Combination Drug Therapy of Oxybutynin and Desmopressin in Children with Primary Nocturnal Enuresis.

Abstract:
PURPOSE: Because enuresis is a symptom rather than a disease, it is likely that no single hypothesis will be able to explain all cases and that, in individual patients, multiple factors may be operative. Despite various treatment modalities, pharmacotherapy still appears to be the common choice. The aim of this prospective study was to evaluate the efficacy of combination drug therapy (oxybutynin plus desmopressin) for primary nocturnal enuresis. MATERIALS AND METHODS: This prospective study was done with 189 primary nocturnal enuretics (M:F=132:57, age: 8.0+/-2.5 years) who were randomized to one of three groups: imipramine monotherapy (I: 43 cases), desmopressin monotherapy (D: 62 cases) and combination therapy of desmopressin with oxybutynin (C: 84 cases). The efficacy of the drugs was measured by the reduction of the number of wet nights per week. The outcomes were classified as excellent (0 to 1 wet night per week), good (over 50% reduction of wet night), or poor responder (less than 50% reduction of wet night). RESULTS: There was a statistically significant difference in response rates between combination therapy (86.9%) and monotherapy (I: 58.1%, D: 61.3%). Also, combination therapy(87.1%) was more effective than monotherapy (I: 50.0%, D: 33.3%) in polysymptomatic enuretic patients. There were no serious side effects in any of the three groups. CONCLUSIONS: Combination drug therapy of desmopressin and oxybutynin is a safe and suitable choice in the initial treatment of primary nocturnal enuresis, especially in a polysymptomatic group.
