1.Fully connecting the Observational Health Data Science and Informatics (OHDSI) initiative with the world of linked open data

The usage of controlled biomedical vocabularies is the cornerstone that enables seamless interoperability when using a common data model across multiple data sites. The Observational Health Data Science and Informatics (OHDSI) initiative combines over 100 controlled vocabularies into its own. However, the OHDSI vocabulary is limited in the sense that it combines multiple terminologies and does not provide a direct way to link them outside of their own self-contained scope. This issue makes the tasks of enriching feature sets by using external resources extremely difficult. In order to address these shortcomings, we have created a linked data version of the OHDSI vocabulary, connecting it with already established linked resources like bioportal, bio2rdf, etc. with the ultimate purpose of enabling the interoperability of resources previously foreign to the OHDSI universe.

2.The Study on Subject Words of Korean Medical Informatics by Expanded MeSH: Based on Journal of Korean Society of Medical Informatics.

In order to maintain a uniformity and consistency in terminology for constructing and searching a literature database, controlled vocabularies should be used as key words in the journal. While most of medical academic societies have recommended MeSH be used as key words, only 23 societies published terminology or index books. We reviewed 172 journals of JKOSMI published during the period from 1995 to 2000, using MeSH brower. Only 11.72% of key words were completely consistent with MeSH terminolgy and 25.56% were partially consistent. Purposes of this study are to examine a current status of using MeSH in the Journal of Korean Society of Medical Informatics(JKOSMI) and to develop a Korean terminology book on medical informatics.

4.Formative Evaluation of Standard Terminology-based Electronic Nursing Record System in Clinical Setting.

The Objectives of this study are to evaluate the user satisfaction and actual data input time through an enterprise ICNP-based electronic nursing record system using the controlled vocabulary in a secondary care hospital (BSNUH). Study design is a formative evaluation using the QUIS (Questionnaire user interaction satisfaction) self-reported in a secondary care hospital operating EMR(electronic medical record) system in Korea. Participants were two hundred fifty nurses in BSNUH. All of them were registered nurses participated in day-to-day nursing care during study periods. Participants were asked to fill in the SNCEQ(The Staggers Nursing Computer Experience Questionnaire) and QUIS(Questionnaire for User Interaction Satisfaction). Also requested were the data input time(sec) required for entry of approximately 20 times of nursing documentation tasks and the number of standardized precoordinated phrases used for documenting routine nursing records. The mean score of user satisfaction was 4.56 (SD 1.25) and the mean time of data input and the average number of precoordinated phrases used in nurses notes was 2.25min and 3.7 respectively.

5.Effective Query Expansion using Condensed UMLS Metathesaurus for Medical Information Retrieval.

Medical vocabularies in medical records are used in several synonyms and various expressions even though they are same concepts. Query expansion using a thesaurus enhances recall of medical information retrieval (IR) system for searching patient records or literatures. This study proposed IR system architecture applied the Metathesaurus of Unified Medical Language System (UMLS). To enhance the retrieval effectiveness at the same time to reduce retrieval time, we reconstructed condensed Metathesaurus (CMT), which is constituted of frequently used terms in medical records. We used 40,000 radiology reports of Brain CT/MRI at Seoul National University Hospital. The retrieval model we used is the Boolean methods. The results showed 15~27% effectiveness for searching relevant documents implementing the UMLS MT into IR system for query expansion. But it took 3.5 times longer for retrieval compared with non-MT implemented IR system. When we applied the CMT into IR system, however, the retrieval time reduced by 50% and the retrieval performance decreased only 8.7% compared with all MT implemented IR system. In this paper, we developed the medical document retrieval system applied UMLS MT for query expansion methods that can improve the relevant document retrieval performance, at the same time it can reduce the retrieval time through consisting condensed Metathesaurus for a specific domain.

6.Open versus closed reduction of mandibular condyle fractures: A systematic review of comparative studies.

Objective: The objective of this review was to provide reliable comparative results regarding the effectiveness of any interventions either open or closed that can be used in the management of fractured mandibular condyle PATIENTS AND METHODS: Research of studies from MEDLINE and Cochrane since 1990 was done. Controlled vocabulary terms were used. MeSH Terms were "Mandibular condyle" AND "Fractures, bone". Only comparative study were considered in this review using the "limit" function. According to the criteria, two review authors independently assessed the abstracts of studies resulting from the searches. The studies were divided according to some criteria, and following were measured: Ramus height, condyle sagittal displacement, condyle Towns's image displacement, Maximum open length, Protrusion & Lateral excursion, TMJ pain, Malocclusion, and TMJ disorder. RESULTS: Many studies were analyzed to review the post-operative result of the two methods of treatment. Ramus height decreased more in when treated by closed reduction as opposed to open reduction. Sagittal condyle displacement was shown to be greater in closed reduction. Condyle Town's image condyle displacement had greater values in closed reduction. Maximum open length showed lower values in closed reduction. In protrusive and lateral movement, closed reduction was less than ORIF. Closed reduction showed greater occurrence of malocclusion than ORIF. However, post-operative pain and discomfort was greater in ORIF. CONCLUSION: In almost all categories, ORIF showed better results than CRIF. However, the use of the open reduction method should be considered due to the potential surgical morbidity and increased hospitalization time and cost. To these days, Endoscopic surgical techniques for ORIF (EORIF) are now in their infancy with the specific aims of eliminating concern for damage to the facial nerve and of reducing or eliminating facial scars. Before performing any types of treatment, patients must be understood of both of the treatment methods, and the best treatment method should be taken on permission.

