- Abstract: Singapore, like her ASEAN counterparts, faces a human and economic burden of chronic diseases such as diabetes. The Singapore diabetes care model features a mixed financing and mixed care delivery model, with an increasing focus on integration of care across traditional boundaries, through information technology, clinical engagement, robust clinical governance and financing schemes, and patient education and self-management.
