1.A prospective study of early maternal intervention on the prevention of infantile eczema effect

Objective To explore the effect of early maternal intervention and provide clinical suggestions for the prevention and treatment of infantile eczema. Methods A total of 340 pregnant woman who were receiving prenatal examine and giving birth to their children from January to June 2016 in People's Hospital of Xinjiang Uygur Autonomous Region were selected in this study. The pregnant woman were divided into two groups--control group and observation group by the random number table method.Each group had 170 history cases.The control group was given regularly health guidance,and the observation group was added early intervention such as translate the latest information on the modern social media to them,and giving suggestions of diet and nutrition of infants and living environment.The proportion of risk factors--the each group of infants' genetic history of allergies, birth of ways,feeding pattern and the kind of environmental of which they exposure and so on were analyzed,then the incidence of eczema, the recurrence rate of eczema, the treatment effects and duration of eczema were contrasted.Results After 6 months of prospective follow-up, the observation group of cesarean section and exposing to smoking environmental proportion was respectively 48.82%(83/170),19.41%(33/170), was lower than that of control group 64.71%(110/170),42.35%(72/170),the difference between 2 groups was statistically significant (χ2=8.736, 20.958, P<0.01). The proportion of 4- 6 months of exclusive breastfeeding and correctly adding supplement food respectively 52.35%(89/170),78.24%(133/170),was higher than that in the control group 42.94%(73/170),58.82%(100/170),the difference between 2 groups was statistically significant (χ2=3.418,14.851, P<0.01). After 6 months of prospective follow-up, the incidence and recurrence rate of infants eczema respectively in observation group was 25.88%(44/170), 22.73%(10/170),was lower than that in control group 49.41%(84/170),42.86%(36/170),the difference between 2 groups was statistically significant (χ2=20.047, 5.082, P<0.05).The treatment effect in observation group was superior to the control group.The last,the period of eczema was shorter than that of control group. All of them had statistical meaning(Z=8.876, 10.195,all P<0.05). Conclusions Early interventions can reduce the proportion of high-risk factors that could induce infant eczema, such as cesarean section,exposure to smoking,environmental and other factors.In order to reduce the incidence of infants eczema, improve family′s compliance, reduce eczema attack, improve symptoms, reduce the burden on the family to a great extent,to ensure the normal growth and development of infants,cut off the"allergy process"to prevent distal disease such as allergic asthma,allergic rhinitis etc.
2.Effect of Dracocephalum Heterophyllum Benth Flavonoid on Myocardial Cell Hypertrophy Induced by Noradrenaline in Rats

Objective To probe into effect of Dracocephalum heterophyllum Benth flavonoids(DHBF) on noradrenaline (NE) -induced myocardial cell hypertrophy, and to study the mechanism. Methods SD neonatal rat myocardial cells were cultured in vitro. The experiment was divided into normal control group, model control group (NE 2 μmol·L-1) , prazosin group (prazosin 50 μmol·L-1) ,low-,medium-and high-dose DHBF group (DHBF 10,25,50 μmol·L-1) .Cardiomyocyte hypertrophy were induced by NE(2 μmol·L-1) . DHBF and prazosin were intervened respectively. CCK-8 method was used to observe the activity of myocardial cells. RT-PCR technique was used to detect the expression of mRNA of cardiac hypertrophy c-jun and brain natriuretic peptide (BNP) ,and Western blotting was used to detect the protein expression of CaN and NFAT-3 in myocardial cells. Confocal laser scanning was used to detect the surface area of myocardial cell. Results Compared with the normal control group, survival rate of cardiomyocytes was significantly decreased, expression of mRNA of c-jun and BNP significantly upregulated, protein expression of CaN and NFAT-3 decreased, and surface area increased in model control group (P<0.05) . Compared with the cell of model control group, low-,medium-and high-dose DHBF significantly reversed NE-induced decrease of the survival rate, increase of surface area, increase of c-jun and BNP mRNA, and increase of CaN and NFAT-3 protein expression (P<0.05) . Conclusion DHBF can improve the survival rate of cardiac hypertrophy patients, down-regulate c-jun and BNP mRNA expression, decrease CaN and NFAT-3 protein expression, and decrease NE-induced surface area of cardiomyocytes.
3.Nested case-control study on the relationship between risk factors of pregnancy and infantile eczema in Uygur and Han ethnic

