1.Cognitive Function, Physical Function, Problematic Behaviors of Elders using Dementia Daycare Service and Reasons for Stopping Daycare.

PURPOSE: Purpose of this retrospective study was to investigate cognitive function, physical function, and problematic behaviors of elders who attended dementia daycare centers, and to identify reasons why they stopped using the center. METHODS: Participants were 176 elders, 60 years or over, attending one of four dementia daycare centers in Incheon. Data were collected from center documents. RESULTS: Mean age was 80.5 years. When admitted to the centers mean scores for the mini-mental status examination, activity of daily living, and instrumental activity of daily living (IADL) were 12.31, 9.53, and 25.09 respectively. Participants received day care service for an average of 17.98 months. The reasons for leaving the center were worsening dementia and health (40.2%), and problematic behaviors (20.1%). CONCLUSION: Results show that elders began to use day care services when their cognitive function and IADL had declined considerably. As the ultimate goal of dementia daycare service is to delay the worsening of cognitive capability and decreases in activities daily living, the effect of the service can be maximized when the service is provided as early as possible in the course of progressively severe dementia. Active promotion should be exerted in the community to encourage early use of this service.

2.A Day Hospital Model for Integrated Therapy of Psychotic Patients.

OBJECTS: The purpose of this study was to present a day hospital model(hereinafter called as "this model") for integrated therapy, to evaluate the result of performing this model, and thus to develop day hospital model for helping psychotic patients effectively and promoting social readaptation. METHODS: The authors performed this day hospital model from 1996, May to December, A total of 25 psychotic Patients participated in this study. BPRS, Life Satisfaction Self-Rating Scale, Program Helpfulness Scale, 13 Therapeutic Factors, Scale, and Most Important Event Questionnaire were used at the start of day hospital treatment and at the end. RESULTS: The results were as follows: 1) In this model, it was possible to offer the wide treatment to heterogeneous patients who varied in age, number of admission, duration of illness, and functional level. 2) In the aspect of therapeutic effect, this model contributed for improving negative symptoms and increasing subjective life satisfaction. 3) When the patients were divided into the higher functional level group and the lower functional level group, the higher functional level group set a high value on group psychotherapy and psychodrama that were unstructured and could make patients explore their inner side deeply, But the lower functional level group did not so. Therefore dividing into two groups by functional level may be more effective for day hospital treatment. 4) Therapeutic factors were different between the periods of day hospital treatment, And in the higher functional level group, 'interpersonal learning' was important at the last period, but in the lower functional level group, 'socializing technique' was important at the last period. So long-term group psychotherapy may be more suitable for the after-care of the higher functional level group, and day care or other psychosocial rehabilitation programs may be more suitable for the after-care of the lower functional level group. 5) This model was easy to apply to Korean day hospital setting, and made staffs treat patients efficiently CONCLUSION: This day hospital model would be used effectively in Korean day hospital setting.

3.Family Membe's Caregiving Stress and Satisfaction of Care for Patients with Dementia in Long Term Care Facilities and Day Care Centers.

PURPOSE: This study was to explore family member's caregiving stress and satisfaction of care for patients with dementia in long term care facilities and day care centers. METHODS: The subjects were 129 family members of elders with dementia from ten long term care facilities and eight day care centers in Daegu and Kyungsangbukdo. Data were collected from August to October, 2007. The instruments were self administered questionnaires and included. The Family Perceptions of Caregiving Role (FPCR) and the Family Perceptions of Care Tool (FPCT) which were developed by Maas and Buckwalter (1990) and translated by Park (2002). RESULTS: Family member's care giving stress in day care centers were higher than that of long term care facilities (t=-2.89, p=.005) especially in the categories of captivity (t=-3.27, p=.001), guilty (t=-2.93, p=.004), and loss (t=-2.44, p=.016). Family member's satisfaction of care in day care centers was higher than that of long term care facilities (t=-3.21, p=.002) in the (use - categories or measures since you are referring to the instrument and delete aspects) aspects of effective management (t=-3.69, p=.000) and activity (t=-2.00, p=.045). CONCLUSION: The results of this study showed that family members' perceptions toward their care giving roles and satisfaction of care differ whether the facilities are long term care or day care centers. This study provides baseline data that could be used for improving the quality of long term care services.

