Operation modes for proximal femur tumor with pathological fracture

Abstract:
[Objective]To explore the operation modes and outcomes of proximal femur tumor with pathological fracture. [Methods]There were 46 cases to be reviewed from october 2001 to June 2007,all were respectively treated by 6 surgery methods:curettage and bone grafts after fracture healing by external fixation in 7 cases (Group A),bone transplantation and internal fixation in 11 cases (Group B),fibula grafting with blood vessel after curettage in 6 cases (Group C),bone cement plugging and interlocking intramedullary nailing after curettage in 8 cases (Group D),tumor segment resection and endoprosthesis in 12 cases (Group E),and hip joints disarticulation in 2 cases (Group F).[Results]According to Enneking evaluation standard in limbs function,the excellent to good rate was 85.7% in Group A,88.8% in Group B with 9 cases followed up,83.3% in Group C,66.6% in Group D with 5 patients died,75% in Group E with 5 patients died and 1 case recurrence.[Conclusion]Proper operation methods of the proximal femur depend on the basis of tumor characteristics,the degree of damage and fracture displacement. Benign tumor could be treated by tumor curettage and bone grafts,femoral neck fracture by rigid fibula graft,malignant tumor by prosthesis replacement or interlocking intramedullary nails.
