Abstract: This paper shows the concept and current research situation of heart failure readmission, and transitional care providers. It focuses on eight transitional care models which can be applied to decrease the readmission for heart failure patients, and puts forward current challenges, so as to provide references for applying transitional care to reduce heart failure readmission in China.
