Abstract:
PeritoneaL diaLysis is a successfuL renaL repLacement therapy for end-stage kidney faiLure patients. TransitionaL care can reduce the incidence of peritonitis of peritoneaL diaLysis patients and improve patients' quaLity of Life. This paper reviewed the methods and evaLuation tooLs of transitionaL care among peritoneaL diaLysis patients both here and abroad so as to provide a reference for hospitaLs and communities to spread transitionaL care of peritoneaL diaLysis patients.
