Application of a Transition Zone Index in Patients of Small Volume Hyperplasia of the Prostate Gland.

Abstract:
PURPOSE: The aim of this study was to define the usefulness of a transition zone index in the treatment strategy of benign prostatic hyperplasia with a poor response to medical therapy. MATERIALS AND METHODS: TUR-P was performed on 50 patients whose symptoms were not improved by medical treatment. The total prostate volume and transition zone index were measured with by TRUS. The IPSS and peak flow rate were measured before and after the TUR-P. RESULTS: The mean prostate volume was 25.6+/-4.2gm and the mean transition zone index was 0.58+/-0.07. There were weak correlations between the prostate volume, IPSS and peak flow rate, but without statistical significance. However, the transition zone index had a strong correlation with the IPSS(r=0.512, p<0.001) and peak flow rate(r=-0.452, p=0.001). Significant correlations were observed between the transition zone index and change in the IPSS(r=0.496, p=0.001), and improvement of the peak flow rate(r=0.661, p=0.001) after the operation. Complications were observed in 2 patients; 1 case of epididymitis and another of bladder neck contracture. CONCLUSIONS: A transition zone index can be a useful parameter in the treatment strategy of benign prostatic hyperplasia with a poor response to medical therapy. Therefore, good results might be achieved via an early TUR-P, especially in small volume benign prostatic hyperplasia with a high transition zone index.