4.A Study on Changes of Primary Caregivers' Fatigue, Depression and Life Satisfaction by Using Dementia Day Care Service.

PURPOSE: This study is to examine how dementia day care service affects fatigue, depression and life satisfaction of caregiver. METHODS: The study was conducted using a convenient sampling method from 6 Dementia Day Care Center in Incheon. Thirty nine primary caregivers answered the questionnaires. Pre-test was done before demented elders start using the dementia day care center and post-test was done five months after. Data were analyzed by using descriptive statistics and paired t-test. RESULTS: There was a significant difference in caregivers' fatigue level after using dementia day care service(t=2.188, p=.035). Results of subcategories were as follows; There was a significant difference in caregivers' physical fatigue level(t=2.270, p=.029) and psychological fatigue level(t=2.277, p=.029) after using dementia day care service. However, there was not significant difference in caregivers' neurological fatigue level(t=1.312, p=.197). There was a significant difference in caregivers' depression level(t=3.066, p=.004) and life satisfaction(t=-2.131, p=.040) after using dementia day care service. CONCLUSION: The results indicated that dementia day care service is helpful for family with demented elders. Therefore it is necessary to expand dementia day care center in terms of its size and numbers in order to support increasing number of demented elders and their families.

5.The Study for Development of Day Hospital Program of Eating Disorders.

PURPOSE: The purpose of this study was to develop the day hospital program for Korean eating disorders patients and to examine the effect of the day hospital program on improving frequency of binging and purging, eating disorders symptoms, self-esteem, and depression. METHOD: The subjects were 24 binge eaters visited at eating disorders clinic "M". They participated in a modified day hospital program based on the Toronto Day Hospital Program. All subjects completed the Eating Disorders Inventory(EDI)-2, Rosenberg Self-Eesteem Scale, Beck Depression Inventory(BDI) at pre and post intervention, and recorded daily food records. RESULT: In paired t-test analysis, frequency of binging and purging, self-esteem, depression, and 11 sub-scales of EDI-2(drive for thinness, bulimia, body dissatisfaction, ineffectiveness, interoceptive awareness, perfectionism, interpersonal distrust, maturity fear, asceticism, impulse regulation, social insecurity) were significantly improved after treatment. CONCLUSION: The results of this study suggest that the day hospital program for Korean eating disorders patients may be an effective initial approach to overcoming various eating disorders symptoms. Further studies should investigate longer term outcome data using a larger sample.

6.The Effects of Perioperative Supplementation of EPA on Immunosuppression Induced by Postoperative Chemo-radiation Therapy for Esophageal Cancer

Aim: The effects of or al orenteral administration of eicosapentaenoic acid (EPA) on suppression of the immune function induced by postoperative chemo-radiation therapy in patients with esophageal cancer were studied.Methods: Thirteen patients received TPN, including Intralipid, from 1 week before the operation until 2 weeks after the operation and were then switched to enteral feeding containing 17.5% MCT and 7.5% safflower oil. EPA methyl esther was administered orally at a dose of 1.8g/day with the parenteral or the enteral diet to the “EPA” group (n=5) from 1 week before the operation until discharge. EPA was not given to the “Control” group (n=8). PHA-and ConA-stimulated lymphocyte proliferation, NK activity, and total lymphocyte and T cell counts were measured on admission as well as in the 3rd, 7th, and 10th postoperative weeks. Starting after 3 to 4 postoperative weeks, postoperative chemoradiation therapy (CBDCA 30mg/day, 5-FU 250mg/day, radiation 1.8Gy/day) was performed for 5 consective day each week for 4 to 5 weeks.Results: Both ConA-and PHA-stimmulated lymphocyte proliferation decreased significantly on the 10th postoperative day in both groups. Improvement in cell-mediated immune function in the 3rd postoperative week was marked in the “EPA” group (p<0.01 in PHA and p<0.02 in ConA) when compared to the “Control” group. Improvement in lymphocyte proliferation was higher in the “EPA” group followong the completion of chemoradiation therapy (p<0.005 “EPA” vs. “Contro” in ConA after 10 weeks). NK activity was also higher in the “EPA” group following the completion of chemoradiation therapy (p<0.004 “EPA” vs. “Control” after 10 weeks). There were no significant differences, however, in WBC, total lymphocyte count, and T cell count.Conclusion: The results suggest that enteral/oral EPA reduced immunosuppression induced by postoperative chemo-radiation therapy in patients who underwent an operation for esophageal cancer.

