1.Cardiac tamponade: a rare but preventable complication of central venous catheter in neonates

Pericardial effusion with cardiac tamponade is a rare and
life-threatening complication of peripherally inserted central
catheter (PICC) in a neonate. We report a 33-week preterm
neonate who had sudden clinical deterioration at day seven
of total parenteral nutrition regime via PICC. Recognition of
pericardial effusion with cardiac tamponade in neonates
with a PICC requires a high index of suspicion and steps in
prevention include proper catheter tip placement and
continuous monitoring of line position and function.

2.Effects of housing conditions on stress, depressive like behavior and sensory‑motor performances of C57BL/6 mice

Background:
The effects of housing conditions on animal physiology, behavior or stress are still debated. The aim of this study was to investigate the effects of three different housing systems, individually ventilated cages (IVC), classical small cages with floor surface area of 500 cm2 (CC500) and classical large cages with floor surface area of 800 cm2 (CC800) on body weight, sensory-motor performances, depression-like behavior, plasma corticosterone and brain oxidative stress parameters in C57BL/6 mice. The mice housed in one of the cages from birth to 6 months of age. Hang wire and adhesive removal tests were performed to evaluate somatosensory and motor performances. The extent of depression was determined by the forced swim test. Blood corticosterone levels were measured. In addition, brain malondialdehyde (MDA), total antioxidant status (TAS) and total oxidant status (TOS) levels were analyzed.
Results:
The depression-like behavior of the groups was similar. Although there were no significant differences in hang wire test among groups, CC500 group required longer durations in adhesive removal test. The body weight and plasma corticosterone levels of CC800 group were significantly higher than other groups. The oxidative stress parameters were highest in CC500 cage.
Conclusions
Our study showed that the least stressful housing condition was IVC cage systems. Interestingly, the number of mice in the classical cages had a significant effect on stress levels and sensory-motor performance.
3.Stature Approximation of Malays, Chinese and Indian in Malaysia Using Radiographs of Femur, Tibia and Fibula

The study was conducted to create specific formulae for stature estimation of adult male population in Malaysia based on measurements of femur, tibia and fibula lengths using radiographic method. A number of thirty two Malaysians from hospitals involved as subjects in this study. The subjects were Malays (14), Chinese (8) and Indians (10) from 25-45 years old. The standing height of the subject was recorded before femur, tibia and fibula were scanned with an x-ray machine. The bones length was measured on the x-ray film in centimetres (cm) and analysed using Statistical Package for Social Sciences (SPSS) version 19.0 for Windows. The results showed a significant relationship (p < 0.05) between
stature and measurements of length of femur, tibia and fibula. Stature of Malays and Indians were significant (p < 0.05)with measurements of length of femur, tibia and fibula but insignificant in Chinese. Simple Linear Regression Analysis was used to derive regression equation for single bone and Multiple Linear Regression Analysis was used to derive regression equation for combination of femur, tibia and fibula. Six formulae for stature estimation of adult male population in Malaysia were derived. The formulae consist of three formulae for single bone, a formulae for combination bones with unknown race, a formulae for combination bones of Malays population and a formulae for combination bones of Indians population. Formulae for combination bones had the highest correlation coefficient compared to the formula using a single bone. Standard error was found to be high in all the formulae due to small sample size. Extension for this study is essential to provide Malaysia with accurate formulae.
4.Increasing Source-Object Distance: A Computed Radiographybased Strategy to Reducing Radiation Dose in Occipital-Frontal Skull X-Ray.

