Value of magnetic resonance image compilation sequence in quantitative diagnosis of osteonecrosis of femoral head

Abstract:
Objective To investigate the value of magnetic resonance image compilation(MAGiC)sequences for diagnosing quantitatively osteonecrosis of the femoral head.Methods The hip joint MAGiC images of 57 femoral head osteonecrosis patients confirmed by clinical history,symptoms and conventional imaging in some hospital from February 2022 to August 2023 were analyzed retrospectively.The Ti,T2 and proton density(PD)values of the necrotic areas of the femoral heads and the bone marrow edema areas of the femoral necks were measured and compared with the normal values of corresponding areas,and ROC curves were plotted for the parameters with statistically significant differences to evaluate its diagnostic efficacy for the femoral head osteonecrosis and femoral neck bone marrow edema;the differences in the Ti,T2 and PD values of the patients with different ARCO stages were compared.SPSS 26.0 software was used for data analysis.Results The T1 value of the necrotic area was significantly higher than that of the normal femoral head,the T2 and PD values were significantly lower,and the differences were statistically significant(P<0.001).The T1 value of the edema area was significantly higher than that of the normal femoral neck,the PD value was significantly lower,and the differences were statistically significant(P<0.001).The T2 and PD values of the necrotic area were significantly lower than those of the edema area,and the differences were statistically significant(P<0.001).The AUCs of the T1,T2 and PD in the necrotic area for diagnosing femoral head osteonecrosis were 1.000,0.947 and 0.914,with sensitivities of 100.0%,89.2%and 92.8%and specificities of 100.0%,96.7%and 76.7%,respectively,and the three quantitative values proved to have high diagnosis efficacy.The AUCs of the T1 and PD in the edema area for diagnosing bone marrow edema were 0.969 and 0.797,with sensitivities of 90.4%and 86.5%and specificities of 100.0%and 60.0%,respectively,and the T,showed high diagnosis efficacy.As the ARCO stage progressed,the T1 value of the necrotic area gradually increased and the T2 and PD values gradually decreased.The edema area had the lowest Ti,T2 and PD values in stage Ⅰ and the highest Ti,T2 and PD values in stage Ⅱ.Conclusion The Ti,T2 and PD of the MAGiC sequences are effective for the diagnosis of femoral head osteonecrosis,the T1 and PD have high efficacy for diagnosing bone marrow edema of the femoral neck,and the T2 and PD gains advantages in differentiating osteonecrosis of the femoral head from bone marrow edema of the femoral neck.MAGiC sequences can provide objective references for quantitative diagnosis of femoral head osteonecrosis.[Chinese Medical Equipment Journal,2024,45(3):66-70]
