Outcomes of 62 patients with non-small cell lung cancer and brain metastasis undergoing whole brain radiation followed by stereotactic radiotherapy and the prognostic factors

Abstract:
Objective: The purpose of this study is to evaluate the local control rate and the survival of sixty-two patients with non-small cell lung cancer (NSCLC) and one to three lesions of brain metastasis (BM) undergoing whole brain radiation therapy(WBRT) followed by stereotactic radiotherapy, and to explore the prognostic factors. Methods: Sixty-two patients with NSCLC and one to three BM lesions undergoing WBRT followed by stereotactic radiotherapy were eligible for this retrospective analysis. The overall survival was estimated by Kaplan-Meier analysis. The potential prognostic factors related to survival were evaluated. Results: The median follow-up time was 12 months. For all patients, the median survival time from the start of WBRT was 16 months (95% confidence interval: 11.27-20.73). Multivariate analysis showed that the age, histology, lesion location, radiation dose and the molecular targeted therapy were independent predictors of survival. The median time of local tumor control was 20 months (95% confidence interval: 18.21-22.45). The six-month, one-year and two-year local control rates were 96.6%, 82.5% and 48.9%, respectively. No severe adverse events and toxicity were observed. Health-related quality of life (HRQOL) was improved one month after radiotherapy (P<0.05). Conclusion: The age, histology, lesion location, radiation dose and the molecular targeted therapy were independent predictive factors of survival. For NSCLC patients with one to three lesions of BM, WBRT followed by stereotactic radiotherapy is safety and effective.