Objective To investigate the prevalence of infantile eczema in Uygur and Han nationalities and the relationship between pregnancy factors and 0-6 months infant eczema, so as to provide a scientific basis for formulating preventive strategies. Methods From January-August 2016 in the Xinjiang Autonomous Region People's Hospital of obstetric and follow-up to fill out the questionnaire in 570 pregnant women and their babies as the research object to establish the baseline survey cohort, pregnancy related information to pregnant women, and 6 months (January, March, on maternal June) follow-up, observe the occurrence of infantile eczema a total of 502, successful follow-up to maternal and child. Using nested case-control study, the cohort occurred in 182 cases of infant eczema as case group, will be in the queue without eczema in 320 infants as control group, using multivariate logistic regression analysis of risk factors during pregnancy baby eczema. Results 0-6 months infant eczema incidence rate was 36.25%, the incidence of Uygur infant eczema (20.43%) was significantly lower than that of Han nationality infant eczema incidence (50.19%) (OR=0.45,95%CI:0.35-0.69) P<0.01). Social and economic factors affecting the occurrence of infantile eczema comparison results showed that the ethnic factors and infant eczema, and more prone to eczema than that of Uygur nationality; and Uygur compared mothers influence allergic history of Han infantile eczema more (χ2=7.09, P=0.008). Multivariate logistic regression analysis showed that the mother has a history of atopy (OR=7.17,95%CI:4.15-12.37), mother′s education level (OR=1.91,95%CI:1.51-2.74), the higher the monthly income (OR=2.68,95%CI:2.17-3.33), maternal smoking during pregnancy (secondhand smoke) (OR=2.42,95%CI:1.96-2.97), frequently eating spicy spicy foods during pregnancy (OR=1.94,95%CI: 1.41-2.68) will increase the risk of infant eczema Supplementation of probiotics (OR=0.13,95%CI:0.07-0.25) and vitamin D (OR=0.13,95%CI:0.06-0.26) during pregnancy is a protective factor in the development of infantile eczema. Conclusions Compared to Uygur, Han Chinese infants are more likely occur eczema, and family history of allergies, education, economic income, pregnancy dietand smoking are risk factors for infant eczema. The same risk factors among Uygur and Han ethnic ,but it had a different results. Compared to Han, Uygur pregnant women eating spicy food caused the risk of infant eczema occurrence is higher, but the short period of housing renovation caused the risk of infant eczema occurrence is lower. Therefore, it is necessary to strengthen nutrition andmonitor living environment during pregnancy. Avoiding to eat spicy food and supplying probiotics and vitamin D, it can prevent or reduce the occurrence of infantile eczema.
4.Qualitative research on demand for nursing personnel with customized mode in different levels of hospitals in Xinjiang

Objective To explore the cognition and demand status of order oriented nursing talents in different hospitals in Xinjiang,so as to provide a scientific basis for establishing the model in Xinjiang. Methods Using qualitative research methods and purposive sampling, 12 nursing directors from different grades of hospitals in North and South Xinjiang were selected and semi structured in-depth interviews were conducted from September 2016 to October 2016. Colaizzi phenomenological seven step analysis method was used to analyze the data and get the theme. Results Through analysis, the three themes of different types of hospitals in Xinjiang were put forward: there are different levels of hospitals in order to model the demand of nursing personnel training;different levels of hospital nursing students'needs of different orders;the hospital expects to establish order type talent training hospital-school cooperation steering committee. Conclusions Xinjiang area hospitals of different levels to construct the pattern of the urgent need for the"order"of nursing personnel training,to meet the needs of nursing professionals of different levels of hospitals, so as to improve the overall level of nursing staff, to provide quality health services along the Silk Road and the surrounding countries.
5.Prospective study on the incidence of PICC complications and its influencing factors in cancer patients during chemotherapy in Xinjiang

Objective To explore the incidence and risk factors of common complications in PICC patients during chemotherapy in Xinjiang.Methods A prospective study was conducted.The patients with PICC were treated with PICC from April 2016 to April 2017 were followed up for 10 months. The incidence of PICC complication and the influencing factors were observed dynamically during chemotherapy.Results The incidence of PICC complication was 13.6%(42/208)during chemotherapy in Xinjiang patients.The incidence of PICC was higher in the third chemotherapy cycle and the incidences of the first and sixth chemotherapy cycles were lower. Age, valve type, number of punctures, maintenance time>7d, and living conditions were the influencing factors of PICC complications(P<0.05). Conclusions The incidence of common complications of PICC in cancer patients during chemotherapy is high in Xinjiang region.The risk factors of complications include age<18 years,number of open-end PICCs,increased number of punctures, maintenance time>7d and living conditions for rural areas. Clinical nurses should give personalized health guidance on the causes of complications in different patients to reduce the occurrence of complications of PICC,and improve the quality of PICC use.
6.Association of related knowledge level and compliance of young and middle-aged hypertensive patients

