Abstract:
Objective To promote chronic care in Mainland China by the inspiration from Hong Kong chronic disease care integrated management model. Methods The research was mainly based on comparative study as well as literature research to understand Hong Kong chronic disease care integrated management model. Results The Hong Kong chronic care integrated management model can improve quality of life of patients and help their families step into a more harmonious society. Conclusions Hong Kong chronic care integrated management model is helpful for promoting Mainland China chronic care.
