Abstract:
Objective To investigate the effect of transitional care on diabetic patients with high risk of diabetic foot .Methods A total of 104 diabetic patients with high risk of diabetic foot were randomly divided into two groups.The observation group (52 cases) received traditional care, and the control group (52 cases) was given the routine health education .Some indexes of two groups were compared , such as foot care knowledge and behaviors, self-management ability, fasting blood glucose, glycosylated hemoglobin (HbA1c).Results After 3 months of nursing care , the scores of foot care knowledge and self-management ability in the observation group were (79.54 ±10.35) and (114.21 ±10.98), and the fasting blood glucose and HbA1c were (7.29 ± 2.02)mmol/L and (7.56 ±0.96)%, and were (65.35 ±11.40),(90.13 ±14.21),(8.22 ±2.18) mmol/L, (8.13 ±1.03)%in the control group.The differences were statistically significant between groups (t=7.617 4, 8.807 6, 2.654 2, 5.635 9, respectively;P<0.01).Conclusions The effect on patients with high risk of diabetic foot is better in transitional care than that in routine health education .The former has advantages in improving their self-management activities and disease control abilities .
