The relationship between coagulation dysfunction and severity and prognosis of acute severe trauma patients

Abstract:
Objective:To analyze the relationship between coagulation dysfunction and severity and prognosis in patients with acute severe trauma.Methods:Seventy-four patients with acute severe trauma treated at our hospital from July 2018 to September 2019 were included. According to the injury severity score (ISS) > 25 points, > 16 points and ≤25 points, and ≤16 points, patients were divided into the extremely critical group, critical group, and non-critical group. And according to the prognosis, the patients were divided into the death and survival groups. All patients had the blood coagulation function test within 24 h of admission. The coagulation function of patients in each group was compared, and the relationship between coagulation dysfunction and the severity and prognosis of patients were retrospectively analyzed.Results:There were significant differences in FIB, PT, D-D, PLT, K value, Ma value, R value and α angle among the extremely critical group, critical group, and non-critical group. FIB, PT, APTT, D-D, PLT and K value of thromboelastogram were negatively correlated with FIB and PLT in the death group and survival group, while FIB, PT, APTT, D-D, PLT and K value of thromboelastogram were positively correlated with PT and DD. Ma value was positively correlated with FIB and PLT, negatively correlated with PT and APTT, R value was positively correlated with PT, APTT, D-D and PLT, and α angle was positively correlated with FIB and PLT, and negatively correlated with PT ( P < 0.05). Conclusions:Coagulation function examination and thromboelastogram can help to determine the severity of the disease and evaluate the prognosis.
