Measurement of the Interventricular Presssure Gradient in Patients with Ventricular Septal Defect—A Simultaneous Doppler and Dual Catheter Study

Abstract:
To assess the fesibility and reliability of Doppler echocardigraphy in measuring the interventricular pressure gradient in patients with ventricular septal defect (VSD), continuous—wave Doppler echocardiography and cardiac catheterization were performed simultaneously in 58 cases with VSD. The shunt velocities were recorded by Doppler and converted into the peak instantaneous pressure gradient (△P_(P-D)), the mean pressure gradient (△P_(m-D)) and the mid—systolic pressure gradient (△p_(ms-D)) using the simplified Bernoulli equation. Both left and right ventricular pressures were measured using a dual catheter technique and the peak instantaneous pressure gradient (△p_(p-c)) and the peak—to—peak pressure gradient (△P_(pp-c)) measured. The results showed that: 1. there was a high correlation between △P_(p-D) and △P_(p-c) (r=0.98), and there was no significant difference between the two measurements; 2. both △P_(p-D) and △P_(m-D) correlated highly with △P_(pp-c) (r=0.98 and r=0.96, respectively), but △P_(p-D) significantly ovestimated(P
