Abstract:
The implementation of discharge preparation services can ensure the continuity of care for patients in different places of residence, shorten the length of hospital stay, reduce the rate of patient readmission, and increase the satisfaction of patients and their families. This article reviews the concept, implementation principles, service content, implementation process, domestic and foreign application status and implementation effects of discharge preparation services in aged patients, aiming to promote the application and practice of discharge preparation services in aged patients as well as their implementation in China.
