Abstract:
Constructing a transitional care model suitable for patients after percutaneous coronary intervention (PCI) can promote patients' rehabilitation and improve their quality of life. This paper summarized the concept and significance of transitional care, the form and content of transitional care for patients after PCI, and put forward the prospect for the development of transitional care in the future.
