2.Discussions on ischemic stroke basic pathogenesis, blood stasis caused by deficiency of both Qi and Yin, and its therapeutic application of Yangyin Yiqi Huoxue

Based on our research team's clinical and basic research results of traditional Chinese medicine (TCM) in the prevention and treatment of ischemic stroke, and combined with TCM syndrome standard manifestations of this disease, wind, phlegm, fire, deficiency and blood stasis, this paper points out definitely that the basic pathogenesis of ischemic stroke is blood stasis caused by deficiency of both Qi and Yin. The pathogenic characteristics are deficiency in origin and excess in superficiality, and the wind, fire, phlegm, deficiency and blood stasis are related one another resulting in this disease; it is established that the basic syndrome types of this disease are deficiency of both Qi and Yin and static blood blocking the collaterals, and the therapeutic regime is a recipe including the TCM ingredients having the actions of nourishing yin, tonifying Qi and improving blood circulation (Yangyin Yiqi Huoxue) and according to the patient's individual variation of syndrome differentiation, the ingredients in the recipe can be added or subtracted. In this report, a new system of treatment for ischemic stroke based on syndrome differentiation is proposed.
3.Cytotoxicity and mitochondrial injury mechanism of matrine on PC12 cells

OBJECTIVE To study the toxicological effect of matrine (MT) on PC12 cells and the mechanism of mitochondrial damage. METHODS After treatment with MT 2, 4 and 8 mmol·L-1for 8, 16,24 and 48 h,respectively,the viability of PC12 cells was measured with methylthiazolyltetrazolium assay.PC12 cells were treated with MT 2,4 and 8 mmol·L-1for 24 h,before the morphological changes were observed by Hoechst33342 staining. The superoxide dismutase (SOD) activity and malondialde-hyde (MDA) content in cells were detected using hydroxylamine method and thiobarbituric acid method, apoptosis rate and reactive oxygen species (ROS) of PC12 cells were detected with flow cytometry, mitochondrial membrane potential (MMP) was detected with JC-1 staining, and the expressions of procaspase 3, procaspase 9, cleaved-caspase 3, Bax and Bcl-2 were detected with Western blotting. RESULTS MT inhibited the growth of PC12 cells in a time-and concentration-dependent manner.After being treated with MT for 24 h,the nuclei of PC12 cells in MT groups showed chromatin agglutination and partial rupture to different degrees,and compared with normal control group,the apoptosis rates of MT 2,4 and 8 mmol·L-1groups were significantly increased(P<0.01).Intracellular ROS and MDA levels increased (P<0.05, P<0.01), SOD activity decreased (P<0.01), and MMP decreased. The expressions of procaspase 9, procaspase 3 and Bcl-2 were significantly decreased (P<0.01, P<0.05), the expres-sions of cleaved-caspase 3 and Bax were significantly increased(P<0.05,P<0.01),and the ratio of Bcl-2/Bax was significantly decreased (P<0.01). CONCLUSION MT has toxic effect on PC12 cells and induces apoptosis through ROS mediated mitochondrial pathway.
4.Extraction Optimization of Quercetin and Kaempferol from Lindera Aggregate Leaves by Response Surface Method

Objective:To obtain the optimum extraction conditions for quercetin and kaempferol from Lindera aggregate leaves. Methods:On the basis of single factor investigation, Box-Behnken experimental design was used to provide the experimental data for establishing a regression model for the extraction of quercetin and kaempferol. Response surface and contour diagrams with the extrac-tion yield of quercetin and kaempferol as the response values,were plotted for analyzing the pairwise interactive effects of hydrochloric acid concentration, hydrolysis temperature and hydrolysis time. Results:The hydrochloric acid concentration of 4. 1%, the hydrolysis temperature of 83℃ and the hydrolysis time of 45min were the optimum extraction conditions. With the above conditions, the content of quercetin and kaempferol from Lindera aggregate leaves was 6. 97 mg·g-1 and 2. 82 mg·g-1 , respectively. The Lindera aggregate beaves from five different habitals were analyzed,and the total content of quercetin and kaempferide from Taizhou Tiantai were highest. Conclusion:The quercetin and kaempferol extraction from Lindera aggregate leaves is optimized by Box-Behnken response surface method and the process is convenient and feasible.

