Abstract:
We report the successful surgical treatment of three patients with a giant aneurysm of the internal carotid artery by resection of the aneurysm and end to end anastomosis. All three patients were female, whose ages ranged from 49 to 72 years old. The aneurysm was saccular type and the aneurysmal size was 5.0 cm to 6.0 cm in long diameter. Diagnosis was done by carotid angiogram in two patients, in one patien, MRI was used. In the general anesthesia, we used EEG monitoring for brain ischemia and distal carotid artery back flow pressure. Shunt was not used during the operation because the distal carotid back flow pressure was more than 40 mmHg. Patients were recovered without any neurological sequelae.
