Comparison between insulin pump therapy and conventional subcutaneous insulin infusion therapy for gestational diabetes mellitus:a systematic review

Abstract:
Objective To systematically review the effect of insulin pump therapy in gestational diabetes.Methods Randomized control trials ( RCTs) related to insulin pump therapy in gestational diabetes were searched in databases such as Chinese National Knowledge Internet ( CNKI), VIP, Wanfang Data, Sinomed, PubMed and the Cochrane Library ( Issue 3, 2016) from the date of establishment to March 2016. Two reviewers screened the literature according to the inclusion and exclusion criteria, extracted the data and assessed the quality of included studies independently. Meta-analysis was performed by RevMan 5.3 software, or only descriptive analysis was conducted instead. Results A total of 13 studies involving 693 patients were included. The results of meta-analysis descriptive analysis showed that the insulin pump therapy group was superior to the conventional subcutaneous insulin therapy group in reducing fasting blood glucose, 2-hour postprandial blood glucose, glycosylated hemoglobin, the time to achieve normal blood glucose, insulin dosage and the incidence of hypoglycemia, with statistically significant differences.Conclusions Insulin pump therapy can reduce the level of blood glucose for the patients with gestational diabetes, and it is also beneficial in reducing the incidence of hypoglycemia. However, because of the limitation of quantity and quality of the included studies, the conclusion should be further confirmed by conducting more large-scale and high quality RCTs.
