Abstract:
Malignant eccrine poroma is a rare malignant tumor arising from the intraepidermal ductal portion of the eccrine sweat gland, which develops either spontaneously or from a long-standing benign eccrine poroma. It usually affects older people and is located most commonly on the lower extremities. We describe a typical case of malignant eccrine poroma developing from benign eccrine poroma, which had been present for 10 years or more, with subsequent regional lymph node metastasis. A biopsy specimen revealed many well-defined intraepidermal nests of poroma cells with dermal invasion and lymphatic involvement of tumor cells. Immunohistochemical studies showed staining with cocktail cytokeratin, carcino-embryonic antigen, epithelial membrane antigen in intracellular vacuoles of tumor cells and early embryonic stages of eccrine duct formation. Local excision with postoperative adjuvant radiation therapy showed only a temporary response.
