Analysis of factors affecting on cognitive function in children with diabetic ketoacidosis complicated with cerebral edema

Abstract:
Objective To analyze the effect of diabetic ketoacidosis (DKA ) complicated with cerebral edema on the cognitive function in children,and to analyze the risk factors of cerebral edema .Methods The clinical data of 120 children with DKA who were treated in our hospital from Feb .2014 to Feb .2016 were retrospectively analyzed .Subjects were divided into brain edema group (BE group,n=35) and simple DKA group (n= 85 ) according to whether or not cerebral edema was complicated .The differences of cognitive function between the two groups were observed and the risk factors of cerebral edema in children were analyzed .Results Cerebral edema in children with diabetic ketoacidosis the scores of verbal IQ (t=11.967,P<0.01),performance IQ (t=4.857,P<0.01) and total IQ (t=5.537, P<0.01) of BE group were lower than those of DKA group (P<0.01) .Compared with DKA group,the levels of blood glucose (t=5.898,P<0.01),PCO2(t=21.369,P<0.01),HCO -3(t=9.511,P<0.01), urea nitrogen(t=13.297,P<0.01) and serum creatinine (t=8.985,P<0.01) were lower in BE group;but the pH (t= 21.369,P< 0.01) and serum sodium (t= 5.090,P< 0.01) were higher .Multivariate Logistic regression analysis showed that pH (OR= 4.356,P= 0.012),urea nitrogen(OR= 5.238,P<0.01) and serum sodium(OR=4.415,P=0.003) were risk factors of cerebral edema in DKA children .Conclusion The cognitive function of children with DKA complicated by cerebral edema is poor .The blood pH,serum sodium and BUN are risk factors for cerebral edema .
