Abstract:
Objective To describe special clinical features of the complex fractures in proxitnal humeral in adolescents, and the role of open reduction in management. Methods 14 cases of complex displaced fractures in upper humerus were openly reduced and internally fixed with T-shaped plate,L-shaped plate, modified cloverleaf plate or trapezoid-shaped plate. The lacerated rolator cuffs and capsules were repaired at the same time. Results 13 cases were followed up from 3 months to 8 years, with the average period being 2 years and 10 months.According to the INcer classification and functional evaluation of shoulder and elbow,5 cases were rated as excellent,7 good and 2 fair. Conclusion Since complex and displaced fractures of the proximal bumerua in adolescents often result in the damage to the vascular supply of the head of the humerus,in choosing operative procedures orthopedists surgeons should consider minimum exposure, limited and appropriate internal fixation rather limn rigid fixation, with an objective to preserve vascularity to the articular segment.
