1.Perianal Episiotomy Scar Endometrioma: A Case Report.

Endometrioma is a common clinical condition, but cases in which an episiotomy scaris present are, however, rare: only two cases have been reported in obstetric journals, and one other in a radiologic journal. All three were in English. We encountered a case in which a solitary endometrioma was present in the perineal region beneath an episiotomy scar. An irregularly marginated hypoechoic mass was revealed by US, and a discrete homogeneous enhancing mass by CT.

2.A case of water birth.

Recently, water birth - labor and/or delivery under warm water - is acceptable and natural to many foreign countries as a method of gentle birth. Although some limitations after water birth were reported, a recent international conference explored many of the issuses and indicated the potential scale and the advantages for its inclusion among the options for labor and delivery under water. We experienced a case of water birth who underwent labor and delivery under water. She delivered normal healthy female baby without conventional episiotomy. No obstetrical ans fetal complications were observed during entire procedure of water birth. So, we report this case with brief review of the literatures.

5.Efficacy of cold magnesium sulfate saturated pad in nursing care after episiotomy

Study on 410 pregnant women had babies at Obstetric Department, Sa Dec General Hospital between March and August 2002, who underwent episiotomy or had perineal laceration, followed by swelling, edema, and painful sutures. All participants were divided into 2 groups: group of women had born babies at even dates were treated by 25% magnesium sulfate cold solution, and group of women had born babies at odd dates treated by cold placebo pad. Results: magnesium sulfate solution has endosmotic effect, leads to reduce swelling and edema. Pain relief effects were similar between 2 groups but magnesium sulfate cold solution was likely to have faster improvement in perineal edema and urination. Using cold magnesium sulfate saturated pad for nursing care after episiotomy is simple, easy method and has fast effects.

Endometriosis is defined as the abnormal growth of endometrial tissue outside the uterine cavity, but in rare cases may also occur on the skin. Cutaneous endometriosis frequently develops at the site of a prior abdominal or pelvic procedure, although apparent spontaneous occurrence has been reported. We present a case of non-symptomatic, cutaneous endometriosis in the episiotomy scar. The patient had no evidence of concurrent pelvic endometriosis.

9.Clinical trial of Aromatherapy on Postpartum Mother's Perineal Healing.

PURPOSE: This study was designed to verify the effect of aromatherapy on a postpartum mother's perineal healing. METHOD: The research design was a clinical trial. The methods of aromatherapy were applied sitz bath or soap application using essential oils with Lavender, Myrrh, Neroli, Rose, Grapefruit, Mandarin, Orange, and Roman Chamomile. The subjects of this experiment were postpartum mothers who delivered vaginally with an episiotomy. They were allocated to one of three groups; the aroma-sitz bath group, aroma-soap application group or control group. To evaluate the effect of aromatherapy, the perineal healing status was measured using the REEDA scale and smears of episiotomy wound were obtained. The data were analyzed by repeated measures of ANOVA, ANCOVA, x2-test, and multiple response analysis via SPSS program. RESULT: The REEDA scale was significantly low in the experimental group at postpartum 5th and 7th days (P=.009, P=.003), respectively. Most were observed 'few'(5-10 bacteria per field) bacteria in the smears of episiotomy wound. The one bacteria was identified in the 50.8% of subjects in pretest and two bacteria in the 60.3% in posttest. Most frequently identified bacteria were Escherichia coli and Enterococcus faecalis. CONCLUSION: In conclusion, these findings indicate that postpartum aromatherapy for perineal care could be effective in healing the perineum perineal care could be effective in healing the perineum.

10.A Case of Endometriosis at the Site of Episiotomy Scar.

Endometriosis at the site of episiotomy scar following vaginal delivery is rare condition among the extrapelvic endometriosis implantation. We have experienced a case of episiotomy scar endometriosis. The precise anatomical location of the endometriotic lesion is confirmed using preoperative anal endosonogarphy. We believe anal endosonography to be essential when history, digital examination, and proctoscopy are not conclusive in the differential diagnosis of perianal pain or mass. The treatment of choice is complete surgical excision of the lesion and the surgical excision usually obtain permanent cure. We report a case of endometriosis at the site of episiotomy scar.
