Successful Treatment of Trichophyton Rubrum Corneal Ulcer Following Posterior Chamber Lens Implantation.

Abstract:
A 55 year old female was admitted for cataract extraction in her right eye. On Jan. 12, 1986 she underwent an uncomplicated ECCE with insertion of Sinskey PC lens. On the 9th. PO day, slight thinning on the superior limbal area adhering with grayishwhite discharges and mild degrees of flares in the anterior chamber were found. On the next day the amout of hypopyon and thinning of wound margin were aggravated, so debridement of wound margin and resuturing of C/S wound with conjunctival flap covering were performed. The symptoms and signs of corneal ulcer worsened in spite of the massive antibiotic and corticosteroid therapy. On the 27th, post-reoperation day, Trichophyton rubrum was found on the culture specimen. After then antimycotic treatment with subconjuctival injection of Amphotericin-B, 5% Natamycin(pimaricin) eye drop, 1% Flucytosine eye drop and oral Nizoral(Ketaconazol) were started. The rethickening and slight healing signs of the corneal ulcer was noticed from 7 days after antimycotic treatment and complete healing with thick pannus formation was noticed 60 days after reoperation. On Jully 14 and July 21, 1986 Nd: YAG Laser iridotomy performed to break the inflammatory membranes. The final visual acuity is 20/(20/40 X Cyl. + 3.00D. A30 degrees) in the right eye.
