3.Treament of lower ureteric stricture due to tuberculosis by extravesical reimplantation

Studying of 15 patients with tuberculous ureteric stricture treated by extraversical ureteral reimplantation (4 males and 11 females, 10 cases of unilateral ureteric stricture and 5 cases of bilateral ureteric stricture). The mean operation time: 101.6 minutes, mean estimated blood loss: 68.6ml, mean time of removal of urethral catheter: 7 days, mean postoperative hospitalization: 8.13 days. Postoperative complications were minimum: 1 case of mild infection because oh ureteral stent migration, 1 case of moderate intraversical bleeding... Long-term outcomes of operation were satisfactory: on 12 patients performed follow-up UIV, 8 with good amelioration and 3 with pretty good amelioration, 1 without amelioration, mean follow-up duration: 15.5 months

Histamine type 2 (H2) receptor antagonists are widely used for stress ulcer prophylaxis in critical and postoperative care. Though ranitidine is one of the most commonly used H2 receptor antagonists, with a low incidence of adverse reactions, a few anaphylactic reactions associated with ranitidine have been reported. This report describes 2 additional cases of anaphylaxis induced by ranitidine used for stress ulcer prophylaxis.

7.Factors Influencing Performance of Delirium Care for Postoperative Delirium of Elderly Patients among Recovery Room Nurses.

PURPOSE: The purpose of this study was to identify factors associated with performance of nursing care for postoperative delirium of elderly patients among recovery room nurses. METHODS: The research was a cross-sectional, descriptive design using questionnaires. The participants were 99 nurses from five urban recovery rooms. Data were analyzed using descriptive statistics, t-test, ANOVA and multiple regression with SPSS WIN 21.0. RESULTS: The performance level of delirium nursing care was low because its mean score was 3.80 out of 5. Performance of nursing care was significantly positively correlated with the importance of nursing care and self-efficacy. Stepwise multiple regression analysis for performance of nursing care revealed that the most powerful predictor was the importance of nursing care. The importance of nursing care and self-efficacy explained 32.3% of the variance. CONCLUSION: The results indicate a need to enhance the performance of nursing care for postoperative delirium of elderly patients among recovery room nurses. The findings also suggest that consideration be given to strategies for improving the importance of nursing care and self-efficacy in developing programs to enhance the performance level of nursing care for elderly patients with postoperative delirium.

8.Intraoperative and Postoperative Glycemic Management in Patients with Diabetes.

Unlike minor operations, major surgeries require strict glycemic control using intravenous insulin infusion in patients with diabetes. The postoperative transition to subcutaneous insulin, if needed, can begin several hours before discontinuing intravenous insulin, by reinitiation of basal insulin re-initiation. Basal-bolus insulin regimens are safer and more effective in hospitalized patients than supplemental-scale regular insulin.

BACKGROUND/AIMS: We aimed to evaluate the cost-effectiveness of preoperative stent insertion for treating left-sided malignant colorectal obstruction. METHODS: Patients with left-sided malignant colorectal obstruction were included in this study. The stent group (ST, n=24) included those patients who were treated with preoperative stent insertion followed by curative surgical resection. The clinical course and management cost of the ST group was compared to those of the emergency operation group (EO, n=22). RESULTS: The patients' age (60.6+/-3.1 yrs vs. 62.1+/-3.2 yrs, p=0.74) and the male to female ratio (12:12 vs. 15:7, p=0.25) were not different between the ST and EO groups. The distribution of the postoperative pathologic stages was also not different. All the patients in the ST group underwent only one surgical operation, while 6 patients (27.3%) in the EO group underwent 2 or more surgeries (p<0.01). The mean hospital stay in the ST group was 22.0+/-0.8 days compared to 26.3+/-2.4 days in the EO group (p=0.09). Postoperative care in the intensive care unit was necessary for one patient (4.2%) in the ST group, while 7 patients (31.8%) in EO group needed postoperative care (p=0.02). Postoperative complications developed in one patient in the ST group, while 6 patients in the EO experienced postoperative complications (p=0.04). The mean total cost per patient was 7,974,236 won for the ST group while this was 9,271,630 won for the EO group (p=0.06). CONCLUSIONS: Preopreative stent insertion for treating left-sided malignant colorectal obstruction is more cost-effective than an emergency operation.

10.Development of a Nursing Practice Guideline for Pre and Post-Operative Care of Gastric Cancer Patients.

OBJECTIVES: The aim of this study was to develop nursing practice guidelines and algorithms for the pre- and postoperative care of gastric cancer patients. METHODS: The guidelines and algorithms were developed based on a literature review and subject-matter experts' knowledge. The usefulness and applicability of the developed guidelines and algorithms were evaluated by the expert group. RESULTS: The guidelines comprised 64 recommendations and algorithms for admission care, preoperative care on the day before the operation, preoperative care on the operation day, immediate postoperative assessment, postoperative care, and discharge care of gastric cancer patients. After appropriate evaluation, the experts declared the guidelines to be both useful and applicable. CONCLUSIONS: The guidelines developed in this study can be used for patient care and for training nursing students and novice nurses. In addition, these guidelines can be integrated into the electronic nursing record system for clinical decision support in the future. This will improve the quality of clinical nursing and the expertise of nurses.