Objective To evaluate the relevant knowledge level and compliance of young and middle-aged hypertensive patients,and explore the association of them. Method A total of 276 young and middle-aged hypertensive patients were surveyed using a hypertension knowledge level scale and treatment compliance questionnaire. Results The average score of young and middle-aged hypertensive patients on relevant knowledge was (62.55 ± 17.79). From different dimensions, the hypertensive patients got the highest score in the knowledge of their lifestyle,while they got the lowest score in their knowledge for complication.The average score of young and middle-aged hypertensive patients on treatment compliance was (71.53±9.87).From different dimensions,the hypertensive patients got the highest score in medication adherence,while they got the lowest score in exercise and pressure-relieving compliance.It showed that medication and drug compliance knowledge of middle-aged hypertensive patients was closely associated with treatment compliance(r=0.648, P<0.001)). Conclusions Both knowledge level and treatment compliance of young and middle-aged hypertensive patients should be improved.We should strengthen the population of knowledge about hypertension among young and middle-aged patients,especially the guidance of drug knowledge to improve their compliance.
7.Psychological distress and post-traumatic growth experience of patients with cervical cancer after chemotherapy: a qualitative study

Objective To investigate the psychological experience of psychological pain and post-traumatic growth of cervical cancer patients with postoperative chemotherapy, and provide evidence for a more comprehensive and scientific psychological intervention plan. Methods In this qualitative study, the phenomenological study method was used to conduct deep semi-structured interviews with 12 cervical cancer patients treated with postoperative chemotherapy. The collected data were summarized by Colaizzi's seven-step content analysis method. Results The analysis based on the main theme of psychological pain reached four sub-themes:practical problems, communication problems, emotional problems, physical problems and the analysis based on the main theme of posttraumatic growth reached three sub-themes: interpersonal relationships, life perceptions and personal strength, spiritual change and new possibilities. Conclusions In the postoperative chemotherapy period, patients with cervical cancer encounter psychological pain and post-traumatic growth. Health care givers should combine the positive and negative emotions of patients to consider the psychological situation and develop a practical and comprehensive psychological intervention plan for them.
8.A comparative study on emergency triage efficiency between emergency rapid triage system and emergency severity classification of 4 level

Objective To probe into the emergency triage efficiency between emergency rapid triage system (ERTS) and emergency severity classification of 4 level. Methods A total of 46965 emergency patients from January to December in 2015 were selected as the research object and utilized a data of patients from the electronic triage system, and triaged the emergency patients by means of ERTS retrospectively. A comparison was performed in patient visits, outcome and hospital rates between ERTS and emergency severity classification of 4 level. Results In both of these triage systems, almost 80%of patients are assigned to the lowest acuity (levelⅢandⅣ). The visits of patients with gradeⅢand grade IV diseases were 71.5%(33580/46965), 50.5%(23717/46965) in ERTS and 12.1%(5683/46965), 35.1%(16485/46965) in emergency severity classification of 4 level respectively. The outcomes of patient under the ERTS were better than emergency severity classification of 4 level at levelⅠandⅡ(χ2=11.79, 1100.62, P<0.05). At levelⅠandⅡ, ERTS in the patient's disease hospital admission rates were 70.5%(311/441), 72.6%(5274/7264), which higher than emergency severity classification of 4 level, 62.5%(371/594), 45.2%(2785/6161), the difference was statistically significant (χ2=7.32, 1043.13, P<0.05). At level Ⅲ and Ⅳ, ERTS in the patient's disease hospital admission rates were 9.6%(3676/38288), 7.9%(77/972) , which lower than emergency severity classification of 4 level, 19.3%(4578/23719) , 9.9%(1632/16491), the difference was statistically significant (χ2=1153.10, 4.05, P<0.05). Conclusions ERTS significantly improves the sensitivity and accuracy of emergency triage compared to emergency severity classification of 4 level. ERTS has obvious significance in the emergency patient distribution, and also has a certain advantage in the patient outcomes and hospitalization rates. It can be used as a auxiliary tool.
9.Dignity status of lymphoma patients during perichemotherapy and its influencing factors

