Abstract:
Blowout fracture of the orbit is one of the common sequelae to blunt facial trauma. Although diplopia is a common finding in patients following orbital trauma, quantative measurements of diplopia has not been well studied. We evaluated the extent and duration of improvement in field of binocular single vision(BSV)using quantitative scoring method. We prospectively studied 17 patients who had undergone repair of orbital blowout fracture. The indication for surgery was diplopia. A minimum of 3 months follow-up was available for all the patients included in the study. Binocular single visual field was tested with Goldman arc perimeter pre-and postoperatively. The analysis of the results was performed using model of Sullivan`s funcitional scoring method. We evaluated the recovery time and the factors which affect the process of recovery. BSV was acutely recovered by 1 month postoperatively and then slowly recovered(p=0.003, Wilcoxon signed ranks test). At 2 months after operation, average BSV score was over 80. We evaluated the delayed recovery group, 5 patients. The factors affecting the process of recovery were trap-door fracture type and hypertropia 10 prism diopter or more in primary position(p=0.02, chi-square test). In blowout fracture patients, the BSV scoring method presented improvement of diplopia quantitatively and it helped to assess the state and prognosis of the patients.