Introduction: Exploring potential optimization strategies and developing evident practices is critical. Previous studies
show that radiation dose can be reduced by increasing the source-image distance (SID). Although most studies use
digital radiography, many hospitals in underdeveloped countries still use computed radiography (CR). Therefore,
research will investigate the relationship between SID and Entrance surface dose (ESD) using the CR. Methods: This
study involved the measurement of radiation dose and image quality of a radiological procedure performed at a reference SID; 100cm and the tested SIDs; 110cm, 120cm, and 130cm, using constant technical factors (70kVp, 25mAs,
grid). A LiF; Mg Ti thermoluminescence dosimeter (TLD-100) chip was placed in the center of the radiation field of
the OF10° skull radiography examination to measure ESD. Image quality was assessed using the European Commission guidelines and graded using relative visual assessment analysis (VGA). Results: Significant ESD reduction from
21% and 45% when SID was increased from 100cm to 130cm (p <0.001), where SID was negatively correlated with
ESD (r= - 0.98). The VGA scores showed no statistical difference in the image quality of the OF10° skull radiography
examination for the tested and reference images (p=0.21). VGA scores for 120cm images showed the highest image
quality among the SIDs tested with a dose reduction of 37%. Conclusion: ESD was statically reduced when SID was
increased from 100cm to 130cm, while image quality was diagnostically acceptable. The study suggests that 120cm
is the optimal SID when both dose and image quality are considered.
5.Classifying Sources of Nitrate Contamination in an Alluvial Deposit Aquifer System Using Hydrogeochemical Properties and Multivariate Statistical Techniques

Introduction: This study determined nitrate concentration and identify the classifying sources of nitrate pollution in the alluvial deposit aquifer system in Bachok, Kelantan. Materials and Methods: A total of 300 groundwater samples were collected in two different areas; agricultural area (150 samples) and non-agricultural area (150 samples). The samples were analyzed for nitrate and other parameters such as pH, EC, NH4+, TDS, turbidity and salinity. The multivariate analyses were used to identify factors that govern the groundwater quality and potential nitrate sources in the study area. Results: Samples in the agricultural area were slightly acidic (5.89 ± 0.67), contained high nitrate (15.10 ± 15.90 mg/L NO3-N), NH4+ (0.82 ± 1.24 mg/L) and turbidity (3.25 ± 2.78 NTU). The principal component analysis (PCA) have identified the groundwater quality in the study area was influenced by the natural processes and anthropogenic activities. Based on the hierarchal cluster analysis (HCA), Cluster II in the agricultural area was identified to be most heavily nitrate contamination, while Cluster III in the non-agricultural area was identified to be strongly affected by seawater intrusion. Conclusion: The findings of this study are useful for developing protection alternatives of private well waters to prevent further deterioration of groundwater quality by nitrate such as control of nitrogen fertilizer use, manure applications and other agricultural practices in the agricultural area. SUMMARY
Introduction: Urinary tract infection (UTI) is a common
bacterial infection affecting children and therefore, prompt
recognition and accurate antimicrobial management are
vital to prevent kidney damage. This study aims to
determine the bacterial pathogens and their patterns of
antimicrobial resistance in children presenting with UTI.
Methods: A retrospective study of 721 cases, involving
children between the ages of 1-day old to 13 years old with
culture-proven UTI in Selayang Hospital, Malaysia between
January 2007 and December 2011. The bacterial pathogens
and antibiotic resistance patterns in the total population,
prophylaxis and no prophylaxis groups were studied.
Results: The 3 most common organisms isolated in the total
population were E.Coli (41.6%), Klebsiella spp. (21.2%) and
Enterococcus spp. (11.0%). With regards to the antibiotic
resistance, E.Coli resistance rates to ampicillin, cefuroxime
and gentamicin were 67.7%, 15.3% and 7.3% respectively.
Ampicillin-resistance was also highest in Klebsiella spp.
(84.3%), Enterococcus spp. (15.5%) and Proteus spp.
(55.5%).
Conclusion: E.coli remains to be the leading bacterial
pathogen causing UTI in children, with ampicillin-resistance
occurring in more than half of these cases. Therefore,
accurate choice of antibiotics is important to ensure optimal
outcome. In our study, cefuroxime and gentamicin have
lower antibiotic resistance rates and can be used in the
treatment of UTI in children.