6.Study on the factors affecting community nurses to carry out continuing nursing service factors in community chronic diseases

Objective To understand the community transitional care in the process of development of the difficulties and problems. Methods The qualitative research method was used to conduct semi-structured interviews with community nurses and community care managers(11 cases) who met the standards and analyzed the interview data with Colaizzi 7 footwork. Results The factors that affected the effective care of the community were:there was no clear continuation of nursing service fees, lack of internal and external incentive mechanism; home service risk, no security; community human resources, job responsibilities were not clear; the mechanism was not perfect, the lack of medical cooperation with large hospitals;community nurses professional ability uneven;patients did not trust the community health service agencies, community nurses were not taken seriously. Conclusions There are many obstacles in the community to carry out continuous care, the establishment of hospital-community-family linkage type of continuous care to the government and the Ministry of Health within the practical issues for the development of effective and effective care service system and policy.
7.The study on the participation of leisure activities and the cognitive function of the elderly

Objective To investigate the status of cognitive function and leisure activity participation among the elderly in urban communities, and to explore the relationship between cognitive activities and leisure activities. Methods Using the General Demographic Data Questionnaire,the Montreal Cognitive Assessment Scale and Leisure Activities Questionnaire, 528 elderly people were investigated. Results The cognitive function were (25.65 ± 2.58), (25.76 ± 2.48)points in elderly with recreation and intelligence activities respectively, and (25.21 ± 2.53), (25.58 ± 2.60)points in elderly without recreation and intelligence activities respectively, the difference was statistically significant ( Z=5.56, 8.16, P<0.01). There was no significant difference between elderly with physical activity and social activities and without physical activity and social activities (P>0.05). The participation of intelligence and recreation was positively correlated with cognitive function (r =0.347, 0.270, P < 0.01), while social activities and physical activities were not correlated with cognitive function (P>0.05). Conclusions Nursing staff should pay more attention to the decline of cognitive function of the elderly, so that the elderly can recognize the importance of leisure activity, improve their enthusiasm, participate in science, enrich the quality of life in later years, improve cognitive function.
8.The mediating effect of organizational commitment on relationship between perceived professional benefit and turnover intention among clinical nurses

Objective To explore the mediating effect of organizational commitment on the relationship between perceived professional benefit and turnover intention among clinical nurses. Methods A total of 456 nurses from 3 hospitals in Hangzhou were investigated with Occupational Benefit Questionnaire,Organizational Commitment Scale and Turnover Intention Scale. Results The score of perceived professional benefit, organizational commitment and turnover intention were(135.41 ± 19.01), (16.10 ± 2.87),(76.28 ± 15.80) respectively. Pearson correlation analysis showed that there was a significant positive correlation between perceived professional benefit and organizational commitment(r=0.476, P<0.01),and a significant negative correlation to turnover intention(r=-0.313, P<0.01),and a significant negative correlation between organizational commitment and turnover intention(r=-0.525, P<0.01).Organizational commitment played a partial mediating effect between perceived professional benefit and turnover intention among nurses. Conclusions Nursing managers should attach an importance to enhance organizational commitment,and take effective measures to improve perceived professional benefit, as well as establish an effective incentive mechanism, fully mobilize positive factors to reduce the nurses ' turnover rate and stable the nursing team.
9.Research progress of compassion fatigue in oncologic nurses

10.Evaluation of the Analgesic Effect of TENS After Cesarean Section and Its Effect on Postpartum Stress

Objective To observe the analgesic effect of transcutaneous electrical nerve stimulation (TENS) after cesarean section, and its effect on postpartum stress-related hormones. Method By following a double-blinded design, 58 primiparas were randomized into TENS group, PCA group, and sham TENS group. The time of pre-analgesia was considered as 1 h after the operation. Visual Analogue Scale (VAS) was measured at 7 time points (pre-analgesia, 5 min after analgesia, 15 min after analgesia, 30 min after analgesia, 60 min after analgesia, 24 h after analgesia, and 48 h after analgesia). The postpartum stress-related hormones adrenocorticotrophic hormone (ACTH) and prostaglandin (PG) were detected at 3 time points: before the operation (24 h prior to cesarean section), 24 h after the operation, and 48 h after the operation. The effects of the three analgesic methods on postpartum stress-related hormones were compared. Result The VAS score was significantly lower in TENS group than in sham TENS group 30 min after analgesia (P<0.05); there were no significant differences in VAS scores between TENS and PCA groups since 30 min after analgesia (P>0.05). At 24 h prior to the operation, and 24 h and 48 h after the operation, there were no significant between-group differences in ACTH (P>0.05). At 24 h prior to the operation, and 24 h and 48 h after the operation, there were no significant between-group differences in the level of PG (P>0.05); but in TENS and PCA groups, the PG level at 48 h after the operation was significantly lower than that at 24 h after the operation (P<0.05). Conclusion TENS hascertain analgesic effect, and it can reach equivalent analgesic effect compared with PCA when working over 30 min. Both TENS and PCA can down-regulate the level of PG. The three analgesia methods produce similar effects on blood ACTH.
