Abstract:
The importance ofdischarge readiness in individual hospital discharge decision-making and health outcomes has been widely recognized. This paper reviews the concept of discharge readiness, assessment objects, implementers, contents, assessment tools and influences of social factors on discharge readiness, in order to provide reference and guidance for promoting targeted interventions to improve patients＇ discharge readiness from the societal level.
