Abstract:
Objective:To explore the application effect of doctor and nurse joint programmed postural sputum evacuation scheme in patients with artificial airway tuberculosis.Methods:By the non-contemporaneous control method, a total of 127 patients with artificial airway tuberculosis who were admitted to the Tuberculosis Intensive Care Unit of the Eighth Medical Center of PLA General Hospital from March 2018 to March 2020 were selected by the convenient sampling. A total of 64 patients admitted from March 2018 to March 2019 were selected as the control group and 63 patients admitted from April 2019 to March 2020 were selected as the experimental group according to their length of stay. Patients in the control group were given routine posture sputum excretion and other measures, while patients in the experimental group were given doctor and nurse joint programmed postural sputum evacuation scheme based on routine posture sputum excretion care. The daily sputum output at 24 hours after the intervention, as well as the heart rate, mean arterial pressure, respiratory rate, blood oxygen saturation, arterial oxygen partial pressure, oxygenation index and arterial CO 2 partial pressure before and after the intervention were compared between the two groups. The sputum excretion effect and other indexes of the two groups were recorded after intervention for 7 days. Results:After 24 hours of intervention, the daily sputum excretion of the experimental group was (28.60±8.15) ml, which was more than (16.02±5.68) ml of the control group, and the difference was statistically significant ( P<0.01) . At 24 h after intervention, the respiratory rate of the experimental group was lower than that of the control group, and the blood oxygen saturation was higher than that of the control group, and the differences were statistically significant ( P<0.01) . There was no statistically significant difference in heart rate and mean arterial pressure at 24 h after intervention between the two groups ( P>0.05) . After 24 hours of intervention, the partial pressure of arterial blood CO 2 in the experimental group was lower than that of the control group, and the arterial blood oxygen partial pressure and oxygenation index were higher than those of the control group, and the differences were statistically significant ( P<0.01) . The total effective rate of sputum excretion in the experimental group was 84.12% (53/63) , which was higher than 59.38% in the control group (38/64) , and the difference was statistically significant ( P<0.01) . Conclusions:Application of doctor and nurse joint programmed postural sputum evacuation scheme can promote the effective sputum discharge in patients with artificial airway tuberculosis, improve patients' respiratory function and improve the therapeutic effect. The curative effect of it is better than the routine sputum excretion measures, which is worthy of clinical promotion.