7.Expert consensus on day care cancer diagnosis and treatment in China (2022 Edition).

In order to thoroughly implement the "Guiding Opinions of the General Office of the State Council on Promoting the Construction of a Hierarchical Diagnosis and Treatment System" (Guobanfa 〔2015〕 No. 70), the day care diagnosis and treatment services are taken as the starting point, to promote the realization of separate treatment of acute and chronic diseases, improve the use of medical resources and the efficiency of medical services, save medical insurance funds, reduce the financial burden of patients, and improve the quality of life of patients, so that more patients can receive much-needed treatment. Based on the current situation of day care cancer diagnosis and treatment in China and the existing problems in management mode, system, process, safety management, information construction, etc, multidisciplinary experts synthesized domestic and foreign literature, combined with clinical practice, discussed and revised repeatedly, finally formulated the "Expert consensus on day care cancer diagnosis and treatment in China (2022 edition)". It aims to provide better day care diagnosis and treatment guidance for clinical oncologists, promote the further development of day care cancer diagnosis and treatment in China, improve the effective utilization of social medical resources, and better serve patients, hospitals and society.

8.Factors Affecting Daycare Center Teachers' Management of Childhood Fever.

PURPOSE: The purposes of this study were to investigate daycare teachers'knowledge, anxiety and management of childhood fever and to identify factors affecting their management of childhood fever. METHODS: A descriptive correlation study was conducted. Participants were 109 daycare center teachers from G metropolitan city. A self-administered questionnaire was used for data collection. RESULTS: The mean percent of correct answers for knowledge about childhood fever was 42.9%. Levels of anxiety related to children's fever were high among day care teachers in this study. Many daycare center teachers used management practices which were not recommended for childhood fever management. There was a statistically significant difference in participants' knowledge about childhood fever by marital status and having a child. Moreover, there were statistically significant positive correlation between daycare center teachers' knowledge and management and negative correlation between daycare center teachers' anxiety and management of childhood fever. Knowledge and anxiety related to childhood fever accounted for 15.2% of variance for fever management. CONCLUSION: Results show that day care teachers' knowledge and anxiety related to fever are important factors in predicting their fever management. Development and evaluation of educational interventions to improve daycare teachers' childhood fever management are recommended.

9.Perception of Child Day Care Center Teachers on Issues and Needs in Child Health Management.

PURPOSE: The purpose of this study was to investigate issues and needs related to child health management as recognized by teachers in day care centers. METHODS: Data were collected from 130 teachers from 20 day care centers. Each teacher was interviewed in depth and the data were examined through content analysis. RESULTS: For issues on child health management, the analysis scheme consisted of 10 categories and 36 subcategories. For needs on child health management, the analysis scheme consisted of 10 categories and 37 subcategories. Teachers indicated that tools for physical examination and playgrounds were insufficient and they had difficulty in controlling attendance of infectious children. They also indicated the lack of a linkage between day care centers and hospitals, lack of knowledge of health management, high teacher versus child ratio, and lack of help and budget needed for health care. CONCLUSION: The results of this study suggest the need to develop a linkage between day care centers and medical institutions. Also there is a need to provide support from health care personnel, health education for teacher, health management manuals for teachers, and financial aid to satisfy the needs for health management in day care center for children.

10.Knowledge Related to Child Obesity of Child Daycare Center Worker.

PURPOSE: The purpose of this study was to investigate knowledge related to child obesity of child daycare center workers. METHODS: The participants in this study were 360 child daycare center workers in 2 cities in South Gyeongsang Province. Data were collected from February to June, 2011, using self-report questionnaires including questions about the general characteristics of the daycare center workers and their knowledge related to child obesity. Frequency, percentage, arithmetic mean, standard deviation, t-test and ANOVA were used to analyze the data with the SPSS WIN 16.0 program. RESULTS: The knowledge scores of child daycare center workers about child obesity averaged 74.8. The domains of diet (0.87 points) and prevention (0.86 points) had high scores, while, the domains of general characteristics (0.64 points) and diagnosis (0.59 points) had low scores. CONCLUSION: These results indicate that obese education programs for child daycare center workers should be developed in order to identify and teach children with obesity appropriately.
