Radical cystectomy with partial prostate sparing for bladder cancer

Abstract:
Objective:To investigate clinic values of partial prostate sparing surgery in total cystectomy. Methods: 29 cases, age from 42~70 years old,(mean 61).Standard for being chosen:1.Distance was below 0.5cm between the carcinoma and bladder-neck.2. Biopsy negective at bladder-neck and urethral mucosa at random.3. PSA is below 4 ng/L.4.the surface of prostate was smooth and no scleroma by rectal examination;B-ultrasound had no unusnal echo in prostate before operation. All of the cases were bladder transitional cell cancer by Pathological examination. Invasive cancinoma 16 cases, superficial recurrence cancinoma 13 cases. Results: During the follow-up of 6 to 60 months(mean,38months). At 6 months follow-up,max flow rate of all the cases were beyond 20 ml/ s,residual volume above 100ml in 4 cases(4/29). All of the cases were fully continent during the day, and 27(93%) at night.16 cases had potency before operation, only 3 cases lost potency after operation;those 13 cases can have sex but no feel of ejaculation. Conclusion:Radical cystectomy with partial prostate sparing for bladder cancer is a relatively safe and reliable surgical procedure. It can improve the continence and potency without compromising the principle of tumor control.
