Study on the etiology and hemorrhage-inducing factors of acute upper gastrointestinal hemorrhage in elderly patients

Abstract:
Objective To explore the etiology and hemorrhage-inducing factors for acute upper gastrointestinal hemorrhage (AUGH) in the elderly.Methods A total of 370 cases diagnosed as acute upper gastrointestinal hemorrhage were collected in our hospital from January.2012 to May 2015.They were divided into two groups according to age.The etiology and hemorrhage-inducing factors for upper gastrointestinal hemorrhage were retrospectively analyzed and compared between the two groups.Results The incidences of gastric ulcer and gastrointestinal tumors-caused AUGH were higher in elderly group than in non-elderly group (36.5％ vs.12.3％,16.4％ vs.2.4％,x2 =30.149 and 23.093,both P＜0.01).The incidences of acute gastric mucosal lesion-,duodenal ulcer-,gastric fundi and esophageal varices-caused AUGH were lower in elderly group than in non-elderly group(8.8％ vs.20.4％,25.2％ vs.38.9％,1.9％ vs.8.5％,x2 =9.320,7.706,7.534,respectively,all P＜0.01).The incidences of excessive medication administration-and unhealthy mental state-induced AUGH were higher in elderly group than in non-elderly group (18.2％ vs.2.8％,17.0％ vs.2.4％,x2=25.092,24.502,both P＜0.01).The incidences of irregular diet-,alcohol-drinking-,and overwork-induced AUGH were lower in elderly group than in non-elderly group (1.3％ vs.17.5％,2.5％ vs.13.7％,2.5％ vs.7.1％,x2=25.478,14.070,3.927,respectively,P＜ 0.01 or 0.05).Conclusions There are many causes for acute upper gastrointestinal hemorrhage,and gastric ulcer and gastrointestinal tumors are the common causes in the elderly.Excessive medication administration and unhealthy mental state are the main inducing factors for acute upper gastrointestinal hemorrhage in the elderly,and the prevention should be strengthened.