8.The Association of Reported Respiratory Symptoms among Children in Malaysia with Particulate Matter Exposure in Municipal Solid Waste Landfills

Background: Prolonged exposure of heavy metals in the respirable particulate matter (PM10) from municipal solid waste (MSW) landfills may affect children health. Objective: To investigate the association of reported respiratory symptom with heavy metals in PM10, with heavy metals in fingernails among children residing close to MSW landfills. Methods: Two groups of children age 7 to 12 years old were involved in this cross-sectional study. Those residing within 3 km radius from a landfill were the exposed group and those residing more than 3 km radius as the unexposed group. Questionnaires adapted from American Thoracic Society were applied in the survey. Fingernails were used as biomarker. Ten heavy metals elements in PM and fingernail samples were analysed using inductively coupled plasma mass spectrometry (ICP-MS). Results: The cadmium, chromium, copper, manganese, nickel and lead concentrations in PM around the MSW landfills and residential areas exceeded the Canada and USEPA standard permissible limit. Heavy metals in fingernails (p<0.001) of exposed group were significantly higher than the unexposed group. Children with no pets have less reported respiratory symptoms. Elevated level of heavy metals in PM and fingernails were associated with high risk of reported respiratory symptoms. Conclusion: Heavy metals in PM10 and fingernails were associated with potential risk factor of respiratory health in children.
9.Cadmium (Cd) Exposure among Waste Collector in Urban Area, Malaysia

Introduction: Cadmium (Cd) in urine and inhaled dust of the municipal waste operators was assessed. Methods: Urine spot samples were collected and analysed for Cd and creatinine of 60 municipal waste operators between April to June 2013. Respirable dust was collected using personal air sampling pump GilAir-3 and GilAir-5 for 8 working hours. Cd in urine and dust were analysed using the Flame Atomic Absorption Spectrometer (Perkin Elmer A Analyst 800) while urinary creatinine was measured using Reflotron® Plus creatinine. Results: The mean and standard deviation (SD) of Cd in the respirable dust (0.59 ± 50.27 μg/m3) was within the permissible exposure limit (PEL). The level of Cd in urine (0.015 ± 0.0097 μg/g Cr) was lower than the safe limit of 5 μg/g. The creatinine level (173.59 ± 50.27 mg/dl) was within the normal range (20 to 350 mg/dl). The multiple regression model shows smoking and years of smoking were the significant predictors for the Cd in the urine (R² = 0.216 F(3,56) = 5.150, p < 0.05). Conclusion: Municipal waste operators were exposed to minimal Cd exposure while handling waste and the accumulation of this metal urine was correlated with smoking habit.
10.Do Our Diabetic Patients Ready for Safe Pregnancy? Attitude Towards Preconception Care and Its Associated Factors Among Women With Diabetes Attending Government Health Clinics in Terengganu

Introduction: Women with uncontrolled diabetes may conceive but will result in poor pregnancy outcomes. This
study aimed to assess the level of knowledge and attitude and determine factors associated with attitude towards
preconception care. Method: A cross-sectional study was conducted in various government health clinics from all
districts in Terengganu by using multistage random sampling. A self-administered questionnaire was applied to assess
the level of knowledge and attitude towards preconception care. Multiple logistic regressions were applied to determine factors associated with attitude towards preconception care. Results: A total of 422 respondents were involved.
The prevalence of respondents with unsatisfactory knowledge and attitude were 41.7% and 84.1%, respectively.
Knowledge was not significantly associated with attitude. Respondents who used contraception had a better attitude
(Adj. OR: 0.44, 95% CI:0.23,0.84). Low household income was associated (Adj. OR: 2.40, 95% CI:1.10,5.26) with
an unsatisfactory attitude towards preconception care. Conclusion: Attitude towards preconception care amongst diabetic women who were attending primary care clinics was unsatisfactory. A structured and focused preconception
educational programme care is necessary, especially amongst low-income women.
