked ketoacidosis (ketosis), with a typical phenotype of autoimmune type 1 diabetes. As the incidence of obesity and type 2 diabetes is universally increasing, the rate of type 2 diabetes in diabetic ketoacidosis in children and adolescents is exptected to increase rapidly. The clinical presentation of atypical ketoacidosis with type 2 diabetes has been reported mostly in adults. We recently experienced a case of a 10-year-old obese girl with new-onset type 2 diabetes who initially presented with severe diabetic ketoacidosis, and introduce it with literature reviews.

3.A Case of Levamisole Treatment for Kimura's Disease-Associated Nephrotic Syndrome.

Kimura's disease is a chronic inflammatory soft tissue disorder with peripheral eosinophila and high serum IgE. It might be associated with nephrotic syndrome. We report a 6-year-old boy with Kimura's disease and the concurrent steroid dependant minimal change nephrotic syndrome. With levamisole treatment fot 24 months, tumor progression and relapse of the associated nephrotic syndrome were not observed.

4.Brachiocephalic Venous Aneurysm Mimicking Metastatic Cervical Lymphadenopathy in a Patient with Gastric Cancer: A Case Report

Brachiocephalic venous aneurysm (BVA) development is an extremely rare, particularly as a primary vascular disorder. BVAs may be misinterpreted as lymphadenopathies owing to the variable degrees of enhancement seen in imaging studies, especially among patients with underlying malignancy. We report a BVA that mimicked lymph node metastasis on CT in a 60-year-old female who had undergone subtotal gastrectomy for stomach cancer. After followup chest CT with different bolus times and Doppler ultrasonography, a venous aneurysm originating from the brachiocephalic vein was diagnosed. We emphasize that, to make an accurate diagnosis, physicians should be aware of the potential diagnostic pitfalls and have a high index of suspicion for BVA when encountering certain lesions in the cervical area.
5.Analysis of Intraocular Lens Rotation during Combined Vitrectomy and Cataract Surgery

Purpose:
To evaluate the prevalence of intraocular lens rotation (IOL) and related factors during combined vitrectomy and cataract surgery.
Methods:
A university hospital, non-comparative pilot study. The medical records of patients who underwent combined vitrectomy and cataract surgery were retrospectively investigated. Surgical videos were analyzed to observe the degree and timing of rotation of IOL during vitrectomy. Scleral indentation and peripheral vitrectomy were started from 6 o’clock of the eyeball and proceeded counterclockwise, and the presence of rotation was defined when IOL rotated more than 2 hours (60 degrees).
Results:
A total of 181 eyes were enrolled in this study, and the rotation of IOL occurred in 13 eyes (7.2%). IOL rotation was observed in 11 out of 117 eyes (9.4%) using 1-piece IOL and 2 out of 64 eyes (3.1%) using 3-piece IOL, and the difference was not significant. The direction of IOL rotation was counterclockwise in 11 eyes (84.6%) and rotated during peripheral vitrectomy and scleral indentation in 12 eyes (92.3%).
Conclusions
Since the rotation of IOL may occur during combined vitrectomy and cataract surgery, the axis of astigmatism should be checked at the end of the operation when using toric IOL.
6.An Esophageal Leiomyoma Mistaken as an Esophageal Duplication Cyst

Esophageal leiomyoma is a benign tumor that accounts for two-thirds of esophageal benign tumors. In general, patients with esophageal leiomyoma are asymptomatic; however, as the tumor grows in size, symptoms such as dysphagia, chest discomfort, obstruction, or bleeding can appear. A 65-year-old man visited our clinic to receive treatment for a subepithelial tumor in the lower esophagus, which was incidentally found during endoscopy for work-up of chest discomfort and indigestion. On endoscopic ultrasonography (EUS), the tumor was 2.6×1.9 cm in size, with a cystic lesion located in the submucosal and proper muscle layers and thick cystic wall. Therefore, it was highly likely to be a duplication cyst. As the patient had symptoms such as chest discomfort, surgical resection was performed. The tumor was finally diagnosed as a leiomyoma with cystic degeneration. Leiomyoma with cystic change that has similar EUS features to duplication cyst is rare. Thus, we report the case with a literature review.

8.Sleep Tracking of Two Smartwatches Against Self-Reported Logs for Circadian Rhythm and Sleep Quality Assessment in Healthy Adults

Although many wearable devices are used to assess sleep, their accuracy remains controversial. This study aimed to investigate the accuracy of the Actiwatch, a research-grade device, and the Fitbit, a consumer-grade device, against sleep diaries to assess sleep patterns. Methods: Twenty participants wore Fitbit and Actiwatch for two weeks and tracked their sleep patterns using sleep diaries. Total sleep time (TST), time-in-bed (TIB), sleep efficiency (SE), sleep onset latency (SOL), and wake after sleep onset (WASO) from the two devices and sleep diaries were analyzed using analysis of variance and Bland-Altman analysis. Results: The TIB measured by the sleep log, Fitbit, and Actiwatch were 420.9 minutes, 417.3 minutes, and 567.4 minutes, respectively. Compared to the sleep log, the Fitbit underestimated TST, TIB, and SE, with significant differences observed for TST (p<0.001) and SE (p<0.001), but not for TIB. The Actiwatch overestimated TIB (p<0.001) and TST (p=0.02) and underestimated SE (p<0.001) compared to the sleep log. The difference between the Fitbit and Actiwatch was significant for TST, TIB, and SE (all p<0.001). Conclusions: The Fitbit showed a smaller difference than the Actiwatch when compared with the sleep logs. The Fitbit could be used as a tool to assess sleep patterns in the clinic as well as in daily life.
