Construction of an evaluation index system of nurse-led discharge planning core competence for elderly patients

Abstract:
Objective? To construct an evaluation index system of nurse-led discharge planning core competence for elderly patients suitable for our country. Methods? Based on the theoretical framework of four stages of discharge planning and combined the physical, psychological and social characteristics of the aged, we constructed the evaluation index system and determined the index weight with the methods of literature review, subject group collective review, Delphi and rank-sum ratio. Results? The recovery rates of questionnaires all were 100% in two rounds of expert consultation with 0.88 and 0.93 for the expert authority coefficients, and with 0.182 and 0.267 for the Kendall W (P＜ 0.01). By two rounds of expert consultation, we established the evaluation index system which included 4 first-level indexes, 13 second-level indexes of capacity characteristic and 57 third-level indexes. Conclusions? The evaluation index system of nurse-led discharge planning core competence for elderly patients built according to discharge planning procedure has specific and comprehensive contents. Besides, the evaluation index system fully takes nursing demands of the elderly patients into account which could provide a basis for evaluation and training for nurses' core competence on discharge planning of the elderly patients.