Objective:To investigate the dignity status of lymphoma patients during perichemotherapy, and to analyze the influencing factors.Methods:The convenience sampling method was used to investigate 230 lymphoma patients during perichemotherapy in Affiliated Tumor Hospital of Xinjiang Medical University from August 2020 to February 2021 by using general data questionnaire and patient dignity scale. Dual logistic regression was used to analyze the influencing factors of dignity.Results:The dignity score of patients with malignant lymphoma during perichemotherapy was 42.1±16.8, and 30.0% (69/230) patients had obvious dignity impairment. Among 5 dimensions of patient dignity, the scores of physical and mental pain, self-cognition, social role function independence and social support were 1.8±0.8, 1.8±0.9, 1.7±0.9, 1.6±0.7 and 1.2±0.5, respectively. Each patient had an average of 4.1 dignity problems, and five items with the highest score were worrying about their future (physical and mental pain dimension) [(2.1±1.1) scores, 50.4% (116/230)], feeling uncomfortable (physical and mental pain dimension) [(2.0±1.1) scores, 56.5% (130/230)], feeling a burden to others (self-perception dimension) [(2.0±1.1) scores, 33.5% (77/230)], feeling depressed (physical and mental pain dimension) [(1.9±1.0) scores, 53.5% (123/230)], feeling like a different person (self-perception dimension) [(1.9±1.1) scores, 49.1% (113/230)]. Logistic regression analysis showed that widowed (compared with unmarried: OR = 2.108, 95% CI 1.562-3.570), barely affordable and difficult to afford (compared with fully affordable: OR = 2.163, 4.307, respectively; 95% CI 1.120-7.469, 1.374-12.807), disease acceptance and negative (compared with positive: OR = 2.003, 3.694, respectively; 95% CI 1.358-6.599, 1.221-9.061), Eastern Cooperative Oncology Group (ECOG) score 2-4 (compared with score 0: OR = 3.753, 4.720, 5.044, respectively; 95% CI 1.352-9.506, 1.514-12.793, 2.221-15.539), patients with B symptom (compared with those without B symptom: OR = 1.962, 95% CI 1.100-3.501), patients with severe chemotherapy reaction (compared with those without severe chemotherapy reaction: OR = 4.814, 95% CI 2.714-8.539) were independent risk factors for obvious dignity loss of patients during perichemotherapy (all P < 0.05), and married was an independent protective factor compared with unmarried ( OR = 0.192, 95% CI 0.036-0.807, P < 0.05). Conclusion:Patients with lymphoma have impaired dignity during perichemotherapy. Medical staff should pay attention to the influencing factors of impaired dignity and develop targeted intervention programs to reduce the impaired dignity of patients.
10.Construction and application of clinical nursing procedure of pulmonary rehabilitation for thoracoscopic surgery of lung cancer

Objective:To construct clinical nursing procedure of pulmonary rehabilitation for thoracoscopic surgery of lung cancer, and to test its application effect.Methods:The control group ( n=41) received routine nursing care from December 2018 to May 2019; the intervention group ( n=41) received clinical nursing pathway of pulmonary rehabilitation from June to October, 2019. The 6MWT, Brog score and incidence of pulmonary complications and quality of life were compared between the two groups. Results:The results showed that 6-minute walk test (6 MWT) was (471.22±54.89) m, (325.38±17.65) m, and (407.27±13.84) m before surgery, at discharge time and 30 d after discharge, in the intervention respectively, which were longer than those of the control group [(440.95±70.51) m, (306.69±21.49) m, and (355.94±26.10) m] ( t values were -2.169, -4.304, -11.128, P<0.05). The total incidence of postoperative pulmonary complications was 4.9% (2/41) in the intervention, which was less than 19.5%(8/41) in the control group ( χ2 value was 0.088, P<0.05). The values of physical function and cognitive function 30 d after surgery was 80(73, 87), 83(83, 83) in the intervention, respectively, higher than those of the control group [73 (33,80), 83 (67,100)] ( Z values were 2.862, 2.448, P<0.05). The fatigue, pain, dyspnea, insomnia and loss of appetite scores was significantly lower (P<0.05). Conclusion:Clinical nursing procedure of pulmonary rehabilitation for patients undergoing thoracoscopic surgery of lung cancer can promote and conducive the cardiopulmonary endurance, reduce the risk of postoperative pulmonary complications, increase the level of physical and cognitive function, improve postoperative symptoms and the quality of life.