7.A clinical research strategy using longitudinal observational data in the post-electronic health records era.

Adoption of electronic health records (EHRs) is increasing worldwide. The worldwide EHR adoption rate is estimated to be around 9% to 12%. Thus, the accumulation of medical records in electronic form is also sharply increasing and is expected to be a precious asset for clinical research. Longitudinal observational studies based on EHRs are also increasing. Observational studies covering more than a million people are not rare at present. However, much of the current EHR data are equivalent in form to those of paper records, but are just stored in electronic stor-age devices, rather than as electronic data that can be transferred and shared without loss of clinical semantics. Current EHR systems must be improved in many ways to be used for anal-yses to yield important clinical knowledge. These improvements, which are addressed in this review, include the adoption of clinical data warehouses, use of controlled vocabulary, avoidance of personal/departmental research databases, a standardized interface of many diagnostic devices with the EHR system, control of time-stamp granularity, preparedness for whole-genome sequencing of every patient, confederation or consolidation of multi-institutional EHR data, protection of privacy and confidentiality, and an education system for clinical informaticians.

8.Establishment of database for food classification and coding in Chinese dietary exposure assessment.

OBJECTIVETo establish the basis for Chinese dietary exposure assessment database by classifying and coding the data from the national dietary survey and pollutant surveillance.
METHODSThe method, which combined CODEX food classifying and coding of Codex Alimentarius Commission (CAC) with Chinese food classification of food composition table, was applied to classify and code the data of 1 810 703 Chinese dietary consumption and 487 819 pollutant surveillance. The coding system was according to the first two letters of the respective food group that represent the type or source of foods, the last four digits represent the serial number of the food in the CAC food classification. If the foods can be found in CAC food code system, its original food code is used. The new codes corresponding with the foods which are not exist in CAC food code system, is added according to CAC coding methods.
RESULTSDietary consumption data were divided into 6 major categories, 19 types, 75 groups, the agricultural products of pollutant surveillance corresponding to 499 codes. Comparing with CAC food coding system, Chinese dietary consumption data have added F (candy snacks) and G (beverages) 2 major categories, 4 types, 33 groups, 302 new codes. The additional groups most were the processing food groups with Chinese characteristics, such as canned, beverages, candy, meat products.
CONCLUSIONThe foundation of data communication to dietary exposure assessment has been established, and the connection of Chinese food classifying and coding with CAC data have been achieved.

9.Structuralization of digestive endoscopy report based on NLP.

This paper presents a method based on NLP to realize structuralization of digestive endoscopy reports. The method is taking advantage of existing NLP's processing technologies and introducing minimal standard terminology (MST) to transform a narrative gastroscopy report into the structuralization report based on MST, whose accuracy rate is 92.3%.

10.Use of Medical Subject Headings (MeSH) in the Journal of the Korean Orthopaedic Association.

PURPOSE: The purpose of this study was to analyze the equality between author key words used in the Journal of the Korean Orthopaedic Association and controlled vocabulary or medical subject headings (MeSH). MATERIALS AND METHODS: A total of 1,058 English key words in 320 papers (average 3.3 words in a paper) from 2009 to 2012 were eligible for this study. We classified them according to matched, partially matched, and non-matched terms. The partially matched terms were further dissected into entry terms, qualifiers, anteriorly or posteriorly matched, abbreviations, and pleurals. After descriptive analysis, we assayed patterns of errors in using MeSH, and reviewed frequently used non-MeSH terms. RESULTS: The rate of matched terms was 23.5% for an average of four years, and 34.8% for 2013, which is on the rise by year. The rate of partially matched terms was 34.8%, and that of non-matched terms was 41.7% for an average of four years. The most frequently used key words were Knee and Total knee arthroplasty (17 times), followed by Osteoarthritis (9), Femur, Hip, and Total hip arthroplasty (8). CONCLUSION: Use of proper keywords aligned with the international standards such as MeSH is important to be properly cited. The authors should pay attention and be educated on correct use of MeSH as key words.
